Ophthalmic Problems at High Altitude
Cardio-pulmonary and physical endurance aspects of high altitude problems have been studied well but not much attention has been paid to the ocular aspects of the problems. 250 cases inducted to a high altitude area in the northern region were examined for ocular parameters within a week of induction and at varying intervals thereafter. Blurred vision was complained of by a number of cases who also suffered from other symptoms of transient mountain sickness. A group of cases who developed acute pulmonary oedema showed fundus changes by way of venous engorgement, papilloedema and superficial retinal haemorrhages. These changes reversed over a period of time as the acute condition subsided. Another group remained asymptomatic but their fundii showed neovascularisation and exudation in addition to retinal haemorrhages.The aetiopathogenesis of these two kinds of changes and other changes is discussed
Risk Factors for Colorectal Cancer in Patients with Multiple Serrated Polyps: A Cross-Sectional Case Series from Genetics Clinics
Patients with multiple serrated polyps are at an increased risk for developing colorectal cancer (CRC). Recent reports have linked cigarette smoking with the subset of CRC that develops from serrated polyps. The aim of this work therefore was to investigate the association between smoking and the risk of CRC in high-risk genetics clinic patients presenting with multiple serrated polyps. Methods and Findings We identified 151 Caucasian individuals with multiple serrated polyps including at least 5 outside the rectum, and classified patients into non-smokers, current or former smokers at the time of initial diagnosis of polyposis. Cases were individuals with multiple serrated polyps who presented with CRC. Controls were individuals with multiple serrated polyps and no CRC. Multivariate logistic regression was performed to estimate associations between smoking and CRC with adjustment for age at first presentation, sex and co-existing traditional adenomas, a feature that has been consistently linked with CRC risk in patients with multiple serrated polyps. CRC was present in 56 (37%) individuals at presentation. Patients with at least one adenoma were 4 times more likely to present with CRC compared with patients without adenomas (OR = 4.09; 95%CI 1.27 to 13.14; P = 0.02). For females, the odds of CRC decreased by 90% in current smokers as compared to never smokers (OR = 0.10; 95%CI 0.02 to 0.47; P = 0.004) after adjusting for age and adenomas. For males, there was no relationship between current smoking and CRC. There was no statistical evidence of an association between former smoking and CRC for both sexes. Conclusion A decreased odds for CRC was identified in females with multiple serrated polyps who currently smoke, independent of age and the presence of a traditional adenoma. Investigations into the biological basis for these observations could lead to non-smoking-related therapies being developed to decrease the risk of CRC and colectomy in these patients.Daniel D. Nanoscale Metallic Iron for Environmental Remediation: Prospects and Limitations
The amendment of the subsurface with nanoscale metallic iron particles
(nano-Fe0) has been discussed in the literature as an efficient in situ
technology for groundwater remediation. However, the introduction of this
technology was controversial and its efficiency has never been univocally
established. This unsatisfying situation has motivated this communication whose
objective was a comprehensive discussion of the intrinsic reactivity of
nano-Fe0 based on the contemporary knowledge on the mechanism of contaminant
removal by Fe0 and a mathematical model. It is showed that due to limitations
of the mass transfer of nano-Fe0 to contaminants, available concepts cannot
explain the success of nano-Fe0 injection for in situ groundwater remediation.
It is recommended to test the possibility of introducing nano-Fe0 to initiate
the formation of roll-fronts which propagation would induce the reductive
transformation of both dissolved and adsorbed contaminants. Within a
roll-front, FeII from nano-Fe0 is the reducing agent for contaminants. FeII is
recycled by biotic or abiotic FeIII reduction. While the roll-front concept
could explain the success of already implemented reaction zones, more research
is needed for a science-based recommendation of nano- Fe0 for subsurface
treatment by roll-front

Top-of-Rail lubricant
Analysis of the volatile and semivolatile fractions collected after use of the TOR lubricant indicated that other than contaminants in the collection laboratory, no compounds on the EPA's Target Compound Lists (Tables 2 and 5) were detected in these fractions. The data of these qualitative analyses, given in the various tables in the text, indicate only the relative amounts of the tentatively identified compounds. The authors recommend that quantitative analysis be performed on the volatile and semivolatile fractions to allow confirmation of the tentatively identified compounds and to obtain absolute amounts of the detected compounds. Additionally, the semivolatile fraction should be analyzed by liquid chromatography/mass spectrometry to identify compounds that are not chromatographable under the temperature program used for determination of semivolatile compounds. Introducing the top-of-rail (TOR) lubricant into the wheel/rail interface results in a reduction of almost 60% of lateral friction force over the forces encountered under dry conditions. This reveals good potential for energy savings, as well as wear reduction, for railroad companies. In TOR lubrication, an increase in the angle of attack and axle load results in increased lateral friction and rate of lubricant consumption. The most efficient TOR lubricant quantity to be used in the wheel/rail interface must be calculated precisely according to the number of cars, axle loads, train speed, and angle of attack

USDOE Top-of-Rail Lubricant Project
Lubrication of wheel/rail systems has been recognized for the last two decades as a very important issue for railroads. Energy savings and less friction and wear can be realized if a lubricant can be used at the wheel/rail interface. On the other hand, adverse influences are seen in operating and wear conditions if improper or excessive lubrication is used. Also, inefficiencies in lubrication need to be avoided for economic and environmental reasons. The top-of-rail (TOR) lubricant concept was developed by Texaco Corporation to lubricate wheels and rails effectively and efficiently. Tranergy Corporation has been developing its SENTRAEN 2000{trademark} lubrication system for the last ten years, and this revolutionary new high-tech on-board rail lubrication system promises to dramatically improve the energy efficiency, performance, safety, and track environment of railroads. The system is fully computer-controlled and ensures that all of the lubricant is consumed as the end of the train passes. Lubricant quantity dispensed is a function of grade, speed, curve, and axle load. Tranergy also has its LA4000{trademark} wheel and rail simulator, a lubrication and traction testing apparatus. The primary task of this project was collecting and analyzing the volatile and semivolatile compounds produced as the lubricant was used. The volatile organic compounds were collected by Carbotrap cartridges and analyzed by adsorption and gas chromatography/mass spectrometry (GC/MS). The semivolatile fraction was obtained by collecting liquid that dripped from the test wheel. The collected material was also analyzed by GC/MS. Both of these analyses were qualitative. The results indicated that in the volatile fraction, the only compounds on the Environmental Protection Agency's (EPA) Superfund List of Analytes detected were contaminants either in the room air or from other potential contamination sources in the laboratory. Similarly, in the semivolatile fraction none of the detected compounds are on the EPA's Superfund List of Analytes. The major compound in the semivolatile fraction is 1,2-propanediol, which was also found as the major component of the TOR lubricant before testing. Other compounds found in trace quantities either were present in the TOR lubricant or were small fragments from the polymeric component of the TOR lubricant. The second task for Argonne in this project was to investigate the effects of axle load, angle of attack, and quantity of lubricant on lateral friction forces, as well as the consumption time of the TOR lubricant.
