Vertical migration of the crab Carcinus maenas first zoea in an estuary: implications for tidal stream transport
The first zoea of Carcinus maenas (L.) was intensively sampled in the Canal de Mira (Ria de Aveiro, Portugal) during the winter and spring of 1990. Each sampling period included a series of 25 h fixed-station plankton sampling cycles (12 in winter and 9 in spring), conducted at 1 station located in the lower part of the Canal. Plankton samples were collected every hour, at several depths along the water column, with the use of a pump. Hydrological measurements (salinity, temperature and current velocity and direction) were taken immediately before the collection of the plankton samples. The average depth of the zoeae changed in phase with the tide: larvae reached their highest position in the water column during ebb and their lowest during flood. The extent and phasing of the vertical displacements were such that the first zoea occupied a significantly higher position during the span of the ebb than during flood (p < 0.001). It was also demonstrated that the larvae were significantly closer to the surface during the night (p < 0.05). The pattern of vertical dispersion of the zoeae changed cyclically, with a period equivalent to the tidal half-cycle. Maximum aggregation usually occurred during periods of high current velocity and was independent from water stratification. These observations support the hypothesis that C. maenas first zoea performs an active vertical migration synchronised with tidal and daily cycles. Analysis of the zoeae instantaneous velocity showed that shifts of vertical position according to the phases of tide and day influenced their transport velocity, due to vertical differences of water velocity in the estuarine shear current system. During ebb the larvae were transported at a velocity that exceeded the vertically integrated water velocity; the reverse relationship was observed during flood (p < 0.001). The larvae were also transported faster during the night than during the day (p < 0.05). These results demonstrate that selective tidal stream transport can be generalised in the sense of a unidirectional transport mechanism in estuaries that can enhance exportation
Aeromagnetometry and aerogammaspectrometry integrated with U-Pb zircon geochronology of northern Bossoroca ophiolite, Brasiliano Orogen
Age delimitation integrated with aeromagnetometric and aerogammaspectrometric survey advances the understanding of ophiolite evolution in the Brasiliano Orogen. We focused on the Bossoroca ophiolite, because oceanic crustal and mantle rocks contain zircon in metasomatic chloritite. A metadiorite and a metavolcanoclastic rock were also studied to delimit relationship between ophiolite and island-arc infrastructure and superstructure. Zircon crystals were dated by laser ablation inductively coupled plasma emission spectroscopy. Ages of zircon from Campestre metavolcanoclastic rock are 920-840 (peak 842) Ma, Bossoroca chloritite 900-800 (peak 868 Ma) and Capivaras metadiorite 698 Ma. Ages 920-800 Ma correspond to processes in the oceanic crust, whereas 698 Ma was a late magmatic intrusion (Capivaras metadiorite) into the island-arc infrastructure. Aeromagnetometric and aerogammaspectrometric data delimit the occurrence and structure of the ophiolite. These are major multiproxy markers of geotectonic processes early in the Brasiliano Orogen
Low clonal propagation in Atlantic and Mediterranean populations of the red gorgonian Paramuricea clavata (Octocorallia)
Clonal propagation is a common feature of benthic marine organisms. In the present study, we investigated the contribution of clonal reproduction in the red gorgonian Paramuricea clavata. Mediterranean populations of P. clavata were severely affected by mass mortality events caused by increased water temperature in 1999 and 2003. The populations are characterized by slow growth and episodic recruitment, but after the observed mortalities, an unexpectedly high recovery rate was observed in the severely affected populations from the Ligurian Sea, NW Mediterranean. Ten years after the last mortality event, we investigated the contribution of clonal propagation in populations from the Ligurian Sea, where some populations were highly affected by mass mortality events, and from the Atlantic, where mortality was never observed. All individuals were genotyped for nine microsatellite loci. The contribution of clonal reproduction varied from 0% to 13% and did not differ significantly between affected and unaffected populations. A Protocol for the Prospective Evaluation of Novel Suction-Based Airway Clearance Devices in the Treatment of Foreign Body Airway Obstructions
