    Molecular mechanisms of the regulation of ATPase cycle in striated muscle

    New data on the molecular mechanism of the regulation of ATPase cycle by troponin-tropomyosin system have been obtained in reconstructed muscle fibers by using the polarized fluorescence technique, which allowed us following the azimuthal movements of tropomyosin, actin subdomain-1 and myosin SH1 helix motor domain during the sequential steps of ATPase cycle. We found that tropomyosin strands "rolling" on thin filament surface from periphery to center at ATPase cycle increases the amplitudes of multistep changes in special arrangement of SH1 helix and subdomain-1 at force generation states. These changes seem to convey to actin monomers and to myosin "lever arm", resulting in enhance of the effectiveness of each cross-bridge work. At high-Ca^2+^ troponin, a shift of tropomyosin strands further to center at strong-binding states increases this effect. At low-Ca^2+^ troponin "freezes" tropomyosin and actin in states typical for weak-binding states, resulting in disturbing the teamwork of actin and myosin

    Effect of prolonged space flight on cardiac function and dimensions

    Echocardiographic studies were performed preflight 5 days before launch and on recovery day and 1, 2, 4, 11, 31 and 68 days postflight. From these echocardiograms measurements were made. From these primary measurements, left ventricular end-diastolic volume, end-systolic volume, stroke volume, and mass were derived using the accepted assumptions. Findings in the Scientist Pilot and Pilot resemble those seen in trained distance runners. Wall thickness measurements were normal in all three crewmembers preflight. Postflight basal studies were unchanged in the Commander on recovery day through 68 days postflight in both the Scientist Pilot and Pilot, however, the left ventricular end-diastolic volume, stroke volume, and mass were decreased slightly. Left ventricular function curves were constructed for the Commander and Pilot by plotting stroke volume versus end-diastolic volume. In both astronauts, preflight and postflight data fell on the same straight line demonstrating that no deterioration in cardiac function had occurred. These data indicate that the cardiovascular system adapts well to prolonged weightlessness and suggest that alterations in cardiac dimensions and function are unlikely to limit man's future in space

    Triple-Vessel Percutaneous Coronary Revascularization In Situs Inversus Dextrocardia

    Dextrocardia with situs inversus occurs in approximately one in 10,000 individuals of whom 20% have primary ciliary dyskinesia inherited as an autosomal recessive trait. These patients have a high incidence of congenital cardiac disease but their risk of coronary artery disease is similar to that of the general population. We report what is, to our knowledge, the first case of total triple-vessel coronary revascularization by percutaneous stent implantation in a 79-year-old woman with situs inversus dextrocardia. We describe the successful use of standard diagnostic and interventional guide catheters with counter rotation and transversely inversed image acquisition techniques. The case also highlights that the right precordial pain may represent cardiac ischemia in this population

    Intranasal administration of RSV antigen-expressing MCMV elicits robust tissue-resident effector and effector memory CD8+ T cells in the lung

    Cytomegalovirus vectors are promising delivery vehicles for vaccine strategies that aim to elicit effector CD8+ T cells. To determine how the route of immunization affects CD8+ T cell responses in the lungs of mice vaccinated with a murine cytomegalovirus vector expressing the respiratory syncytial virus matrix (M) protein, we infected CB6F1 mice via the intranasal or intraperitoneal route and evaluated the M-specific CD8+ T cell response at early and late time points. We found that intranasal vaccination generated robust and durable tissue-resident effector and effector memory CD8+ T cell populations that were undetectable after intraperitoneal vaccination. The generation of these antigen-experienced cells by intranasal vaccination resulted in earlier T cell responses, interferon gamma secretion, and viral clearance after respiratory syncytial virus challenge. Collectively, these findings validate a novel approach to vaccination that emphasizes the route of delivery as a key determinant of immune priming at the site of vulnerability

    Decreasing referrals to transient ischemic attack clinics during the COVID-19 outbreak: results from a multi-centre cross-sectional survey

