    A Social Constructionist Approach to the Facilitation of Professional Development among Community College Faculty

    My purpose in conducting this research project was to engage in collaborative action research with a group of faculty members in order to learn more about my own facilitation of collaborative learning and to identify ways in which I could improve. The participants and I used dialogue during our group meetings to help each other with the development and implementation of Classroom Assessment Techniques in the classroom and the review of the results. The structure for the semester-long project was the development of a Classroom Assessment Techniques seminar handbook for future use with other faculty in the college. The data analysis focused in two areas: 1) describing what the experience was like for the participants, and 2) describing my own experience. Themes emerged from the hermeneutic analysis of data about the experience of participants: the faculty participants engaged because they had an “interest in making changes” in their practices; they had an appreciation for the diverse backgrounds of fellow participants – “what people bring to the table.” In their descriptions of the collaborative processes of the project, they described their experience as “figuring it out together” and characterized the interactions with the group as a “shot in the arm.” Participants also described several aspects of “just having the time,” related to the need for dedicated time to discuss teaching and learning; feeling there wasn’t always enough time to engage in collaborative learning; differing preferences for the best time of day to meet; the fact that collaborative learning processes have to “build over time;” and an observation about the tension between allowing the time for social construction and dialogue versus the desire for the efficient use of time preferred by busy people. Project participants described not only changes in their knowledge through learning new content about Classroom Assessment Techniques but also described personal and professional changes such as a shift to a student- centered approach to teaching. They described their experience of the facilitator as a “guide from the side,” who facilitated with flexibility, intentionality, and mutuality and stressed the need for relationships and a safe environment for collaborative learning to occur

    Restoring Functional Status: A Long-Term Case Report of Severe Lung and Ventilatory Muscle Pump Dysfunction Involving Recurrent Bacterial Pneumonias

    Background and Purpose: Prolonged mechanical ventilation contributes to immobility and deconditioning making efforts to safely discontinue ventilator support desirable. This case report documents how implementing physical therapy treatment interventions, based on the Guide to Physical Therapist Practice, can help to restore a person’s functional status even after multiple years of mechanical ventilation dependency. Case Description: A patient (female; aged 63 years) with severe restrictive and obstructive ventilatory impairment has survived 34 recurrent pneumonias involving 6 bacterial pathogens while being mechanically ventilated at home. A 3-year study was approved and informed consent obtained for a home exercise program of resistive extremity and inspiratory muscle training along with exercise reconditioning. Tolerable distances walked, maximal inspiratory and expiratory pressures, hours spent on versus off mechanical ventilation, activities performed within and around her home, and community excursions taken were charted. Outcomes: Daily time tolerated off the ventilator improved from less than one to 12 hours, distance walked in 6 minutes increased 33%, and maximal inspiratory and expiratory pressures improved 62% and 9.6% respectively. These improvements made out-of-home social excursions possible. Discussion and Conclusions: This patient’s functional status improved following multiple physical therapy interventions dictated by the evaluation of initial physical therapy examination findings according to the Guide to Physical Therapist Practice. Long term mechanical ventilator dependency in the home environment did not exclude this patient from achieving clinically significant gains in functional status even when having severe restrictive and obstructive ventilator impairment

    In this study, we sought to confirm the theoretical framework underlying an Online Graduate Mentoring Scale by establishing the scale’s factorial validity and reliability. Analysis of data received from doctoral students and alumni/ae of the College of Education of one large, online, accredited university reduced the initial theoretical framework from seven to six attributes, and resulted in a revision of the scale.     Page Length and Methodological Characteristics of Recently Published Doctoral Dissertations in Education

    In this methodological review, we conducted a quantitative content analysis of a random sample of 107 education-related doctoral dissertations published in Proquest Dissertations and Theses database in 2011. Seven raters coded each article in terms of page lengths (overall and within each chapter), research method (qualitative, quantitative, or mixed-methods), author gender, and university characteristics (online or traditional). We found that the median education dissertation length was 161 pages long, but those page lengths differed between research methods. The median page lengths of qualitative, mixed method, and quantitative dissertations were 210, 187, and 147 respectively. The median page length of education dissertations from online universities was 44 pages shorter than education dissertations from their traditional counterparts. Contrary to previous research, we found no statistically significant relationship between gender and methods choice

