    A Rare Case of Reversible Encephalopathy Syndrome Accompanying Late Postpartum Eclampsia or Hypertensive Encephalopathy-A Clinical Dilemma

    Posterior Reversible Encephalopathy Syndrome (PRES) refers to a clinic-radiologic diagnosis. Clinically it is characterized by non specific symptoms such as headache, confusion, visual disturbances and seizures. The radiological findings in PRES are thought to be due to vasogenic oedema, predominantly in the posterior cerebral hemispheres, and are reversible with appropriate management. We report a case of reversible encephalopathy diagnosed by MRI scan occurring in atypical areas like the caudate and lentiform nuclei of the brain following an uneventful lower segment caesarean section in a normotensive patient, who was successfully treated with antihypertensives, anticonvulsants and supportive treatment.     India has 22 officially recognized languages: Assamese, Bengali, English, Gujarati, Hindi, Kannada, Kashmiri, Konkani, Malayalam, Manipuri, Marathi, Nepali, Oriya, Punjabi, Sanskrit, Tamil, Telugu, and Urdu. Clearly, India owns the language diversity problem. In the age of Internet, the multiplicity of languages makes it even more necessary to have sophisticated Systems for Natural Language Process. In this paper we are developing the phonetic dictionary for natural language processing particularly for Kannada. Phonetics is the scientific study of speech sounds. Acoustic phonetics studies the physical properties of sounds and provides a language to distinguish one sound from another in quality and quantity. Kannada language is one of the major Dravidian languages of India. The language uses forty nine phonemic letters, divided into three groups: Swaragalu (thirteen letters); Yogavaahakagalu (two letters); and Vyanjanagalu (thirty-four letters), similar to the vowels and consonants of English, respectively

    Ultrasonographic evaluation of abnormal uterine bleeding in postmenopausal women

    Background: Objectives of current study were to diagnose causes of Abnormal Uterine Bleeding (AUB) in postmenopausal women (PMW) and to correlate it with curettage and histopathological findings, hysteroscopy and thereby minimizing unnecessary interventions in the form of operations and hysteroscopy where sonography depicts normal findings.Methods: After obtaining ethical clearance present prospective observational study was conducted from November 2010 to November 2012, to evaluate the endometrium in 50 postmenopausal women (PMW) with bleeding per vagina referred to the department of Radio diagnosis by the department of gynaecology in Bangalore medical college and research institute. After applying inclusion and exclusion criterias the cases were evaluated with ultrasonography both transabdominal (TAS) and transvaginal scan (TVS where ever necessary). Histopathological and hysteroscopic correlation was done in all cases.Results: 58% of the PMW with bleed were in the age group of 51-60 years. Most common cause of PMB was atrophic endometrium (44%), endometrial polyp (22%), followed by malignancy (14%), and hyperplastic endometrium (6%). At Endometrium thickness less than 4 mm there were nil chances of carcinoma.Conclusions: In women with AUB in postmenopausal age ultrasonography (USG) can be considered as an initial imaging modality for diagnosing endometrial diseases. The sensitivity and specificity of USG for Atrophic endometrium is 100% & 84% respectively with accuracy of 100%, endometrial polyp the specificity is 100% with accuracy of 88%. For malignancy USG showed 100% specificity & accuracy of 100%. Hence USG is highly accurate for evaluating endometrial pathologies. Being noninvasive, less costly & good patient compliance USG should be considered as an initial imaging modality over invasive investigations like D&C, hysteroscopy in evaluating endometrial disorders

    Role of diagnostic hysterolaparoscopy in the evaluation of female infertility

    Background: Infertility is defined by WHO as a disease of the reproductive system defined by the failure to achieve a clinical pregnancy after 12 months or more of regular unprotected sexual intercourse. The prevalence of infertility is about 10%-15% of reproductive age couples. Female factor is responsible for 40-45% of etiology of infertility. Aim of the study was to evaluate the role of diagnostic hysterolaparoscopy in female infertility at a tertiary care centre.Methods: This is a retrospective hospital based study done at a tertiary care hospital attached to JSS Medical College, Mysuru between January 2017 and December 2018. Infertile women with primary or secondary infertility in the 18-40 years age group, with normal hormonal profile and no known male factor were included.Results: In our study, primary infertility was found in 77% of the 96 patients and secondary infertility was found in 23% of patients. In primary infertility, ovarian pathology was the most common finding while Endometriosis was the most common finding in secondary infertility group. 77 % patients were found to have bilaterally patent tubes while remaining had unilateral or bilateral blockage. On hysteroscopy, endometrial polyps were the commonest hysteroscopic finding in both groups.Conclusions: Combined hystero-laparoscopy is a safe, effective and reliable method in comprehensive evaluation of infertility. It helps in the diagnosis of pelvic pathology which may have been missed by routine examination and thereby helps in optimal management of female infertility

    Advances in Gene Ontology Utilization Improve Statistical Power of Annotation Enrichment

