    Word retrieval in bilingual individuals: how do language, test type, and self-reported fluency relate to naming accuracy in English and Hebrew

    Thesis (M.S.)--Boston UniversityBackground: Although research on lexical access in Hebrew-English bilinguals exists, there is a need to quantify and determine if proficiency correlates with scores on language tasks like naming in structurally different languages, such as Hebrew and English. Objective: The aim of this study is to determine if language, test type, and self-reported proficiency is reflected in Hebrew-English bilinguals' naming ability in the two languages. Methods: Twenty healthy Hebrew-English bilinguals completed the Mini Mental State Examination (MMSE), the Language Use Questionnaire (LUQ), the Boston Naming Test (BNT), a Hebrew naming test (Kave, 2005), a 100-item category picture naming task, and category generation in both languages. Results: There was a significant effect of language and test type on accuracy scores on picture naming tests. While language did not have a significant effect on category generation, category had a significant effect on correct words, means semantic cluster score, and mean semantic switching score. There were several language proficiency variables that were significantly correlated with naming tasks in English (Language Ability Rating (LAR), Confidence, Lifetime Exposure, Education History), yet LUQ proficiency variables were only significantly correlated with Kave's test (LAR, Education History) on the Hebrew naming tasks. Conclusion: It appears that confrontation-naming assessments result in more variable performance in English and Hebrew than fluency tests. Based on correlations between LUQ variables and Hebrew naming performance, Kave's test appears to be the only test that captures the unique aspects that allow individuals to be proficient in Hebrew. The BNT is a useful measure for examining English naming. Therefore, using the BNT and Kave's Hebrew naming test appears to be the most effective in measuring naming performance in Hebrew-English bilinguals. Significant LUQ variables in English and Hebrew indicate that self-reported acquisition patterns and lifetime usage provide valuable information regarding naming performance

    Auditory naming is impaired in posterior cortical atrophy and early-onset Alzheimer’s disease

    IntroductionVisual naming ability reflects semantic memory retrieval and is a hallmark deficit of Alzheimer’s disease (AD). Naming impairment is most prominently observed in the late-onset amnestic and logopenic variant Primary Progressive Aphasia (lvPPA) syndromes. However, little is known about how other patients across the atypical AD syndromic spectrum perform on tests of auditory naming, particularly those with primary visuospatial deficits (Posterior Cortical Atrophy; PCA) and early onset (EOAD) syndromes. Auditory naming tests may be of particular relevance to more accurately measuring anomia in PCA syndrome and in others with visual perceptual deficits.MethodsForty-six patients with biomarker-confirmed AD (16 PCA, 12 lvPPA, 18 multi-domain EOAD), at the stage of mild cognitive impairment or mild dementia, were administered the Auditory Naming Test (ANT). Performance differences between groups were evaluated using one-way ANOVA and post-hoc t-tests. Correlation analyses were used to examine ANT performance in relation to measures of working memory and word retrieval to elucidate cognitive mechanisms underlying word retrieval deficits. Whole-cortex general linear models were generated to determine the relationship between ANT performance and cortical atrophy.ResultsBased on published cutoffs, out of a total possible score of 50 on the ANT, 56% of PCA patients (mean score = 45.3), 83% of EOAD patients (mean = 39.2), and 83% of lvPPA patients (mean = 29.8) were impaired. Total uncued ANT performance differed across groups, with lvPPA performing most poorly, followed by EOAD, and then PCA. ANT performance was still impaired in lvPPA and EOAD after cuing, while performance in PCA patients improved to the normal range with phonemic cues. ANT performance was also directly correlated with measures of verbal fluency and working memory, and was associated with cortical atrophy in a circumscribed semantic language network.DiscussionAuditory confrontation naming is impaired across the syndromic spectrum of AD including in PCA and EOAD, and is likely related to auditory-verbal working memory and verbal fluency which represent the nexus of language and executive functions. The left-lateralized semantic language network was implicated in ANT performance. Auditory naming, in the absence of a visual perceptual demand, may be particularly sensitive to measuring naming deficits in PCA

    Neural substrates of verbal repetition deficits in primary progressive aphasia.

