Diagnostic imaging of neuroendocrine tumours
Guzy neuroendokrynne (NET) stanowią heterogenną grupę nowotworów posiadających mechanizmy
gromadzenia prekursorów amin biogennych oraz ekspresję specyficznych białek receptorowych na swojej
błonie komórkowej, które pomagają w lokalizacji oraz w leczeniu. Guzy typu NET różnią się między
sobą substancjami wydzielanymi przez komórki, obecnością czynności hormonalnej lub jej brakiem,
objawami klinicznymi, cechami histopatologicznymi oraz rokowaniem.
Pochodzą one z gruczołów wydzielania wewnętrznego (przysadka, przytarczyce, rdzeń nadnercza) oraz
dodatkowo z komórek rozlanego systemu endokrynnego o lokalizacji w ścianie przewodu pokarmowego,
trzustce, tarczycy, grasicy czy w oskrzelach. Guzy NET o pochodzeniu GEP-NET stanowią większość
tego typu guzów (> 70% wszystkich NET).
Badania obrazowe i ocena swoistych markerów guzów NET umożliwiają identyfikację i ocenę stadium
zaawansowania tych rzadkich nowotworów, a ponadto mają wartość prognostyczną. Najbardziej uniwersalną
techniką obrazową wykorzystywaną w badaniu NET jest SRS. Innych badań obrazowych, takich jak
spiralna wielorzędowa CT, MRI, endoskopowe oraz śródoperacyjne badanie USG, używa się do precyzyjnej
anatomicznej lokalizacji zmian patologicznych. Kolejnym badaniem czynnościowym wykorzystywanym
w diagnostyce NET jest scyntygrafia MIBG (metajodobenzyloguanidyna). Badanie to pozwala
identyfikować przede wszystkim guz chromochłonny oraz MTC. W postaciach złośliwych guza chromochłonnego
oraz raka rdzeniastego tarczycy znaczenie ma również badanie SRS. Bardzo ważnym aspektem
wykorzystania technik obrazowych (zwłaszcza CT, SRS i MRI) jest ocena odpowiedzi na leczenie.
Coraz większe znaczenie kliniczne w lokalizacji guzów NET ma badanie PET z użyciem nowych ligandów
receptorowych wyznakowanych 68Ga. Wykorzystanie standardowego FDG PET stosuje się w guzach
NET o wysokiej złośliwości.Neuroendocrine tumours (NET) consists of a heterogeneous group of neoplasms, that are able to express
cell membrane neuroamine uptake mechanisms and/or specific receptors, such as somatostatin
receptors, which can be used in the localization and treatment of these tumours. Conventionally NETs
may present with a wide variety of functional or nonfunctional endocrine syndromes and may be familial
and have other associated tumors, also they have different histology pattern and prognosis. They originate
from endocrine glands such as the pituitary, the parathyroids, and the (neuroendocrine) adrenal, as
well as endocrine islets within glandular tissue (thyroid or pancreatic) and cells dispersed between exocrine
cells, such as endocrine cells of the digestive system (gastroenteropancreatic GEP-NET) and respiratory
tracts. GEP-NET are the most common including more then 70% of all NETs.Imaging modalities and assessment of specific tumor markers offers high sensitivity in establishing the
diagnosis and can also have prognostic significance. Most important single imaging technique in terms
of initial identification and staging of GEP-NET seems to be somatostatin receptor scintigraphy (SRS).
Other investigations like helical computed tomography (CT), magnetic resonance imaging (MRI), endoscopic
and/or peri-operative ultrasonography are used for the precise localization of NET.
Another one functional approach include MIBG (meta-iodobenzylguanidine scintigraphy). This technique
is sensitive in the identification of chromaffin cell tumours pheochromocytoma, and also medullary thyroid
carcinoma (MTC), although SRS seems to be very useful in the localization of malignant chromaffin
cell tumours and MTC as well. The further localization and monitoring of the response to treatment CT,
SRS and MRI are used with high diagnostic accuracy.
