    Restoration of former agricultural fields on acid sandy soils:Conversion to heathland, rangeland or forest?

    Three restoration strategies on agricultural fields with acid sandy soil were evaluated after 18 and 25 years: conversion to heathland, rangeland and forest. Changes in soil microstructure, chemical characteristics, availability of N and P, and vegetation composition were analyzed in agricultural soils, three undisturbed reference sites and five types of restored former agricultural fields. Agricultural soils were characterized by organic slurry without much soil life. Soil nutrients were especially high for P, mostly in mineral form, and P-desorption rates were high. Partial and complete topsoil removal, aiming at heathland restoration, led to (much) lower soil organic matter and nutrients, but not to recovery of soil life, nor to P-limited soils. Heather was accompanied by many grassland species, even with complete topsoil removal. Conversion to rangeland did not decrease nutrient stocks, but led to improved soil life, although different from reference grasslands due to the higher pH. P-availability remained high, but net N-mineralization and plant N-content were clearly lower after 25 than after 18 years. Plant diversity was relatively high, and cover of eutrophic grasses decreased to 8–39% in intermediate and productive rangelands. Nutrient-poor species remained absent, but the slightly higher pH improved conditions for many grassland herbs. Afforestation did also not lead to nutrient-poor conditions, but soil life clearly increased and nutrients were used for rapid tree growth. Undergrowth species however remained eutrophic. It was impossible to retrieve the P-limited reference ecosystems within 25 years of restoration, not even with complete topsoil removal. Differences in plant diversity between expensive topsoil removal and much cheaper conversion to rangeland were also relatively small. For restoration on a landscape scale, it may thus be better to focus on conversion to semi-natural grasslands and afforestation. The half-open, nutrient-, mineral-, and species-rich landscape offers opportunities for large grazers, but also for many insects and birds

    Recovery and the use of postoperative physical therapy after total hip or knee replacement

    BACKGROUND: Total hip or knee arthroplasties (THA/TKA) show favorable long-term effects, yet the recovery process may take weeks to months. Physical therapy (PT) following discharge from hospital is an effective intervention to enhance this recovery process. To investigate the relation between recovery and postoperative PT usage, including the presence of comorbidities, 6 months after THA/TKA. METHODS: Multicenter, observational study in primary THA/TKA patients who completed preoperative and 6 months postoperative assessments. The assessments included questions on PT use (yes/no and duration; long term use defined as ≥ 12 weeks), comorbidities (musculoskeletal, non-musculoskeletal, sensory comorbidities and frequency of comorbidities). Recovery was assessed with the HOOS/KOOS on all 5 subdomains. Logistic regression with long term PT as outcome was performed adjusted for confounding including an interaction term (comorbidity*HOOS/KOOS-subdomain). RESULTS: In total, 1289 THA and 1333 TKA patients were included, of whom 95% received postoperative PT, 56% and 67% received postoperative PT ≥ 12 weeks respectively. In both THA and TKA group, less improvement on all HOOS/KOOS domain scores was associated with ≥ 12 weeks of postoperative PT (range Odds Ratios 0.97–0.99). In the THA group the impact of recovery was smaller in patient with comorbidities as non- musculoskeletal comorbidities modified all associations between recovery and postoperative PT duration (Odds Ratios range 1.01–1.05). Musculoskeletal comorbidities modified the associations between Function-in-Daily-Living-and Sport-and-recreation recovery and postoperative PT. Sensory comorbidities only had an effect on Sport-and-recreation recovery and postoperative PT. Also the frequency of comorbidities modified the relation between Function-in-Daily-Living, pain and symptoms recovery and postoperative PT. In the TKA group comorbidity did not modify the associations. CONCLUSION: Worse recovery was associated with longer duration of postoperative PT suggesting that PT provision is in line with patients’ needs. The impact of physical recovery on the use of long-term postoperative PT was smaller in THA patients with comorbidities. TRIAL REGISTRATION: Registered in the Dutch Trial Registry on March 13, 2012.     Transparency and (no) more in the Political Advertising Regulation

    The EU has taken its first steps into a sensitive space by proposing a new Regulation on Political Advertising (RPA). Simply put, the RPA does two things, which this commentary will address in turn. First, it replaces national laws on the transparency of political advertising with a single set of rules. These provide progressively more information to citizens who see an ad, to the public through ad libraries, and to regulators and private actors who are authorised to request information. Second, the RPA tightens the GDPR’s ban on using sensitive data for targeted political advertising. It leaves member states free, however, to further regulate the use of political advertising.The RPA takes a number of important steps in political advertising law. It strengthens the transparency of the (so far largely unregulated) online political advertising environment. It expands ad libraries with information on targeting and funding. And it allows a broad range of private actors (including civil society and journalists) to request data from a broad range of companies (including ad agencies and small platforms). At the same time, the RPA not only represents the EU’s most significant effort to address concerns about political advertising’s democratic impact, but (because it fully harmonises transparency) also shapes how individuals, researchers, and national regulators can scrutinise political advertising. It is therefore important to determine whether the regulation lives up to the Commission’s hype

    Can we predict the clinical outcome of arthroscopic partial meniscectomy? A systematic review

