    Concurrent associations among sleep problems, indicators of inadequate sleep, psychopathology, and shared risk factors in a population-based sample of healthy Ontario children

    Objectives Examine the contribution of sleep problems and indicators of inadequate sleep to psychopathology among children after accounting for shared risk and comorbid psychopathology. Methods Secondary analyses of cross-sectional data on 4- to 11-year-old (N = 1,550) children without chronic illness or developmental delay or disability. Parents provided information about sleep problems, indicators of inadequate sleep, symptoms of psychopathology, and risk factors for psychopathology. Teachers provided information about indicators of inadequate sleep and symptoms of psychopathology. Results Adjusting for risk factors and comorbid psychopathology, sleeping more than other children was related to parent-rated aggression. Nightmares and trouble sleeping were related to parent-rated anxious/depressed mood. Sleep problems were not related to attention problems. Being overtired was related to parent- and teacher-rated psychopathology. Conclusions Relations among sleep problems, indicators of inadequate sleep, and psychopathology are complex; accounting for potential confounding variables and considering sleep variables separately may clarify these relations. © 2009 The Author.     Medical Management of Growth Hormone-Secreting Pituitary Adenomas

    The primary treatment of acromegaly remains transsphenoidal adenomectomy, yet the tissue overgrowth of acromegaly often progresses following surgery, and responds to radiotherapy only after significant delay. Persistently elevated serum growth hormone (GH) and insulin-like growth factor-I (IGF-I) concentrations can be normalized in about half of post-surgery acromegalics using the pharmacologic alternatives presently available, the dopamine agonists (DA) and somatostatin (SST) analogs. Cabergoline, the most efficacious DA, normalizes IGF-I in approximately 37% of patients, whereas the long-acting SST analogs, Octreotide LAR and Lanreotide SR, do so in 66%. Significant tumor shrinkage may be attained with SST analogs in particular, and when necessary, the primary medical treatment of acromegaly may be successfully addressed with this class of drugs. Greatly enhanced efficacy is expected from the GH receptor antagonist pegvisomant, which is nearing market availability and will enable the normalization of serum IGF-I in virtually all patients treated.     UK science press officers, professional vision and the generation of expectations

    Science press officers can play an integral role in helping promote expectations and hype about biomedical research. Using this as a starting point, this article draws on interviews with 10 UK-based science press officers, which explored how they view their role as science reporters and as generators of expectations. Using Goodwin’s notion of ‘professional vision’, we argue that science press officers have a specific professional vision that shapes how they produce biomedical press releases, engage in promotion of biomedical research and make sense of hype. This expository paper describes a uniform method for giving a concrete realization of Tits's geometry and works through several examples. We also give a criterion for recognizing the automorphism of the geometry induced by an automorphism of the group.     Nonclassic lipoid congenital adrenal hyperplasia masquerading as familial glucocorticoid deficiency

    Context: Familial glucocorticoid deficiency (FGD) is an autosomal recessive disorder resulting from resistance to the action of ACTH on the adrenal cortex. Affected individuals are deficient in cortisol and, if untreated, are likely to succumb to hypoglycemia and/or overwhelming infection. Mutations of the ACTH receptor (MC2R) and the melanocortin 2 receptor accessory protein (MRAP), FGD types 1 and 2 respectively, account for approximately 45% of cases. Objective: A locus on chromosome 8 has previously been linked to the disease in three families, but no underlying gene defect has to date been identified. Design: The study design comprised single-nucleotide polymorphism genotyping and mutation detection. Setting: The study was conducted at secondary and tertiary referral centers. Patients: Eighty probands from families referred for investigation of the genetic cause of FGD participated in the study. Interventions: There were no interventions. Results: Analysis by single-nucleotide polymorphism array of the genotype of one individual with FGD previously linked to chromosome 8 revealed a large region of homozygosity encompassing the steroidogenic acute regulatory protein gene, STAR. We identified homozygous STAR mutations in this patient and his affected siblings. Screening of our total FGD patient cohort revealed homozygous STAR mutations in a further nine individuals from four other families. Conclusions: Mutations in STAR usually cause lipoid congenital adrenal hyperplasia, a disorder characterized by both gonadal and adrenal steroid deficiency. Our results demonstrate that certain mutations in STAR (R192C and the previously reported R188C) can present with a phenotype indistinguishable from that seen in FGD

    Discovery of two L & T binaries with wide separations and peculiar photometric properties

    We present spatially resolved photometric and spectroscopic observations of two wide brown dwarf binaries uncovered by the SIMP near-infrared proper motion survey. The first pair (SIMP J1619275+031350AB) has a separation of 0.691" (15.2 AU) and components T2.5+T4.0, at the cooler end of the ill-understood J-band brightening. The system is unusual in that the earlier-type primary is bluer in J-Ks than the later-type secondary, whereas the reverse is expected for binaries in the late-L to T dwarf range. This remarkable color reversal can possibly be explained by very different cloud properties between the two components. The second pair (SIMP J1501530-013506AB) consists of an L4.5+L5.5 (separation 0.96", 30-47 AU) with a surprisingly large flux ratio (Delta J =1.79 mag) considering the similar spectral types of its components. The large flux ratio could be explained if the primary is itself an equal-luminosity binary, which would make it one of the first known triple brown dwarf systems. Adaptive optics observations could not confirm this hypothesis, but it remains a likely one, which may be verified by high-resolution near-infrared spectroscopy. These two systems add to the handful of known brown dwarf binaries amenable to resolved spectroscopy without the aid of adaptive optics and constitute prime targets to test brown dwarf atmosphere models.Comment: accepted for publication in Ap

    Panic at the ISCO: the visible accretion disks powering optical variability in ZTF AGN

    About 3-10% of Type I active galactic nuclei (AGN) have double-peaked broad Balmer lines in their optical spectra originating from the motion of gas in their accretion disk. Double-peaked profiles arise not only in AGN, but occasionally appear during optical flares from tidal disruption events and changing-state AGN. In this paper we identify 250 double-peaked emitters (DPEs) amongst a parent sample of optically variable broad-line AGN in the Zwicky Transient Facility (ZTF) survey, corresponding to a DPE fraction of 19%. We model spectra of the broad H alpha emission line regions and provide a catalog of the fitted accretion disk properties for the 250 DPEs. Analysis of power spectra derived from the 5 year ZTF light curves finds that DPEs have similar amplitudes and power law indices to other broad-line AGN, but have lower turnover frequencies. Follow-up spectroscopy of 12 DPEs reveals that ~50% display significant changes in the relative strengths of their red and blue peaks over long 10-20 year timescales, indicating that broad-line profile changes arising from spiral arm or hotspot rotation are common amongst optically variable DPEs. Analysis of the accretion disk parameters derived from spectroscopic modeling provides evidence that DPEs are not in a special accretion state, but are simply normal broad-line AGN viewed under the right conditions for the accretion disk to be easily visible. We compare the radio variability properties of the two samples and present radio jet imaging of 3 DPEs with disks of inclination angle 14-35 degrees. We discuss some objects with notable light curves or unusual broad line profiles which are outliers amongst the variable DPE population. We include inspiraling SMBH binary candidate SDSSJ1430+2303 in our analysis, and discuss how its photometric and spectroscopic variability is consistent with the disk-emitting AGN population in ZTF.Comment: Submitted to ApJ. 30 pages, 10 figures, 4 tables.
