The IASLC Lung Cancer Staging Project: A Renewed Call to Participation
Over the past two decades, the International Association for the Study of Lung Cancer (IASLC) Staging Project has been a steady source of evidence-based recommendations for the TNM classification for lung cancer published by the Union for International Cancer Control and the American Joint Committee on Cancer. The Staging and Prognostic Factors Committee of the IASLC is now issuing a call for participation in the next phase of the project, which is designed to inform the ninth edition of the TNM classification for lung cancer. Following the case recruitment model for the eighth edition database, volunteer site participants are asked to submit data on patients whose lung cancer was diagnosed between January 1, 2011, and December 31, 2019, to the project by means of a secure, electronic data capture system provided by Cancer Research And Biostatistics in Seattle, Washington. Alternatively, participants may transfer existing data sets. The continued success of the IASLC Staging Project in achieving its objectives will depend on the extent of international participation, the degree to which cases are entered directly into the electronic data capture system, and how closely externally submitted cases conform to the data elements for the project
Edema pulmonar pós-pneumonectomia Postpneumonectomy pulmonary edema
A pneumonectomia, embora seja tecnicamente simples, está associada a alta incidência de complicações (cerca de 60%). As complicações respiratórias correspondem a aproximadamente 15% deste total. A mortalidade global dessa cirurgia é de 8,6%, mas em presença de complicações respiratórias, a taxa de mortalidade chega a 30%. O edema pulmonar pós-pneumonectomia é uma complicação rara (3% a 5%), mas muito grave, sendo fatal na maioria dos casos. Foi descrito pela primeira vez há pouco mais de vinte anos mas, apesar da gravidade alarmante, pouco sabemos acerca de sua fisiopatologia, embora muitas hipóteses tenham sido levantadas. Uma vez instalado, nenhuma medida é comprovadamente eficaz no seu tratamento. Vários fatores de risco estão associados ao aparecimento do edema pulmonar pós-pneumonectomia, dentre os quais a sobrecarga hídrica, que foi o primeiro fator evitado. Entretanto, muitos trabalhos mostram não haver relação direta entre o volume recebido e o desenvolvimento do edema. A prevenção é a melhor forma de evitá-lo e deve ser realizada de maneira multifatorial, envolvendo toda a equipe médica, desde o momento da anestesia até os cuidados cirúrgicos e na terapia intensiva. No entanto, tão importante quanto a prevenção, é a suspeita clínica precoce, identificando os pacientes em risco para essa grave complicação.Although pneumonectomy is a technically simple procedure, it has been associated with a high (60%) incidence of complications. Respiratory complications account for approximately 15% of such complications. Worldwide, the mortality rate among patients subjected to pneumonectomy is 8.6%. However, the rate among patients developing respiratory complications is 30%. Although postpneumonectomy pulmonary edema is rare (occurring in 3% to 5% of cases), it is a serious complication and is almost always fatal. It was first described twenty years ago and, despite these alarming statistics, little is known regarding the physiopathology of postpneumonectomy pulmonary edema. Once it has become entrenched, the condition is difficult to treat, and there is no proven efficient treatment. Various risk factors have been correlated with postpneumonectomy pulmonary edema onset. Fluid overload was the first to be identified as a factor to avoid. However, many studies have shown that there is no direct correlation between fluid overload and the development of edema. Prevention is the best way to avoid postpneumonectomy pulmonary edema and must be performed in a multidisciplinary fashion, involving the entire medical staff - from the initiation of anesthesia through the surgical procedure and extending into the critical care management phase. Equally important is early identification and testing of patients at risk for postpneumonectomy pulmonary edema when there is clinical suspicion of this serious complication