    Objective. The COVID-19 pandemic is having major implications for stroke care with a documented significant fall in hospital acute stroke admissions. We investigated whether COVID-19 has resulted in a decreased number of referrals to the Transient Ischemic Attack (TIA) clinics across the North West London region. Setting and Design. All the TIA clinical leads of the North West London region received an invitation by email to participate in an online survey in May 2020. The survey questionnaire aimed to assess the number of patients with suspected TIA consecutively referred to each of the TIA clinics of the North West London region between 1st March to 30th April 2020, the COVID-19 period, and between 1st March to 30th April 2019. Results. We had a response rate of 100%. During the COVID-19 period, the TIA clinics of the North West London region received 440 referrals compared to 616 referrals received between 1st March to 30th April 2019 with a fall in the number of the referrals by 28.6%. In April 2020 compared with April 2019, the number of the referrals declined by 40.1%. Conclusions. This multicentre analysis documented a significant reduction in the number of patients referred with suspected TIA to the specialised rapid access outpatient clinics in the North West London region during the COVID-19 pandemic. Future studies are needed to confirm our findings and to better characterise the incidence of cerebrovascular disease during the COVID-19 pandemic

    Re-imagining the futures of geographical thought and praxis

    The question of geography\u27s future has recurred throughout the history of geographical thought, and responses to it often presume a linear trajectory from the past and present to a possible future. Yet one of the major contributions that geographers have made to understanding spatio-temporality is reconceiving both space and time as plural, fluid, and co-constituted through multiple space–time trajectories simultaneously. Amidst the ongoing crises of the present, this article opens the current special issue with a call to pluralize geography\u27s futures by diversifying the voices speaking in the name of ‘geography’ and broadening the horizon of possibilities for the futures of geographical thought and praxis. We have assembled the contributions in this collection with the aim of raising important theoretical, methodological, and empirical questions about how geography\u27s past and present shape the conditions of possibility for its potential futures. In doing so, we seek to demonstrate how the worlding of geography\u27s futures is fundamentally a matter of transforming its disciplinary reproduction in the here-and-now

    Variation in emergency percutaneous coronary intervention in ventilated patients in the UK: insights from a national database

    Aims Pre-procedural ventilation is a marker of high risk in PCI patients. Causes include out-of-hospital cardiac arrest (OHCA) and cardiogenic shock. OHCA occurs in approximately 60,000 patients in the UK per annum. No consensus exists regarding the need/timing of coronary angiography ± revascularization without ST elevation. The aim was to describe the national variation in the rate of emergency PCI in ventilated patients. Methods and Results Using the UK national database for PCI in 2013, we identified all procedures performed as ‘emergency’ or ‘salvage’ for whom ventilation had been initiated before the PCI. Of the 92,589 patients who underwent PCI, 1342 (5.5%) fulfilled those criteria. There was wide variation in practice. There was no demonstrable relationship between the number of emergency PCI patients with pre-procedure ventilation per annum and (i) total number of PPCIs in a unit (r = −0.186), and (ii) availability of 24 h PCI, (iii) on-site surgical cover. Conclusion We demonstrated a wide variation in practice across the UK in rates of pre-procedural ventilation in emergency PCI. The majority of individuals will have suffered an OHCA. In the absence of a plausible explanation for this discrepant practice, it is possible that (a) some patients presenting with OHCA that may benefit from revascularization are being denied treatment and (b) procedures may be being undertaken that are futile. Further prospective data are needed to aid in production of guidelines aiming at standardized care in OHCA

    Dehalogenation of polychlorinated biphenyls and polybrominated diphenyl ethers using a hybrid bioinorganic catalyst

    The environmentally prevalent polybrominated diphenyl ether (PBDE) #47 and polychlorinated biphenyls (PCBs) #28 and #118 were challenged for 24 hours with a novel biomass-supported Pd catalyst (BioPd0). Analysis of the products via GC/MS revealed the BioPd0 to cause the challenged compounds to undergo stepwise dehalogenation with preferential loss of the least sterically hindered halogen atom. A mass balance for PCB #28 showed that it is degraded to three dichlorobiphenyls (33.9 %), two monochlorobiphenyls (12 %), and biphenyl (30.7 %). The remaining mass was starting material. In contrast, while PCB #118 underwent degradation to yield five tetra- and five trichlorinated biphenyls; no less chlorinated products or biphenyl were detected, and the total mass of degraded products was 0.3 %. Although the BioPd0 material was developed for treatment of PCBs, a mass balance for PBDE #47 showed that the biocatalyst could prove a useful method for treatment of PBDEs. Specifically, 10 % of PBDE # 47 was converted to identifiable lower brominated congeners, predominantly the tribrominated BDE 17, and the dibrominated BDE 4, 75 % remained intact, while 15 % of the starting mass was unaccounted for
