The research study on Vaalai karappan was chosen with an idea that the patient suffering from this disease should have a great relief from it. Relevant Siddha literature and Modern system of Medicine literature were collected correlating to the disease “Vaalai karappan”. Twenty patients from both sex of different age groups were selected and treated with “Pungampoo ennai” internally and Vaalai karappanukku thylum” as external medicine in In-patients ward of post graduate department of Kuzhanthai Maruthuvam. Another 20 patients were treated in the out patient ward. The patients were treated for 20-40 days depending upon the severity of the illness, further follow up was done for 1 to 2 months to check the recurrence. The clinical diagnosis of all the cases of “Vaalaikarappan” were done on the basis of signs and symptoms explained in Siddha texts of Balavagadam, Sirappu Maruthuvam, Noi Naadal and Noi Mudhal Naadal. The efficacies of the drugs “Pungampoo ennai” and “Vaalai karappanukku thylum” were studied and observed during the study. The clinical trial was conducted with 20 patients. Among these 20 patients, fourteen patients (70%) had good relief, four patients (20%) had moderate relief and two patients had mild relief. Contact allergen was observed to be an important etiological factor for the incidence of Vaalai karappan in infants and children. During and after the course of treatment no untoward effects were reported. The potency of the drugs was studied by biochemical analysis in Government Siddha Medical College, Palayamkottai. The biochemical analysis indicates the presence of Calcium, Chloride, Starch, Amino acid in Pungampoo ennai and (Internal medicine) and the presence of Calcium, Chloride, Starch, Iron ferrous, Amino acid in Vaalai karappanukku thylum (External medicine). The pharmacological analysis of the drug “Pungampoo ennai” reveals that it has significant acute and chronic anti inflammatory action and antihistaminic action. The drug “Vaalai karappanku thylum” possesses anti inflammatory action externally. These analysis ensure the efficacies of the trial drugs which were proved clinically. CONCLUSION: Good results were obtained when the internal medicine “Pungam poo ennai” and the external medicine “Vaalai karappanukku thylum” were administered to the patients who were admitted in In- patients ward. Based on the finding of the research 70% patients were completely cured and was also proved clinically. There was no recurrence of the symptoms found. There were no untoward side effects of the drugs which were administered internally and externally. Cost of the drug is cheep and the availability of the drugs is also easy
