    Increase in the selenium content of extra virgin olive oil: quantitative and qualitative implications

    The biofortification of food crops for human consumption is a direct strategy increasing dietary intake of selenium (Se). The aim of this study was to evaluate the possibility of increasing the Se content of extra virgin olive oil (EVOO) by spraying the olive tree canopy with sodium selenate and the effect of the increase in Se on the chemical properties and sensory characteristics of the EVOO. Se treatments were up to 50 times more effective in enhancing Se content in the EVOO compared with the untreated controls. Se concentration in all the EVOO samples can be considered adequate and useful for providing the human diet with the correct dose of Se. Se-enriched EVOO showed a significant increase in pigment and phenol content. Also, Se treatment does not produce negative effects on fruit characteristics or the sensory quality of EVOO.La biofortificación de cultivos alimenticios para el consumo humano es una estrategia directa para aumentar la ingesta de selenio (Se) en la dieta. El objetivo de este estudio fue evaluar la posibilidad de aumentar el contenido de Se en aceites de oliva virgen extra (AOVE) pulverizando la copa de los olivos con selenato de sodio y el efecto del aumento en el contenido de Se en las propiedades químicas y características sensoriales del AOVE. Los tratamientos con Se fueron muy eficaces consiguiendo aumentar el contenido de Se en el AOVE hasta 50 veces más en comparación con los controles no tratados. La concentración de Se en todas las muestras EVOO puede considerarse adecuada y útil para proporcionar a la dieta humana con la dosis correcta de Se. EVOO-Se enriquecido mostró un aumento significativo en pigmentos y contenido de fenoles. Además, el tratamiento de Se no implica efectos negativos sobre caracteristicas frutales ni sobre la calidad sensorial de AOVE

    Thoracic Unilateral Spinal Cord Injury After Spinal Anaesthesia for Total Hip Replacement: Fate or Mistake?

    Spinal anaesthesia is the most preferred anesthesia technique for total hip replacement, and its complications range from low entity (insignificant) to life threatening. The incidence of neurologic complications after neuraxial anaesthesia is not perfectly clear, although there are several described cases of spinal cord ischaemia. We present a case of unilateral T8–T11 spinal cord ischaemia following L2–L3 spinal anaesthesia for total hip replacement. Magnetic resonance imaging showed a hyperintense T8–T11 signal alteration on the leftside of paramedian spinal cord. A temporal epidemiologic linkage between the damage and the surgery seems to be present. The injury occurred without anatomical proximity between the injury site and the spinal needle entry site. This may be due to multiple contributing factors, each of them is probably not enough to determine the damage by itself; however, acting simultaneously, they could have been responsible for the complication. The result was unpredictable and unavoidable and was caused by unforeseeable circumstances and not by inadequate medical practice

    Low NT-proBNP levels in overweight and obese patients do not rule out a diagnosis of heart failure with preserved ejection fraction

    Background Heart failure with preserved ejection fraction (HFpEF) is a heterogeneous syndrome that presents clinicians with a diagnostic challenge. The use of natriuretic peptides to exclude a diagnosis of HFpEF has been proposed. We sought to compare HFpEF patients with N-terminal pro-brain natriuretic peptide (NT-proBNP) level above and below the proposed cut-off. Methods Stable patients (n = 30) with left ventricular (LV) ejection fraction ≥ 50% were eligible if they had a diagnosis of HF according to the European Society of Cardiology diagnostic criteria. Characteristics of patients with NT-proBNP below (≤125 pg/mL) and above (\u3e125 pg/mL) the diagnostic criterion were compared. Results There were 19 (66%) women with median age 54 years. Half were African American (16, 53%), and most were obese. There were no significant differences in clinical characteristics or medication use between groups. LV end-diastolic volume index was greater in high NT-proBNP patients (P = 0.03). Left atrial volume index, E/e\u27 ratio, and E/e\u27 ratio at peak exercise were not significantly different between NT-proBNP groups. Peak oxygen consumption (VO2), VO2 at ventilatory threshold, and ventilatory efficiency measures were impaired in all patients and were not significantly different between high and low NT-proBNP patients. Conclusions NT-proBNP was below the proposed diagnostic cut-off point of 125 pg/mL in half of this obese study cohort. Cardiac diastolic dysfunction and cardiorespiratory fitness were not significantly different between high and low NT-proBNP patients. These data indicate that excluding the diagnosis of HFpEF based solely on NT-proBNP levels should be discouraged

    Random forests highlight the combined effect of environmental heavy metals exposure and genetic damages for cardiovascular diseases

