    Sickness certification system in the United Kingdom: qualitative study of views of general practitioners in Scotland

    Objectives: To explore how general practitioners operate the sickness certification system, their views on the system, and suggestions for change. Design: Qualitative focus group study consisting of 11 focus groups with 67 participants. Setting: General practitioners in practices in Glasgow, Tayside, and Highland regions, Scotland. Sample: Purposive sample of general practitioners, with further theoretical sampling of key informant general practitioners to examine emerging themes. Results: General practitioners believed that the sickness certification system failed to address complex, chronic, or doubtful cases. They seemed to develop various operational strategies for its implementation. There appeared to be important deliberate misuse of the system by general practitioners, possibly related to conflicts about roles and incongruities in the system. The doctor-patient relationship was perceived to conflict with the current role of general practitioners in sickness certification. When making decisions about certification, the general practitioners considered a wide variety of factors. They experienced contradictory demands from other system stakeholders and felt blamed for failing to make impossible reconciliations. They clearly identified the difficulties of operating the system when there was no continuity of patient care. Many wished either to relinquish their gatekeeper role or to continue only with major changes. Conclusions: Policy makers need to recognise and accommodate the range and complexity of factors that influence the behaviour of general practitioners operating as gatekeepers to the sickness certification system, before making changes. Such changes are otherwise unlikely to result in improvement. Models other than the primary care gatekeeper model should be considered

    350 Micron Observations of Ultraluminous Infrared Galaxies at Intermediate Redshifts

    We present 350micron observations of 36 ultraluminous infrared galaxies (ULIRGs) at intermediate redshifts (0.089 <= z <= 0.926) using the Submillimeter High Angular Resolution Camera II (SHARC-II) on the Caltech Submillimeter Observatory (CSO). In total, 28 sources are detected at S/N >= 3, providing the first flux measurements longward of 100micron for a statistically significant sample of ULIRGs in the redshift range of 0.1 < z < 1.0. Combining our 350micron flux measurements with the existing IRAS 60 and 100micron data, we fit a single-temperature model to the spectral energy distribution (SED), and thereby estimate dust temperatures and far-IR luminosities. Assuming an emissivity index of beta = 1.5, we find a median dust temperature and far-IR luminosity of Td = 42.8+-7.1K and log(Lfir/Lsolar) = 12.2+-0.5, respectively. The far-IR/radio correlation observed in local star-forming galaxies is found to hold for ULIRGs in the redshift range 0.1 < z < 0.5, suggesting that the dust in these sources is predominantly heated by starbursts. We compare the far-IR luminosities and dust temperatures derived for dusty galaxy samples at low and high redshifts with our sample of ULIRGs at intermediate redshift. A general Lfir-Td relation is observed, albeit with significant scatter, due to differing selection effects and variations in dust mass and grain properties. The relatively high dust temperatures observed for our sample compared to that of high-z submillimeter-selected starbursts with similar far-IR luminosities suggest that the dominant star formation in ULIRGs at moderate redshifts takes place on smaller spatial scales than at higher redshifts.Comment: (24 pages in preprint format, 1 table, 7 figures, accepted for publication in ApJ

    How Firms Respond to Mandatory Information Disclosure

    Mandatory information disclosure regulations seek to create institutional pressure to spur performance improvement. By examining how organizational characteristics moderate establishments' responses to a prominent environmental information disclosure program, we provide among the first empirical evidence characterizing heterogeneous responses by those mandated to disclose information. We find particularly rapid improvement among establishments located close to their headquarters and among establishments with proximate siblings, especially when the proximate siblings are in the same industry. Large establishments improve more slowly than small establishments in sparse regions, but both groups improve similarly in dense regions, suggesting that density mitigates the power of large establishments to resist institutional pressures. Finally, privately held firms' establishments outperform those owned by public firms.