    Race and Sex Differences in QRS Interval and Associated Outcome Among Patients with Left Ventricular Systolic Dysfunction

    BACKGROUND: Prolonged QRS duration is associated with increased mortality among heart failure patients, but race or sex differences in QRS duration and associated effect on outcomes are unknown. METHODS AND RESULTS: We investigated QRS duration and morphology among 2463 black and white patients with heart failure and left ventricular ejection fraction ≤35% who underwent coronary angiography and 12-lead electrocardiography at Duke University Hospital from 1995 through 2011. We used multivariable Cox regression models to assess the relationship between QRS duration and all-cause mortality and investigate race-QRS and sex-QRS duration interaction. Median QRS duration was 105 ms (interquartile range [IQR], 92-132) with variation by race and sex (P<0.001). QRS duration was longest in white men (111 ms; IQR, 98-139) followed by white women (108 ms; IQR, 92-140), black men (100 ms; IQR, 91-120), and black women (94 ms; IQR, 86-118). Left bundle branch block was more common in women than men (24% vs 14%) and in white (21%) versus black individuals (12%). In black patients, there was a 16% increase in risk of mortality for every 10 ms increase in QRS duration up to 112 ms (hazard ratio, 1.16; 95% CI, 1.07, 1.25) that was not present among white patients (interaction, P=0.06). CONCLUSIONS: Black individuals with heart failure had a shorter QRS duration and more often had non-left bundle branch block morphology than white patients. Women had left bundle branch block more commonly than men. Among black patients, modest QRS prolongation was associated with increased mortality

    Clinician Perspectives of an Intensive Comprehensive Aphasia Program

    Background: Intensive comprehensive aphasia programs (ICAPs) have increased in number in recent years in the United States and abroad. Objective: To describe the experiences of clinicians working in an ICAP. Methods: A phenomenological approach was taken. Seven clinicians from 3 ICAPs were interviewed in person or on the phone. Their interviews were transcribed and coded for themes relating to their experiences. Results: Clinicians described 3 major themes. The first theme related to the intensity component of the ICAP that allowed clinicians to provide in-depth treatment and gave them a different perspective with regard to providing treatment and the potential impact on the person with aphasia. The second theme of rewards for the clinicians included learning and support, seeing progress, and developing relationships with their clients and family members. Third, challenges were noted, including the time involved in learning new therapy techniques, patient characteristics such as chronicity of the aphasia, and the difficulty of returning to work in typical clinical settings after having experienced an ICAP. Conclusions: Although there is a potential for bias with the small sample size, this pilot study gives insight into the clinician perspective of what makes working in an ICAP both worthwhile and challenging

    The Effect of Race and Chronic Obstructive Pulmonary Disease on Long-Term Survival after Coronary Artery Bypass Grafting

    Background: Chronic obstructive pulmonary disease (COPD) is a known predictor of decreased long-term survival after coronary artery bypass grafting (CABG). Differences in survival by race have not been examined. Methods: A retrospective cohort study was conducted of CABG patients between 2002 and 2011. Long-term survival was compared in patients with and without COPD and stratified by race. Hazard ratios (HR) and 95% confidence intervals (CI) were computed using a Cox regression model. Results: A total of 984 (20%) patients had COPD (black n = 182; white n = 802) at the time of CABG (N = 4,801). The median follow-up for study participants was 4.4 years. COPD was observed to be a statistically significant predictor of decreased survival independent of race following CABG (no COPD: HR = 1.0; white COPD: adjusted HR = 1.9, 95% CI = 1.7–2.3; black COPD: adjusted HR = 1.6, 95% CI = 1.1–2.2). Conclusion: Contrary to the expected increased risk of mortality among black COPD patients in the general population, a similar survival disadvantage was not observed in our CABG population