More recently, positron emission tomography (PET) scanning is being increasingly used for the localization
of NETs, due to develop new PET tracers (68Ga), the standard one FDG PET is currently used in
groups of high malignant NET
The Influence of Temperature on Coumarin 153 Fluorescence Kinetics
The influence of temperature varied in the range 183 K–323 K on the fluorescence quantum yield, fluorescence lifetime, absorption and emission transition moments and non-radiative deactivation rate was determined for the well known and largely used dye Coumarin 153, dissolved in 1-chloropropane. The Kennard-Stepanov relation connecting the absorption and emission spectra was used to check for the presence of more than one absorbing/emitting species and to investigate whether intramolecular vibrational redistribution completes in the C153 excited S1 state before the emission takes place. The emission spectrum corresponding to S1→S0 transition, was fitted at each temperature to the model function including the information on the dye vibrational modes coupling. In this way the displacement in equilibrium distance for the most active vibrational mode was determined for C153 in S1 and in S0. Using the temperature dependence of the fluorescence decay time and quantum yield, the non-radiative deactivation rate was determined. Its temperature dependence was compared to that calculated using the theoretical model with the most active vibrational mode displacement values taken from steady-state spectra analysis. The somewhat surprising dependence of the fluorescence decay time and quantum yield on temperature was related to non-trivial coupling between low-frequency vibrational modes of C153 in the excited and ground states
Related to Anxiety: Arbitrarily Applicable Relational Responding and Experimental Psychopathology Research on Fear and Avoidance
Humans have an unparalleled ability to engage in arbitrarily applicable
relational responding (AARR). One of the consequences of this ability to spontaneously
combine and relate events from the past, present, and future may, in fact, be a
propensity to suffer. For instance, maladaptive fear and avoidance of remote or derived
threats may actually perpetuate anxiety. In this narrative review, we consider contemporary
AARR research on fear and avoidance as it relates to anxiety. We first describe
laboratory-based research on the emergent spread of fear- and avoidance-eliciting
functions in humans. Next, we consider the validity of AARR research on fear and
avoidance and address the therapeutic implications of the work. Finally, we outline
challenges and opportunities for a greater synthesis between behavior analysis research
on AARR and experimental psychopathology
Application of optical topography in non-invasive study of brain blood perfusion and its preliminary verification by SPECT technique
Obserwacja zjawiska napływu i klirensu dożylnych środków kontrastujących jest techniką często stosowaną w diagnostyce medycznej. Zastosowanie w tym celu barwników, których widmo absorpcyjne leży w zakresie bliskiej podczerwieni, może być wykorzystane do badań hemodynamiki mózgu metodą optyczną. Opublikowane w ostatnich latach wyniki badań potwierdzają, że cyrkulacja barwnika ICG (zieleń indocjaninowa) w tkance mózgowej może być wykryta bezinwazyjnie metodą optyczną. W niniejszej pracy zostały przedstawione wstępne wyniki badań uzyskane z wykorzystaniem wielokanałowego sytemu do czasowo rozdzielczych pomiarów optycznych, skonstruowanego w IBIB PAN. Badania z zastosowaniem środka kontrastującego ICG przeprowadzono u osób zdrowych i w grupie pacjentów ze stwierdzonymi średnimi dolegliwościami neurologicznymi. Badania porównawcze zostały wykonane metodą radioizotopową SPECT używaną rutynowo w badaniach ukrwienia mózgu.Monitoring of inflow and washout of an exogenous contrast agent circulating in the human body is frequently utilized for medical diagnosis. Injection of a absorbing dye in near infrared wavelength region can be applied for estimation of cerebral blood flow or perfusion using optical method. It was reported that ICG (indocyanine green) circulating in the brain can be detected non-invasively on the surface of the human adult head. In the present paper we show results of analysis of time-resolved optical signals measured with the use of the self-constructed brain imager based on previously reported brain oxygenation topography system. The study were curried out on healthy volunteers and patients with mild cerebrovascular disorders after injections of the ICG contrast agent. Obtained results from optical measurements were compared with routinely used SPECT technique
Zastosowanie metod medycyny nuklearnej w diabetologii
Cukrzyca jest chorobą wieloukładową, charakteryzującą się niedoborem insuliny lub opornością tkanek na działanie insuliny, które prowadzą do stanów hiperglikemii. W przebiegu długotrwałej cukrzycy dochodzi do upośledzenia licznych funkcji organizmu oraz uszkodzenia narządów na skutek mikro- i makroangiopatii. Proces ten ma charakter postępujący i wieloletni. Patologiczne zmiany na poziomie mikroskopowym zaczynają się kilka miesięcy lub nawet kilka lat przed kliniczną manifestacją choroby. W tym okresie diagnostyka tego typu zaburzeń przy użyciu klasycznych metod obrazowania jest niezwykle trudna, a czasem niemożliwa. Metody radioizotopowe, obrazując procesy zachodzące na poziomie komórkowym, pozwalają na bardzo wczesną diagnostykę procesów chorobowych występujących w przebiegu cukrzycy
