    Generating inferences from written and spoken language: a comparison of children with visual impairment and children with sight

    The two experiments reported here investigated the ability of sighted children and children with visual impairment to comprehend text and, in particular, to draw inferences both while reading and while listening. Children were assigned into `comprehension skill' groups, depending on the degree to which their reading comprehension skill was in line with that predicted by their decoding skill. They then read (either print or Braille) and listened to a series of novel short stories. These were followed by a series of questions, which required either the generation of inferences, or an answer that could be taken literally from the text. The results suggest that children with and without sight are comparable in their ability to draw inferences, and that children with visual impairment show an advantage for literal questions under auditory presentation

    Micron- to nano-scale intergrowths among members of the cuprobismutite series and paderaite: HRTEM and microanalytical evidence

    Copyright © 2004 The Mineralogical SocietyCoherent intergrowths, at the lattice scale, between cuprobismutite (N = 2) and structurally related padraite along both major axes (15 Åand 17 Årepeats) of the two minerals are reported within skarn from Ocna de Fier, Romania. The structural subunit, DTD, 3 layers of padraite, is involved at interfaces of the two minerals along the 15 Årepeat, as well as in transposition of 1 padraite unit to 2 cuprobismutite units along the 17 Årepeat in slip defects. Lattice images obtained by HRTEM across intervals of 200 -400 nm show short- to long-range stacking sequences of cuprobismutite and padraite ribbons. Such nanoscale slabs mimic µm-scale intergrowths observed in back-scattered electron images at three orders of magnitude greater. These slabs are compositionally equivalent to intermediaries in the cuprobismutite-padraite range encountered during microanalysis. Hodrushite (N = 1.5) is identified in the µm-scale intergrowths, but its absence in the lattice images indicates that, in this case, formation of polysomes between structurally related phases is favoured instead of stacking disorder among cuprobismutite homologues. The tendency for short-range ordering and semi-periodic occurrence of polysomes suggests they are the result of an oscillatory chemical signal with periodicity varying from one to three repeats of 15 Å, rather than simple 'accidents' or irregular structural defects. Lead distribution along the polysomes is modelled as an output signal modulated by the periodicity of stacking sequences, with Pb carried within the D units of padraite. This type of modulator acts as a patterning operator activated by chemical waves with amplitudes that encompass the chemical difference between the minerals. Conversion of the padraite structural subunit DTD to the C unit of cuprobismutite, conserving interval width, emphasizes that polysomatic modularity also assists interference of chemical signals with opposite amplitudes. Observed coarsening of lattice-scale intergrowths up to the µm-scale implies coupling between diffusion-controlled structural modulation, and rhythmic precipitation at the skarn front during crystallization.C.L.     Composition-induced structural phase transitions in the (Ba1xLax)2In2O5+x (0pxp0.6) system

    Composition-induced structural phase changes across the high temperature, fast oxide ion conducting (Ba1xLax)2In2O5+x, 0pxp0.6, system have been carefully analysed using hard mode infrared (IR) powder absorption spectroscopy, X-ray powder diffraction and electron diffraction. An orthorhombic brownmillerite to three-dimensionally disordered cubic perovskite phase transition in this system is signalled by a drastic change in slope of both wavenumber and average line widths of IR spectra as a function of composition. Some evidence is found for the existence of an intermediate tetragonal phase (previously reported to exist from electron diffraction data) around x 0:2: The new spectroscopic data have been used to compare microscopic and macroscopic strain parameters arising from variation in composition. The strain and spectroscopic data are consistent with firstorder character for the tetragonal-orthorhombic transition, while the cubic-tetragonal transition could be continuous. Differences between the variation with composition of spectral parameters and of macroscopic strain parameters are consistent with a substantial order/disorder component for the transitions. There is also evidence for precursor effects within the cubic structure before symmetry is broken

    Effectiveness and Challenges: The Current Situation of Electronic Health Records (EHRs) to Improve Health Care Outcomes

    Background. In the Philippines, although software for Electronic Health Records (EHRs) is still new, knowledge about EHRs benefits is increasing.Objective: This study investigated the effectiveness of electronic health records to health care outcomes at selected health facilities in the National Capital Region (NCR) and CALABARZON regions of the Philippines. The effectiveness of the current EHRs in government and private health facilities were determined in terms of quality patient care, patient engagement, support team-based care, workload, and data liquidity; and the challenges facing health providers in the implementation of electronic health records were identified.Methods. The research utilized the descriptive, cross-sectional design. Three hundred seven (N=307) health providers from the medical, nursing, and paramedical departments were the study participants. A three-part adapted questionnaire was the main tool used for data collection. Mean, standard deviation, and Mann-Whitney & Kruskal-Wallis’s tests were utilized for data analysis.Results. The results revealed that in the Philippines, EHRs are moderately effective in improving health care outcomes. EHRs effectiveness is primarily achieved through better access and retrieval of health information. EHRs implementation poses major challenges in the Philippines’ health facilities which are central to socio-economic and organizational factors.Conclusions. The more effective the use of EHR systems, the greater the challenges experienced by care providers and health facilities

    Assessment of the 2020 NICE criteria for preoperative radiotherapy in patients with rectal cancer treated by surgery alone in comparison with proven MRI prognostic factors: a retrospective cohort study