Background Foreign body airway obstructions (FBAOs, choking) are a significant cause of preventable mortality. Abdominal thrusts, back blows, and chest compressions are traditional interventions. However, suction-based airway clearance devices (ACDs) have recently been marketed as an alternative. Of note, there is limited published evidence regarding their efficacy and safety. Our research has two aims: (1) to investigate what situational and patient factors are frequently identified, and which are associated with relief of the FBAO and survival in individuals with FBAOs treated with an ACD; and (2) to describe the experience of individuals who have used ACDs in response to a FBAO and identify facilitators and barriers to the use of ACDs compared to traditional interventions. Methods and analysis A prospective database will be developed using an online reporting system to capture ACD uses, independent of manufacturers, from July 1st, 2021 to December 31st, 2023. Descriptive statistics will be used to summarize cases, outcomes, and adverse events. Clinically important subgroups will be stratified for analysis, including the severity of obstruction, patient demographics, and training of ACD users. Semi-structured interviews will also be conducted with a subset of ACD users to describe in detail their experience using the device. Themes from these interviews will be assessed using the theoretical domains framework. Discussion This study will improve the evidence surrounding ACDs and compare it to current data for traditional techniques, with the aim of optimizing FBAO treatment. Data on ACDs are urgently needed as these devices are already being used by parents, caregivers, lay rescuers, and healthcare professionals to respond to choking emergencies. This evaluation will provide important information about their effectiveness and any safety concerns which can inform the public, resuscitation guidelines, and future research studies
A 2-year prospective evaluation of airway clearance devices in foreign body airway obstructions
Aim: To collect, analyze and report the first prospective, industry-independent, data on airway clearance devices as novel foreign body airway obstruction interventions. Methods: We recruited adult airway clearance device users between July 1, 2021 and June 30, 2023 using a centralized website and email follow-up. The data collection tool captured patient, responder, situation, and outcome variables. Multi-step respondent validation occurred using electronic and geolocation verification, a random selection follow-up process, and physician review of all submitted cases. Results: We recruited 186 airway clearance device users (LifeVac©:157 [84.4%]; Dechoker©:29 [15.6%]). LifeVac© was the last intervention before foreign body airway obstruction relief in 151 of 157 cases. Of these, 150 survived to discharge. A basic life support intervention was used before LifeVac© in 119 cases, including the 6 cases where LifeVac© also failed. We identified two adverse events using LifeVac© (perioral bruising), while we could not ascertain whether another 7 were due to the foreign body or LifeVac© (3 = airway edema; 3 = oropharyngeal abrasions; 1 = esophageal perforation). Dechoker© was the last intervention before obstruction relief in 27 of 29 cases and all cases survived. A basic life support intervention was used before Dechoker© in 21 cases, including both where Dechoker© also failed. We identified one adverse event using Dechoker© (oropharyngeal abrasions). Conclusion: Within these cases, airway clearance devices appear to be effective at relieving foreign body airway obstructions. However, this data should be considered preliminary and hypothesis generating due to several limitations. Resuscitation of Drowned Persons during the COVID-19 Pandemic: A Consensus Statement
Importance: Resuscitation is a niche example of how the COVID-19 pandemic has affected society in the long term. Those trained in cardiopulmonary resuscitation (CPR) face the dilemma that attempting to save a life may result in their own harm. This is most of all a problem for drowning, where hypoxia is the cause of cardiac arrest and ventilation is the essential first step in reversing the situation. Objective: To develop recommendations for water rescue organizations in providing their rescuers with safe drowning resuscitation procedures during the COVID-19 pandemic. Evidence Review: Two consecutive modified Delphi procedures involving 56 participants from 17 countries with expertise in drowning prevention research, resuscitation, and programming were performed from March 28, 2020, to March 29, 2021. In parallel, PubMed and Google Scholar were searched to identify new emerging evidence relevant to each core element, acknowledge previous studies relevant in the new context, and identify knowledge gaps. Findings: Seven core elements, each with their own specific recommendations, were identified in the initial consensus procedure and were grouped into 4 categories: (1) prevention and mitigation of the risks of becoming infected, (2) resuscitation of drowned persons during the COVID-19 pandemic, (3) organizational responsibilities, and (4) organizations unable to meet the recommended guidelines. The common measures of infection risk mitigation, personal protective equipment, and vaccination are the base of the recommendations. Measures to increase drowning prevention efforts reduce the root cause of the dilemma. Additional infection risk mitigation measures include screening all people entering aquatic facilities, defining criteria for futile resuscitation, and avoiding contact with drowned persons by rescuers with a high-risk profile. Ventilation techniques must balance required skill level, oxygen delivery, infection risk, and costs of equipment and training. Bag-mask ventilation with a high-efficiency particulate air filter by 2 trained rescuers is advised. Major implications for the methods, facilities, and environment of CPR training have been identified, including nonpractical skills to avoid being infected or to infect others. Most of all, the organization is responsible for informing their members about the impact of the COVID-19 pandemic and taking measures that maximize rescuer safety. Research is urgently needed to better understand, develop, and implement strategies to reduce infection transmission during drowning resuscitation. Conclusions and Relevance: This consensus document provides an overview of recommendations for water rescue organizations to improve the safety of their rescuers during the COVID-19 pandemic and balances the competing interests between a potentially lifesaving intervention and risk to the rescuer. The consensus-based recommendations can also serve as an example for other volunteer organizations and altruistic laypeople who may provide resuscitation.. © 2022 American Medical Association. Biometrics, physiology, production, and quality of Cantaloupe melons grown with saline waters under semi-arid conditions