    Gene-annotation enrichment is a common method for utilizing ontology-based annotations in gene and gene-product centric knowledgebases. Effective utilization of these annotations requires inferring semantic linkages by tracing paths through edges in the ontological graph, referred to as relations. However, some relations are semantically problematic with respect to scope, necessitating their omission or modification lest erroneous term mappings occur. To address these issues, we created the Gene Ontology Categorization Suite, or GOcats—a novel tool that organizes the Gene Ontology into subgraphs representing user-defined concepts, while ensuring that all appropriate relations are congruent with respect to scoping semantics. Here, we demonstrate the improvements in annotation enrichment by re-interpreting edges that would otherwise be omitted by traditional ancestor path-tracing methods. Specifically, we show that GOcats’ unique handling of relations improves enrichment over conventional methods in the analysis of two different gene-expression datasets: a breast cancer microarray dataset and several horse cartilage development RNAseq datasets. With the breast cancer microarray dataset, we observed significant improvement (one-sided binomial test p-value = 1.86E-25) in 182 of 217 significantly enriched GO terms identified from the conventional path traversal method when GOcats’ path traversal was used. We also found new significantly enriched terms using GOcats, whose biological relevancy has been experimentally demonstrated elsewhere. Likewise, on the horse RNAseq datasets, we observed a significant improvement in GO term enrichment when using GOcat’s path traversal: one-sided binomial test p-values range from 1.32E-03 to 2.58E-44

    Revival of Superconductivity by Y3+/Ca2+ substitution in YBa2Cu2.7Co0.3O7 without reported phase transformation

    Results of phase formation, resistivity (r), and thermo-electric power (S), are reported on Y1-xCaxBa2Cu2.7Co0.3O7 compounds with x = 0.1 and 0.2. Pristine compound i.e. without Co or Ca substitution crystallizes in orthorhombic structure with space group P/mmm. Cu-site Co substituted compound i.e. YBa2Cu2.7Co0.3O7 is tetragonal. With simultaneous doping of Ca at Y site in Co substituted compound i.e. Y1-xCaxBa2Cu2.7Co0.3O7 the tetragonal nature still remains. r(T) measurements showed superconducting transition temperature (Tc) to decrease from 90K (YBa2Cu3O7) to 33 K for YBa2Cu2.7Co0.3O7 which with further Ca substitution increases from 33K to 53K (Y0.9 Ca0.1Ba2Cu2.7Co0.3O7) and 67 K for Y0.8 Ca0.2Ba2Cu2.7Co0.3O7. Tc decreases first with Cu-site Co substitution by hole-filling and later recovers by simultaneous hole creation by Y site Ca substitution. Room temperature thermoelectric power S(300 K), which is an indirect measure of mobile carriers shows the decrease of carriers with Co doping and creation by Ca substitution. Our results demonstrate the hole filling by Co substitution is compensated by simultaneous Ca substitution.Comment: 10 pages of TEXT and Fig

    Lifestyle, dietary and treatment adherence pattern of uncontrolled diabetics in coastal Karnataka, India

    Background: Diabetes Mellitus shows a rising trend in India, driven by a combination of factors like sedentary lifestyle, unhealthy diet and tobacco use. The cornerstone for interventions to reduce this is lifestyle modification. Aim & Objective: This study aims to determine lifestyle behaviours among uncontrolled diabetics in rural South India. Settings and Design: This is a pilot study conducted as part of a community trial which enrolled uncontrolled diabetics (Glycosylated haemoglobin, HbA1C of 7% or more) selected from baseline survey of 2 RBS readings. Methods and Material: The sociodemographic details, lifestyle habits and treatment adherence of eligible participants were recorded with a validated questionnaire. Statistical analysis used: Data was compared among 2 groups of poor glycaemic control using Chi square test. Results: There was no significant association of age or gender with HbA1C levels. Majority were non-smokers, non-alcoholics and did not exercise. Higher proportions of those with hospital admissions, longer duration of disease and less frequent check-ups had poor control; but these were not statistically significant. Dietary control was inadequate. However, there were no significant association of dietary habits with poor control. Conclusions: Although overall adherence to medication and follow up was satisfactory, lifestyle modification is not being sufficiently followed

    Acquired heart block: A possible complication of patent ductus arteriosus in a preterm infant

    A large patent ductus arteriosus (PDA) is a frequently encountered clinical problem in extremely low birth weight (ELBW) infants. It leads to an increased pulmonary blood flow and in a decreased or reversed diastolic flow in the systemic circulation, resulting in complications. Here we report a possible complication of PDA not previously published. On day 8 of life, a male ELBW infant (birth weight 650 g) born at a gestational age of 23 weeks and 3 days developed an atrioventricular block (AV block). The heart rate dropped from 168/min to 90/min, and the ECG showed a Wenckebach second-degree AV block and intraventricular conduction disturbances. Echocardiography demonstrated a PDA with a large left-to-right shunt and large left atrium and left ventricle with high contractility. Within several minutes after surgical closure of the PDA, the heart rate increased, and after 30 min the AV block had improved to a 1: 1 conduction ratio. Echocardiography after 2 h revealed a significant decrease of the left ventricular and atrial dimensions. Within 12 h, the AV block completely reversed together with the intraventricular conduction disturbances. We suggest that PDA with a large left-to-right shunt and left ventricular volume overload may lead to an AV block in an ELBW infant. Surgical closure of the PDA may be indicated.     Encryption and Decryption of Images with Pixel Data Modification Using Hand Gesture Passcodes

    To ensure data security and safeguard sensitive information in society, image encryption and decryption as well as pixel data modifications, are essential. To avoid misuse and preserve trust in our digital environment, it is crucial to use these technologies responsibly and ethically. So, to overcome some of the issues, the authors designed a way to modify pixel data that would hold the hidden information. The objective of this work is to change the pixel values in a way that can be used to store information about black and white image pixel data. Prior to encryption and decryption, by using Python we were able to construct a passcode with hand gestures in the air, then encrypt it without any data loss. It concentrates on keeping track of simply two pixel values. Thus, pixel values are slightly changed to ensure the masked image is not misleading. Considering that the RGB values are at their border values of 254, 255 the test cases of masking overcome issues with the corner values susceptibility