    In this cross-sectional study, we examined the relationship between cortical thickness and performance on several verbal repetition tasks in a cohort of patients with primary progressive aphasia in order to test predictions generated by theoretical accounts of phonological working memory that predict phonological content buffers in left posterior inferior frontal sulcus and supramarginal gyrus. Cortical surfaces were reconstructed from magnetic resonance imaging scans from 42 participants diagnosed with primary progressive aphasia. Cortical thickness was measured in a set of anatomical regions spanning the entire cerebral cortex. Correlation analyses were performed between cortical thickness and average score across three phonological working memory-related tasks: the Repetition sub-test from the Western Aphasia Battery, a forward digit span task, and a backward digit span task. Significant correlations were found between average working memory score across tasks and cortical thickness in left supramarginal gyrus and left posterior inferior frontal sulcus, in support of prior theoretical accounts of phonological working memory. Exploratory whole-brain correlation analyses performed for each of the three behavioural tasks individually revealed a distinct set of positively correlated regions for each task. Comparison of cortical thickness measures from different primary progressive aphasia sub-types to cortical thickness in age-matched controls further revealed unique patterns of atrophy in the different subtypes.R01 DC007683 - NIDCD NIH HHSPublished versio

    Protein profiling of the dimorphic, pathogenic fungus, Penicillium marneffei

    <p>Abstract</p> <p>Background</p> <p><it>Penicillium marneffei </it>is a pathogenic fungus that afflicts immunocompromised individuals having lived or traveled in Southeast Asia. This species is unique in that it is the only dimorphic member of the genus. Dimorphism results from a process, termed phase transition, which is regulated by temperature of incubation. At room temperature, the fungus grows filamentously (mould phase), but at body temperature (37°C), a uninucleate yeast form develops that reproduces by fission. Formation of the yeast phase appears to be a requisite for pathogenicity. To date, no genes have been identified in <it>P. marneffei </it>that strictly induce mould-to-yeast phase conversion. In an effort to help identify potential gene products associated with morphogenesis, protein profiles were generated from the yeast and mould phases of <it>P. marneffei</it>.</p> <p>Results</p> <p>Whole cell proteins from the early stages of mould and yeast development in <it>P. marneffei </it>were resolved by two-dimensional gel electrophoresis. Selected proteins were recovered and sequenced by capillary-liquid chromatography-nanospray tandem mass spectrometry. Putative identifications were derived by searching available databases for homologous fungal sequences. Proteins found common to both mould and yeast phases included the signal transduction proteins cyclophilin and a RACK1-like ortholog, as well as those related to general metabolism, energy production, and protection from oxygen radicals. Many of the mould-specific proteins identified possessed similar functions. By comparison, proteins exhibiting increased expression during development of the parasitic yeast phase comprised those involved in heat-shock responses, general metabolism, and cell-wall biosynthesis, as well as a small GTPase that regulates nuclear membrane transport and mitotic processes in fungi. The cognate gene encoding the latter protein, designated <it>RanA</it>, was subsequently cloned and characterized. The <it>P. marneffei </it>RanA protein sequence, which contained the signature motif of Ran-GTPases, exhibited 90% homology to homologous <it>Aspergillus </it>proteins.</p> <p>Conclusion</p> <p>This study clearly demonstrates the utility of proteomic approaches to studying dimorphism in <it>P. marneffei</it>. Moreover, this strategy complements and extends current genetic methodologies directed towards understanding the molecular mechanisms of phase transition. Finally, the documented increased levels of RanA expression suggest that cellular development in this fungus involves additional signaling mechanisms than have been previously described in <it>P. marneffei</it>.</p

    Relearning and Retaining Personally-Relevant Words using Computer-Based Flashcard Software in Primary Progressive Aphasia

    Although anomia treatments have often focused on training small sets of words in the hopes of promoting generalization to untrained items, an alternative is to directly train a larger set of words more efficiently. The current case report study reports on a novel treatment for a patient with semantic variant Primary Progressive Aphasia (svPPA), in which the patient was taught to make and practice flashcards for personally-relevant words using an open-source computer program (Anki). Results show that the patient was able to relearn and retain a large subset of her studied words over a 20-month period. At the end of treatment, she showed good retention for 139 studied words, far more than the number typically treated in svPPA studies. Furthermore, she showed evidence of stimulus generalization to confrontation-naming tasks for studied items, and of relearning forgotten items with additional practice. This case represents a successful example of patient-centered computer-based asynchronous telepractice. It also illustrates how data captured from computer-based treatments can provide powerful practice-based evidence, obtained during routine clinical care