    Heavy metals are a dangerous source of pollution due to their toxicity, permanence in the environment and chemical nature. It is well known that long-term exposure to heavy metals is related to several chronic degenerative diseases (cardiovascular diseases, neoplasms, neurodegenerative syndromes, etc.). In this work, we propose a machine learning framework to evaluate the severity of cardiovascular diseases (CVD) from Human scalp hair analysis (HSHA) tests and genetic analysis and identify a small group of these clinical features mostly associated with the CVD risk. Using a private dataset provided by the DD Clinic foundation in Caserta, Italy, we cross-validated the classification performance of a Random Forests model with 90 subjects affected by CVD. The proposed model reached an AUC of 0.78 ± 0.01 on a three class classification problem. The robustness of the predictions was assessed by comparison with different cross-validation schemes and two state-ofthe-art classifiers, such as Artificial Neural Network and General Linear Model. Thus, is the first work that studies, through a machine learning approach, the tight link between CVD severity, heavy metal concentrations and SNPs. Then, the selected features appear highly correlated with the CVD phenotype, and they could represent targets for future CVD therapies

    Copper accumulation in vineyard soils: Rhizosphere processes and agronomic practices to limit its toxicity.

    Viticulture represents an important agricultural practice in many countries worldwide. Yet, the continuous use of fungicides has caused copper (Cu) accumulation in soils, which represent a major environmental and toxicological concern. Despite being an important micronutrient, Cu can be a potential toxicant at high concentrations since it may cause morphological, anatomical and physiological changes in plants, decreasing both food productivity and quality. Rhizosphere processes can, however, actively control the uptake and translocation of Cu in plants. In particular, root exudates affecting the chemical, physical and biological characteristics of the rhizosphere, might reduce the availability of Cu in the soil and hence its absorption. In addition, this review will aim at discussing the advantages and disadvantages of agronomic practices, such as liming, the use of pesticides, the application of organic matter, biochar and coal fly ashes, the inoculation with bacteria and/or mycorrhizal fungi and the intercropping, in alleviating Cu toxicity symptoms

    Optimal stent design for high bleeding risk patients: Evidence from a network meta-analysis

    Objective. To determine the best stent design for high bleeding risk (HBR) patients. Background. Polymer-free (PF) drug eluting stent (DES) devices have a proven benefit over bare-metal stent (BMS) devices in previous trials. It is unknown, however, whether polymer-based (PB)-DES devices are as safe as PF-DES devices. Methods. A network meta-analysis including all randomized controlled trials (RCTs) that compared different stent technology in HBR patients with a 1-month course of dual-antiplatelet therapy (DAPT) was performed. The main efficacy outcome was major adverse cardiac event (MACE) rate, defined as the composite of all-cause mortality, myocardial infarction (MI), and target-lesion revascularization (TLR). Secondary efficacy events included all-cause and cardiac mortality, MI, stroke, TLR, and target-vessel revascularization (TVR). Safety outcomes included all bleeding, major bleeding, and stent thrombosis (ST). Results. A total of 4 RCTs with 6456 patients were included. PF-DES and PB-DES yielded a reduced rate of MACE, MI, TLR, and TVR events compared with BMS (all P<.05). ST events were reduced in PB-DES compared with BMS (P=.01). No differences were found in all-cause death, cardiac death, or stroke events in PF-DES and PB-DES compared with BMS. Furthermore, no differences were found between PF-DES and PB-DES regarding any of the outcomes. Conclusion. DES devices were associated with lower MACE and TVR rates compared with BMS, whereas there were no statistical differences in other efficacy endpoints. Also, PB-DES were associated with fewer ST events compared with BMS.     Sparse Exploratory Factor Analysis

    Sparse principal component analysis is a very active research area in the last decade. It produces component loadings with many zero entries which facilitates their interpretation and helps avoid redundant variables. The classic factor analysis is another popular dimension reduction technique which shares similar interpretation problems and could greatly benefit from sparse solutions. Unfortunately, there are very few works considering sparse versions of the classic factor analysis. Our goal is to contribute further in this direction. We revisit the most popular procedures for exploratory factor analysis, maximum likelihood and least squares. Sparse factor loadings are obtained for them by, first, adopting a special reparameterization and, second, by introducing additional [Formula: see text]-norm penalties into the standard factor analysis problems. As a result, we propose sparse versions of the major factor analysis procedures. We illustrate the developed algorithms on well-known psychometric problems. Our sparse solutions are critically compared to ones obtained by other existing methods
