    Omental Patch Technique for the Ileal Perforation Secondary to Typhoid Fever

    Background/Aim: Enteric perforation is a grave complication of typhoid fever. Laparotomy with primary closure is the treatment of choice depending upon the bowel condition. Fecal fistula formation is the main concern in primary closure and the incidence of this complication dramatically decreases when omentum is used as a patch over primary closure. Materials and Methods: A total of 176 patients underwent laparotomy for enteric perforation and they were divided into two groups randomly; Group I-Primary closure with omental patch and Group II--Only primary closure. The outcomes were measured in relation to hospital stay, wound infection, septicemia, fecal fistula, and mortality. Results: The incidence of complications including fecal fistula and mortality is significantly lower in the group I patients. Fecal fistula occurs in 7.7% in group II, while in only 1.1% in group I. The mortality is also lower 3.3% in group II, while 1.1% in group I. Conclusion: Primary closure with omental patch is a better option as compared with only primary closure in enteric perforation patients. It can be recommended as an alternative method to primary closure only in enteric perforation patients

    Therapeutic platelet reduction: Use in postsplenectomy thrombocytosis

    Therapeutic platelet reduction is an effective modality for the reduction of platelet count in patients with treatment of extreme thrombocytosis resulting from a variety of primary and secondary causes of thrombocytosis, which may be associated with thrombotic or hemorrhagic complications of varying degrees. These cases when symptomatic fall into the ASFA Category II indication for therapeutic platelet apheresis procedure. Here, we report a case of postsplenectomy secondary thrombocytosis presenting with extremely high platelet counts and subsequent thrombosis in the shunt and successful treatment after therapeutic platelet reduction. The case is being presented to bring forth the fact that therapeutic platelet reduction is an easy procedure that gives quick and good results and also to bring to the attention of transfusion specialists an associated but as yet unreported procedural finding

    A Comparative Study of Land Cover Classification by Using Multispectral and Texture Data

    The main objective of this study is to find out the importance of machine vision approach for the classification of five types of land cover data such as bare land, desert rangeland, green pasture, fertile cultivated land, and Sutlej river land. A novel spectra-statistical framework is designed to classify the subjective land cover data types accurately. Multispectral data of these land covers were acquired by using a handheld device named multispectral radiometer in the form of five spectral bands (blue, green, red, near infrared, and shortwave infrared) while texture data were acquired with a digital camera by the transformation of acquired images into 229 texture features for each image. The most discriminant 30 features of each image were obtained by integrating the three statistical features selection techniques such as Fisher, Probability of Error plus Average Correlation, and Mutual Information (F + PA + MI). Selected texture data clustering was verified by nonlinear discriminant analysis while linear discriminant analysis approach was applied for multispectral data. For classification, the texture and multispectral data were deployed to artificial neural network (ANN: n-class). By implementing a cross validation method (80-20), we received an accuracy of 91.332% for texture data and 96.40% for multispectral data, respectively

    Regional climate assessment of precipitation and temperature in Southern Punjab (Pakistan) using SimCLIM climate model for different temporal scales

    Unbalanced climate during the last decades has created spatially alarming and destructive situations in the world. Anomalies in temperature and precipitation enhance the risks for crop production in large agricultural region (especially the Southern Punjab) of Pakistan. Detailed analysis of historic weather data (1980–2011) record helped in creating baseline data to compare with model projection (SimCLIM) for regional level. Ensemble of 40 GCMs used for climatic projections with greenhouse gas (GHG) representative concentration pathways (RCP-4.5, 6.0, 8.5) was selected on the baseline comparison and used for 2025 and 2050 climate projection. Precipitation projected by ensemble and regional weather observatory at baseline showed highly unpredictable nature while both temperature extremes showed 95 % confidence level on a monthly projection. Percentage change in precipitation projected by model with RCP-4.5, RCP-6.0, and RCP-8.5 showed uncertainty 3.3 to 5.6 %, 2.9 to 5.2 %, and 3.6 to 7.9 % for 2025 and 2050, respectively. Percentage change of minimum temperature from base temperature showed that 5.1, 4.7, and 5.8 % for 2025 and 9.0, 8.1, and 12.0 % increase for projection year 2050 with RCP-4.5, 6.0, and 8.5 and maximum temperature 2.7, 2.5, and 3.0 % for 2025 and 4.7, 4.4, and 6.4 % for 2050 will be increased with RCP-4.5, 6.0, and 8.5, respectively. Uneven increase in precipitation and asymmetric increase in temperature extremes in future would also increase the risk associated with management of climatic uncertainties. Future climate projection will enable us for better risk management decisions