Brazilian Northeast region emerges as both producer and exporter relevant of cantaloupe melon to several countries; however, this region is vulnerable to salinization problems due to it is inserted in a semi-arid area. This study aimed to evaluate the biometrics, physiological, production and quality of new cantaloupe melon hybrids cultivated with different saline waters under semi-arid conditions. The experiment was conducted using a drip irrigation system in a split-plot randomized design. The plot was different irrigation water electrical conductivity (ECw) levels: 0.5, 2.0, 3.5, and 5.0 dS m-1, while the subplot was two new cantaloupe melon hybrids: Zielo and SV1044. Plant biometrics (stems and leaves analysis), plant physiology (solutes accumulation and gas exchange in leaves), fruit production (number, weight, and yield), and fruit quality (physical and chemical analysis) were investigated. Stem length, leaf numbers, leaf area, and shoot dry mass decreased when ECw increased from 0.5 to 5.0 dS m-1. Also, there was an increase of Na and Cl concentration and reduction of K concentration in leaves due to ECw increase. Gas exchange reduction, losses in fruit numbers, weight, yield, and in quality were observed with the ECw increase. It is possible to conclude that the ECw increase salinizes the soil and increases Na and Cl concentration in plants causing reduction in gas exchange, in growth, and in fruit production and quality. Role of Suction-Based Airway Clearance Devices in Pediatric Foreign Body Airway Obstruction
Introduction Foreign body airway obstruction (FBAO) represents a significant cause of morbidity and mortality in the pediatric population. Factors including pediatric anatomy, chewing and swallowing difficulties, developmental stages, habits and behaviors, and children’s toys and foods increase risk of FBAO in children.1,2 BLS guidelines for FBAO management in responsive children ages 1-8 call for a series of subdiaphragmatic abdominal thrusts, which increase intrathoracic pressure to force air and the foreign body out of the airway.3,4 Abdominal thrusts have higher reports of injury compared to all other FBAO interventions, with risk of thoracic, vascular, and gastro-esophageal injury.5-7 Novel suction-based airway clearance devices (ACDs), including LifeVac and Dechoker, are non-powered, externally applied, and utilize a negative pressure system to remove an obstruction from the airway.6,8 They are straight-forward to use and less invasive than abdominal thrusts. Effectiveness, cost-utility, and benefits of a multicomponent therapy to improve the quality of life of patients with fibromyalgia in primary care : a mixed methods study protocol
Introduction:Fibromyalgia (FM) is a chronic condition characterized by chronic pain, fatigue and loss of function which significantly impairs quality of life. Although treatment of FM remains disputed, some studies point at the efficacy of interdisciplinary therapy. This study aims to analyze the effectiveness, cost-utility and benefits of a multicomponent therapy on quality of life (main variable), functional impact, mood and pain in people suffering from FM that attend primary care centers (PCCs) of the Catalan Institute of Health (ICS).Methods and analysis:A 2-phase, mixed methods study has been designed following Medical Research Council guidance. Phase 1: Pragmatic randomized clinical trial with patients diagnosed with FM that attend one of the 11 PCCs of the ICS Gerència Territorial Terres de l'Ebre. We estimate a total sample of 336 patients. The control group will receive usual clinical care, while the multicomponent therapy group (MT group) will receive usual clinical care plus group therapy (consisting of health education, exercise and cognitive-behavioural therapy) during 12 weeks in 2-hourly weekly sessions. Analysis: the standardized mean response and the standardized effect size will be assessed at 3, 9, and 15 months after the beginning of the study using multiple linear regression models. Utility measurements will be used for the economic analysis. Phase 2: Qualitative socio constructivist study to evaluate the intervention according to the results obtained and the opinions and experiences of participants (patients and professionals). We will use theoretical sampling, with 2 discussion groups of participants in the multicomponent therapy and 2 discussion groups of professionals of different PCCs. A thematic content analysis will be carried out.Ethics and dissemination:This study protocol has been approved by the Clinical Research Ethics Committee of the Fundació Institut Universitari per a la recerca a l'Atenció Primà ria de Salut Jordi Gol i Gurina (code P18/068).