Previous issue date: 2002Seqüestro pulmonar é uma anomalia congênita que envolve parênquima e vascularização pulmonar e apresenta-se como extralobar ou intralobar. Objetivo: Descrever os casos de seqüestro pulmonar tratados no InCor e Hospital das Clínicas da FMUSP no período de 1987 a 1996. Método: Análise retrospectiva dos prontuários. Resultados: Foram tratados nove pacientes, sendo quatro mulheres e cinco homens; duas crianças e sete adultos. Infecção respiratória de repetição e hemoptise foram achados clínicos freqüentes nesses pacientes. Todos os casos eram intralobares. A principal localização foi no lobo inferior esquerdo (66%). Apenas um diagnóstico foi intra-operatório. Nos outros oito casos, o diagnóstico foi suspeitado pela radiografia de tórax (100%) e confirmado pela arteriografia (77%) e/ou tomografia computadorizada (66%). Lobectomia (77%) foi o principal tratamento cirúrgico, com baixa morbidade pós-operatória e sem mortalidade. Exame anatomopatológico foi realizado em sete casos e confirmou a doença. Conclusões: O seqüestro pulmonar é uma entidade incomum, em que a tomografia computadorizada e a arteriografia são os exames que mais informações oferecem para um diagnóstico definitivo e seguro. A ressecção do tecido envolvido leva a excelentes resultados.Background: Pulmonary sequestration is a congenital anomaly involving the pulmonary parenchyma and its vascularization, and it presents as extralobar or intralobar. Aim: Describe the cases treated at the Institute of Heart (InCor) and Clinical Hospital of the Medical School of the University of São Paulo (FMUSP). Method: Retrospective analyses of the files of the patients treated over the 10-year period taking demographic, clinical and laboratorial data. Results: The patients were recurrent pulmonary infection and hemoptysis were frequent clinical findings in these patients. All cases were intralobar. The main localization was the left lower lobe (66%). Only one diagnosis was made intra-operatively. In the other eight cases, diagnosis was suggested on chest radiographs (100%) and confirmed by angiography (77%) and computed tomography (66%). Lobectomy (77%) was the most common surgical treatment, with low post-operative morbidity, and no mortality. Anatomic and histologic examination was performed in seven cases and confirmed the pathology. Conclusion: Pulmonary sequestration is an uncommon entity, in which the computed tomography and the angiography are the radiological investigations that provide the most information to a definitive and secure diagnosis. A insuficiência da valva aórtica na dissecção crônica da aorta proximal: troca ou reconstrução valvar? Aortic valve insufficiency in chronic aortic dissection: valve replacement or reconstruction?
Com o objetivo de avaliar os resultados clínicos e ecocardiográficos tardios obtidos com a correção da insuficiência aórtica decorrente da dissecção crônica da aorta proximal, foram estudados 48 pacientes consecutivos operados entre janeiro de 1980 e dezembro de 1989, separados em 2 grupos de 24 pacientes cada. Grupo A - pacientes nos quais a valva aórtica foi preservada pela "resuspensão comissural"; Grupo B - pacientes nos quais a valva aórtica foi substituída. Na avaliação ecocardiográfica pré-operatória, os pacientes do Grupo B apresentavam grau de insuficiência aórtica, diâmetros ventriculares (sistólico e diastólico) e da aorta ascendente significativamente maiores do que os do Grupo A (p=0,03), sendo comparáveis nos demais parâmetros. A mortalidade hospitalar foi 12,5% no Grupo A e de 4,17% no Grupo B e a sobrevida aos 7 anos, respectivamente, 75,75% ± 9,82% e 82,72% ± 7,87% (NS). A avaliação clínica mostrou que, no pós-operatório, houve melhora significativa (pFrom January 1980 to December 1989, 48 consecutive patients with chronic aortic dissection and aortic valve insufficiency underwent operation. They were analyzed in 2 groups (24 patients each) to evaluate the late clinical and echocardiography outcome of those in whom the aortic valve was preserved (Group A) compared with those having valve replacement (Group B). The overall preoperative characteristics of the two groups were similar except for the age (Group A 47.9 ± 10.8 versus Group B 40.2 ± 9.5 years, p=0.03). In the preoperative echocardiographic evaluation, Group B patients had significantly higher aortic root and left ventricle systolic and diastolic diameters (p<0.03), and aortic insufficiency grade (p=0.02). The hospital mortality was 12.5% in Group A and 4.2% in Group B and the seven years actuarial survival rate was respectively 75.7% ± 9.8% and 82.7% ± 7.9%. Postoperatively there was, in both groups, a significant improvement in all clinical parameters evaluated (dyspnea, chest pain, arterial hypertension, aortic insufficiency murmurs (p<0.001). There was also a marked reduction in the aortic root and left ventricle diameters in all patients compared to preoperative values (p<0.05) without statistical difference between the groups. Echocardiographic mild aortic insufficiency remained in 20% of the patients in whom the valve was preserved, without long term hemodynamic or clinical repercussion. Hence, we conclude that aortic insufficiency in chronic dissection can be equally treated with both techniques. Aortic valve preservation should, whenever possible, be attempted and considered as the procedure of choice