    BACKGROUND: Selection of patients for preoperative treatment in rectal cancer is controversial. The new 2020 National Institute for Health and Care Excellence (NICE) guidelines, consistent with the National Comprehensive Cancer Network guidelines, recommend preoperative radiotherapy for all patients except for those with radiologically staged T1-T2, N0 tumours. We aimed to assess outcomes in non-irradiated patients with rectal cancer and to stratify results on the basis of NICE criteria, compared with known MRI prognostic factors now omitted by NICE. METHODS: For this retrospective cohort study, we identified patients undergoing primary resectional surgery for rectal cancer, without preoperative radiotherapy, at Basingstoke Hospital (Basingstoke, UK) between Jan 1, 2011, and Dec 31, 2016, and at St Marks Hospital (London, UK) between Jan 1, 2007, and Dec 31, 2017. Patients with MRI-detected extramural venous invasion, MRI-detected tumour deposits, and MRI-detected circumferential resection margin involvement were categorised as MRI high-risk for recurrence (local or distant), and their outcomes (disease-free survival, overall survival, and recurrence) were compared with patients defined as high-risk according to NICE criteria (MRI-detected T3+ or MRI-detected N+ status). Kaplan-Meier and Cox proportional hazards analyses were used to compare the groups. FINDINGS: 378 patients were evaluated, with a median of 66 months (IQR 44-95) of follow up. 22 (6%) of 378 patients had local recurrence and 68 (18%) of 378 patients had distant recurrence. 248 (66%) of 378 were classified as high-risk according to NICE criteria, compared with 121 (32%) of 378 according to MRI criteria. On Kaplan-Meier analysis, NICE high-risk patients had poorer 5-year disease-free survival compared with NICE low-risk patients (76% [95% CI 70-81] vs 87% [80-92]; hazard ratio [HR] 1·91 [95% CI 1·20-3·03]; p=0·0051) but not 5-year overall survival (80% [74-84] vs 88% [81-92]; 1·55 [0·94-2·53]; p=0·077). MRI criteria separated patients into high-risk versus low-risk groups that predicted 5-year disease-free survival (66% [95% CI 57-74] vs 88% [83-91]; HR 3·01 [95% CI 2·02-4·47]; p<0·0001) and 5-year overall survival (71% [62-78] vs 89% [84-92]; 2·59 [1·62-3·88]; p<0·0001). On multivariable analysis, NICE risk assessment was not associated with either disease-free survival or overall survival, whereas MRI criteria predicted disease-free survival (HR 2·74 [95% CI 1·80-4·17]; p<0·0001) and overall survival (HR 2·44 [95% CI 1·51-3·95]; p=0·00027). 139 NICE high-risk patients who were defined as low-risk based on MRI criteria had similar disease-free survival as 118 NICE low-risk patients; therefore, 37% (139 of 378) of patients in this study cohort would have been overtreated with NICE 2020 guidelines. Of the 130 patients defined as low-risk by NICE guidelines, 12 were defined as high-risk on MRI risk stratification and would have potentially been missed for treatment. INTERPRETATION: Compared to previous guidelines, implementation of the 2020 NICE guidelines will result in significantly more patients receiving preoperative radiotherapy. High-quality MRI selects patients with good outcomes (particularly low local recurrence) without radiotherapy, with little margin for improvement. Overuse of radiotherapy could occur with this unselective approach. The high-risk group, with the most chance of benefiting from preoperative radiotherapy, is not well selected on the basis of NICE 2020 criteria and is better identified with proven MRI prognostic factors (extramural venous invasion, tumour deposits, and circumferential resection margin).     Thermal expansion of troilite and pyrrhotite determined by in situ cooling (873 to 373 K) neutron powder diffraction measurements

    The thermal expansion coefficients for natural troilite, FeS, Ni-rich pyrrhotite, Fe0.84Ni0.11S, and Ni-poor pyrrhotite, Fe0.87Ni0.02S, were measured during cooling by in situ neutron powder diffraction over the temperature range 873–373 K. Between 873 and 573 K, the mean thermal expansion coefficients for the three compositions are 7.4(3) × 10-5 {FeS}, 8.0(4) × 10-5 {Fe0.84Ni0.11S} and 8.5(4) × 10-5 K–1 {Fe0.87Ni0.02S}. Below 573 down to 373 K, the first two increase considerably to 14.1(7) × 10-5 {FeS} and 9.3(5) × 10-5 {Fe0.84Ni0.11S} while the latter sample shows no significant variation, 8.4(5) × 10-5 K-1. Below 573 K, the thermal expansion is highly anisotropic, with Deltaa/100 K-1 ranging from 0.89(9)% {FeS} to 0.48(12)% {Fe0.87Ni0.02S} while Deltac/100 K-1 ranges from -0.39(11)% {FeS} to -0.13(2)% {Fe0.87Ni0.02S}. Upon cooling through 573 K, troilite and pyrrhotite undergo a transition where the FeS6 octahedra distort and in the case of pyrrhotite, cation-vacancy clustering occurs. The thermal expansion coefficients are bigger for low cation-vacancy concentrations and decrease as the pyrrhotites become less stoichiometric. This indicates that the thermal expansion in these minerals is damped by vacancy ordering or clustering. The thermal expansion coefficients for troilite and pyrrhotite are amongst the largest reported for sulphide minerals and their role in the formation of ore textures is discussed briefly
