    Self-inflicted penetrating eye injuries using a razor blade: Case report

    <p>Abstract</p> <p>Background</p> <p>A 23 year old white male with a history of social and behavioural problems attempted to blind himself chemically, with alcohol, and mechanically, with a razor blade.</p> <p>Methods</p> <p>Observational case report of a patient who self-inflicted bilateral scleral lacerations with a razor blade, after losing his job.</p> <p>Results</p> <p>The patient sustained bilateral inferior scleral perforations, with hypotony and a right traumatic cataract. He received urgent surgical repair, and prophylactic antibiotics. There were no retinal breaks or detachments. He later underwent successful cataract surgery to the right eye.</p> <p>Conclusion</p> <p>Self-inflicted ocular injury may be possible in non-psychotic patients, as a situational response to a life event. Urgent repair can completely restore vision in some cases. Referral for psychiatric counseling is mandatory.</p

    Self-determination theory in ophthalmology education: factors influencing autonomy, competence and relatedness in medical students

    ABSTRACTBackground The affective components of learning, including student motivation, has yet to be thoroughly investigated in undergraduate ophthalmology education. This study aims to use Self-Determination Theory (SDT) as a framework to describe the variations in student perceptions of motivation in studying ophthalmology through their satisfactions of autonomy, competence and relatedness, and to highlight factors that stimulate or hinder this.Methods Penultimate year medical students from a single tertiary educational institution undertaking a clinical placement in ophthalmology participated in in-depth interviews to explore factors affecting their perceptions of motivation in studying ophthalmology. Interviews were transcribed and analysed according to the principles of interpretive phenomenography through the theoretical framework of SDT.Results Of the 39 students invited, 10 agreed to participate. Variations in perceptions of experiences generated the outcome space. Participants experienced either amotivation, external locus extrinsic motivation, internal locus extrinsic motivation and intrinsic motivation (conceptions of the outcome space). This was described with respect to their satisfaction of autonomy, competence and relatedness (dimensions of the outcome space). Additionally, 21 factors that impacted on motivation were identified, of which five over-arching factors impacted all three basic psychological needs – guidance, growth mindset, assessment, curricular pressure and extracurricular pressure.Conclusions The findings of this study provide a unique insight into the motivation of medical students studying ophthalmology. This provides an exciting opportunity for medical educators to address the affective aspect of learning

    Refractive error in Nigerian adults: prevalence, type, and spectacle coverage.

    PURPOSE: To provide data on prevalence and types of refractive error and the spectacle-wearing rate among adults in Nigeria and the degree to which the need for distance correction could be met by off-the-shelf spectacles. METHODS: Multistage, stratified, cluster random sampling with probability proportional to size was used to identify a nationally representative sample of 15,027 persons aged ≥40 years. Distance vision was measured using a reduced logMAR tumbling-E chart. All participants underwent autorefraction, and those with presenting acuity of <6/12 in one or both eyes had their corrected acuity measured and underwent detailed clinical examination to determine the cause. RESULTS: Included in the survey were 13,599 (89.9%) of the 15,122 persons aged ≥40 years who were enumerated. Uncorrected refractive error was responsible for 77.9% of mild visual impairment (<6/12-6/18), 57.1% of moderate visual impairment (<6/18-6/60), 11.3% of severe visual impairment (<6/60-3/60), and 1.4% of blindness (<3/60). The crude prevalence of myopia (≤0.5 D) and high myopia (≤5.0 D) were 16.2% and 2.1%, respectively. Spectacles could improve the vision of 1279 (9.4%) and 882 (6.5%) participants at the 6/12 and 6/18 level, respectively, but only 3.4% and 4.4% of these individuals wore spectacles to the examination site. Approximately 2,140,000 adults in Nigeria would benefit from spectacles that improved their vision from <6/12 to ≥6/12. More than a third of the need could be met by low-cost, off-the-shelf spectacles. CONCLUSIONS: Uncorrected refractive errors are an important cause of visual impairment in Nigeria, and services must be dramatically improved to meet the need

    PURPOSE: To estimate the prevalence and causes of severe visual impairment and blindness among children in Lorestan province of Iran, and to assess the feasibility of the Key Informant Method in this setting. METHODS: Potential cases were identified using the Key Informant Method, in 3 counties of Lorestan province during June through August 2008, and referred for examination. Causes of severe visual impairment/blindness were determined and categorized using standard World Health Organization methods. RESULTS: Of 123 children referred for examination, 27 children were confirmed to have severe visual impairment/blindness or blindness. The median age was11 years (interquartile range 6-13), and 59% were girls. After adjusting for non-attenders, the estimated prevalence of severe visual impairment/blindness was 0.04% (0.03-0.05). The main site of abnormality was retina (44%), followed by disorders of the whole eye (33%). The majority of causes had a hereditary etiology (70%), which was associated with a family history of blindness (P = 0.002). Potentially avoidable causes of severe visual impairment/blindness were found in 14 children (52%). Almost all children with severe visual impairment/blindness had a history of parental consanguinity (93%). CONCLUSIONS: Our findings suggest a moderate prevalence of childhood blindness in the Lorestan province of Iran, a high proportion of which may be avoidable, given improved access to ophthalmic and genetic counselling services in rural areas.