    NHS-Prospero registration number 42016048592 Objective In order to make a more evidence-based selection of patients who would benefit the most from arthroscopic partial meniscectomy (APM), knowledge of prognostic factors is essential. We conducted a systematic review of predictors for the clinical outcome following APM. Design Systematic review Data sources Medline, Embase, Cochrane Central Register, Web of Science, SPORTDiscus, PubMed Publisher, Google Scholar Inclusion criteria Report an association between factor(s) and clinical outcome; validated questionnaire; follow-up >1 year. Exclusion criteria 1 year) are associated with worse clinical outcome following APM. In addition, resecting >50% of meniscal tissue and leaving a non-intact meniscal rim after meniscectomy are intra-articular predictive factors for worse clinical outcome. Moderate evidence was found that sex, onset of symptoms (acute or chronic), tear type or preoperative sport level are not predictors for clinical outcome. Conflicting evidence was found for the prognostic value of age, perioperative chondral damage, body mass index and leg alignment. Summary/conclusion Long duration of symptoms (>1 year), radiological knee osteoarthritis and resecting >50% of meniscus are associated with a worse clinical outcome following APM. These prognostic factors should be considered in clinical decision making for patients with meniscal tears

    Adverse events and survival after closing- and opening-wedge high tibial osteotomy: a comparative study of 412 patients

    Purpose: Varus medial knee osteoarthritis (OA) can be treated with a closing-wedge (CW) or opening-wedge (OW) high tibial osteotomy (HTO). Little is known about the adverse event (AE) rate of these techniques. The purpose of this study was to examine the AE rate and survival rate of a consecutive series of 412 patients undergoing CW- or OW-HTO. Methods: Medical records were retrospectively screened, and all patients who underwent HTO from 1993 to 2012 at the Erasmus University Medical Centre were assessed with a self-administered questionnaire. Patients filled in the intermittent and constant osteoarthritis pain score, knee injury and osteoarthritis outcome score, and a general questionnaire focusing on AE. Results: Medical records of 412 patients (354 CW- and 112 OW-HTOs) were screened. Of the 358 eligible patients, 291 (81 %) returned their questionnaire. A total of 80 AE (17 %) were found in 466 osteotomies. In the CW-group, 47 (13 %) serious adverse events (SAE) and 2 (0.6 %) AE were found. In the OW-group, 17 (15 %) SAE and 14 (13 %) AE were found. The most common AE was in 14 (4 %) patients of the CW-group sensory palsy of the common peroneal nerve. The most common AE in the OW-group was persistent pain at the iliac crest [11 (9.8 %) patients]. Hardware was removed in 48 % of the CW-osteotomies and 71 % of the OW-osteotomies (p < 0.05). The probability of survival was 75 % after 10 years in the CW-group versus 90 % in the OW-group (p < 0.05). In both groups, an equal number of patients were “in need for prosthesis” according to OARSI criteria. Conclusion: OW-HTO was associated with more AE than CW-HTO. OW-HTO resulted in better survival than CW-HTO. However, in both groups an equal number of patients were in need for prosthesis. Level of evidence: Retrospective comparative study, Level III

    The effect of glucosamine sulphate on osteoarthritis: design of a long-term randomised clinical trial [ISRCTN54513166]

    BACKGROUND: Pharmacological treatment for osteoarthritis (OA) can be divided into two groups: symptom-modifying drugs and disease-modifying drugs. Symptom-modifying drugs are currently the prescription of choice for patients with OA, as disease-modifying drugs are not yet available in usual care. However, there has recently been a lot of debate about glucosamine sulphate (GS), a biological agent that is thought to have both symptom-modifying and disease-modifying properties. This assumption has yet to be proved. The objective of this article is to present the design of a blind randomised clinical trial that examines the long-term symptom-modifying and disease-modifying effectiveness of GS in patients with hip OA. This trial is ongoing and will finish in March 2006. METHODS/DESIGN: Patients with hip OA meeting the ACR-criteria are randomly allocated to either 1500 mg of oral GS or placebo for the duration of two years. The primary outcome measures, which are joint space narrowing (JSN), and change in the pain and function score of the Western Ontario McMaster Universities Osteoarthritis index (WOMAC), are determined at baseline and after two years of follow-up during the final assessment. Intermediate measures at three-month intervals throughout the trial are used to study secondary outcome measures. Secondary outcome measures are changes in WOMAC stiffness score, quality of life, medical consumption, side effects and differences in biomarker CTX-II

    Varus inclination of the proximal tibia or the distal femur does not influence high tibial osteotomy outcome

    We have analysed retrospectively the influence of different sources of knee deformity on failure of closing wedge high tibial valgus osteotomy (HTO). Preoperative frontal plane varus deformities of the lower extremity, distal femur and proximal tibia, and medial convergence of the knee joint line were assessed on a standard whole leg radiograph in 76 patients. Using the logistic regression model, the probability of survival for HTO was 77% (SD 4%) at 10-years follow-up. Varus deformity of the lower extremity ( 3 degrees ) were identified as preoperative risk factors for conversion to arthroplasty (P = 0.03 and P = 0.006). We found no evidence that varus inclination of the proximal tibia or distal femur influences long-term survival of HTO
