    Perceptions on ILE’s access in North and Center argentinian provinces during 2020

    Este estudio se sitúa en la región centro y noroeste de Argentina durante el 2020, escenario previo a la legalización de la Interrupción Voluntaria del Embarazo. Mediante un abordaje cuantitativo, el estudio pretende analizar el acceso a la Interrupción Legal del Embarazo (ILE). Se abordan las barreras objetivas y subjetivas desde las percepciones, experiencias y conocimientos en torno al aborto. Los resultados dan cuenta de una implementación desigual de la ILE en el territorio argentino. La encuestadas perciben que el aborto es una práctica de difícil acceso, apuntando a un conjunto de obstáculos tanto institucionales -vinculados principalmente a su disponibilidad en el sistema de salud- como subjetivos, que se corresponden principalmente con la estigmatización de la práctica. El ejercicio universal del derecho al aborto requiere de un análisis particular respecto a la inequidad que suscita la disponibilidad de servicios de calidad y las posibilidades de acceso en esta región del paísThis study is situated in the year 2020 in Argentina, in the country’s central and northwest region (NW), the previous scenario to the legalization of the Voluntary Interruption Of Pregnancy (VIP). Through a quantitative study, this study aims to analyze de access to Legal Pregnancy Interruption. Objective and subjective barriers are adressed from the perceptions, experiences and knowledge surrounding abortion. The respondents perceive that abortion is a difficult-to-access practice, pointing to a set of institutional obstacles -mainly linked to its availability in the health system- and subjective obstacles, which correspond mainly to the stigmatization of the practice. The universal exercise to the right to abortion requires a thorough analysis of the inequities in the availability to a quality service and the differences in access to services in these country regions.Fil: Aizenberg, Lila.     Psychological aspects in the management of patients with essential hypertension

    The hypertensive patient is the most stable phenotype in psychosomatic medicine. Hypertensive patients represent a vulnerable population that deserves special attention from health care providers and systems, and psychosomatic medicine may be an important tool in the management of high blood pressure. Depression, anxiety disorders and personality features are often associated with elevated blood pressure (BP) and they may have a role in the development of mild high-renin essential hypertension. Besides, "white coat" hypertension and "masked" hypertension demonstrate how clinic blood pressure could be strongly related to trait anxiety. Hypertension is largely asymptomatic, and patients often have little understanding of the importance of achieving BP control. Medication adverse effects may become an important factor in poor adherence to the treatment and the antidepressant use increases the risk of hypertension. So, the challenge in the management of hypertensive patients is the adherence to non-pharmacological and behavioural treatments for hypertension

    Mortality and pulmonary complications in patients undergoing surgery with perioperative SARS-CoV-2 infection: an international cohort study

    Background: The impact of severe acute respiratory syndrome coronavirus 2 (SARS-CoV-2) on postoperative recovery needs to be understood to inform clinical decision making during and after the COVID-19 pandemic. This study reports 30-day mortality and pulmonary complication rates in patients with perioperative SARS-CoV-2 infection. Methods: This international, multicentre, cohort study at 235 hospitals in 24 countries included all patients undergoing surgery who had SARS-CoV-2 infection confirmed within 7 days before or 30 days after surgery. The primary outcome measure was 30-day postoperative mortality and was assessed in all enrolled patients. The main secondary outcome measure was pulmonary complications, defined as pneumonia, acute respiratory distress syndrome, or unexpected postoperative ventilation. Findings: This analysis includes 1128 patients who had surgery between Jan 1 and March 31, 2020, of whom 835 (74·0%) had emergency surgery and 280 (24·8%) had elective surgery. SARS-CoV-2 infection was confirmed preoperatively in 294 (26·1%) patients. 30-day mortality was 23·8% (268 of 1128). Pulmonary complications occurred in 577 (51·2%) of 1128 patients; 30-day mortality in these patients was 38·0% (219 of 577), accounting for 81·7% (219 of 268) of all deaths. In adjusted analyses, 30-day mortality was associated with male sex (odds ratio 1·75 [95% CI 1·28–2·40], p\textless0·0001), age 70 years or older versus younger than 70 years (2·30 [1·65–3·22], p\textless0·0001), American Society of Anesthesiologists grades 3–5 versus grades 1–2 (2·35 [1·57–3·53], p\textless0·0001), malignant versus benign or obstetric diagnosis (1·55 [1·01–2·39], p=0·046), emergency versus elective surgery (1·67 [1·06–2·63], p=0·026), and major versus minor surgery (1·52 [1·01–2·31], p=0·047). Interpretation: Postoperative pulmonary complications occur in half of patients with perioperative SARS-CoV-2 infection and are associated with high mortality. Thresholds for surgery during the COVID-19 pandemic should be higher than during normal practice, particularly in men aged 70 years and older. Consideration should be given for postponing non-urgent procedures and promoting non-operative treatment to delay or avoid the need for surgery.     Pharmacological activation of mesenchymal stem cells increases gene expression pattern of cell adhesion molecules and fusion with neonatal cardiomyocytes

    Cellular therapy is considered a better option for the treatment of degenerative disorders. Different cell types are being used for tissue regeneration. Despite extensive research in this field, several issues remain to be addressed concerning cell transplantation. One of these issues is the survival and homing of administered cells in the injured tissue, which depends on the ability of these cells to adhere. To enhance cell adherence and survival, Rap1 GTPase was activated in mesenchymal stem cells (MSCs) as well as in cardiomyocytes (CMs) by using 8-pCPT-2\u27-O-Me-cAMP, and the effect on gene expression dynamics was determined through quantitative reverse transcriptase-polymerase chain reaction analysis. Pharmacological activation of MSCs and CMs resulted in the upregulation of connexin-43 and cell adhesion genes, which increased the cell adhesion ability of MSCs and CMs, and increased the fusion of MSCs with neonatal CMs. Treating stem cells with a pharmacological agent that activates Rap1a before transplantation can enhance their fusion with CMs and increase cellular regeneration
