However, if the items have different degrees of importance, they should be weighted differently before being compiled. The principal aims of this study were to show how the "analytic hierarchy process" (AHP), which has never been used for this purpose, can be applied to weighting the six items of the "London handicap scale", and to compare the AHP to the "conjoint analysis" (CA), which was previously implemented by Harwood et al. (1994) [1]. In order to assess the relative importance of the six items, we submitted AHP and CA to a group of 10 physiatrists. We compared the methods in terms of item ranking according to importance, assessment of fictitious patients based on weights determined by each method, and perceived difficulty by the physiatrist. For both techniques, "Physical independence" (PHY) was the best-weighted item, but other ranks varied depending on the technique. AHP was better than CA in terms of accuracy (global assessment of the clinical status) and perceived difficulty. AHP may be used to reveal the importance that experts assign to the items of a multidimensional scale, and to calculate the appropriate weights for specific items. For this purpose, AHP seems to be more accurate than CA

    Protection des travailleuses enceintes et des enfants à naître: ce que doit savoir le médecin

    Quarante-cinq pour cent de la population active suisse est constituée de femmes. Parmi celles-ci beaucoup auront à concilier vie professionnelle et grossesse. La grossesse n'est certes pas une maladie mais elle engendre des changements dans l'organisme de la femme qui rendent certains travaux plus pénibles, voire dangereux pour leur santé et celle de l'enfant à naître. Employeurs et médecins se doivent donc d'assurer à la travailleuse enceinte une grossesse sans danger. Pour ce faire, il existe des directives légales relatives à la loi sur le travail et portant sur la protection de la femme enceinte qui doivent être connues et appliquées. L'article résume ainsi cette problématique et propose aux médecins des sources d'informations pratiques tout en leur faisant part de l'utilité de faire appel au médecin du travail. 45% of workers in Switzerland are women who will sooner or later have to accomodate work and pregnancy. Pregnancy is not a disease but some occupational activities become more difficult and may cause health problems for the pregnant woman or the child to be born. Employers and medical doctors have to assure to the working pregnant women a pregnancy without work risks. Consequently they have to know the legal recommandations for the pregnancy protection which are noticed in the Swiss law. This article summarizes this topic in giving to medical doctors the necessary informations and advices, and explains the role of the occupational physicia

    Dans les pays industrialisés, 20% des salariés sont concernés par le travail de nuit. Pourtant, les effets du travail de nuit sur la santé sont peu traités par les revues médicales générales, alors que le travail de nuit a des conséquences non négligeables sur les systèmes cardiovasculaires et digestifs entre autres, comme l'ont démontré de nombreuses études ces dernières décennies. Le travail de nuit a encore récemment attiré l'attention quand il a été déclaré cancérigène probable (catégorie 2A) par le Centre international de recherches sur le cancer. Ainsi dans cet article, seront passés en revue les troubles de la santé qui peuvent être générés ou aggravés par le travail de nuit. Deux cas pratiques illustreront la problématique et permettront d'aborder la conduite à tenir. 20% of employees in industrialized countries are concerned by shift work. Shift work can have several major effects on health such as cardiovascular and digestive disorders among others, as demonstrated by several studies in recent decades.     Occupational asthma in workers exposed to Mesalazine powder inhalation

    Background: Due to complains of respiratory symptoms of some employees a pharmaceutical company asked in 2008 the occupational medical department of the Institute for Work and Health in Lausanne to evaluate the health status of their workers exposed to Mesalazine powder, which is the active agent of a drug used for the treatment of bowels inflammation. Therefore we examined the 21 workers exposed to Mesalazine powder. Method: After a visit of the pharmaceutical company in order to investigate the Mesalazine powder production, we performed an individual medical evaluation of the 21 workers. Our medical protocol was based on the safety data sheet of Mesalazine, the data found in the scientific literature and the «Compendium Suisse des Médicaments» and covered upper and lower respiratory tract as well as skin and eyes. Results: Sixty two percent (62%) of the exposed employees had symptoms of skin, eyes and throat irritation. Three employees reported respiratory symptoms such as dyspnoea, cough and expiratory wheezing, which appeared during the working hours. The Peak Flow series performed at the workplace was lowered in the three employees with lower respiratory tract symptoms. None of the three had consulted a physician, even though the symptoms had been present since some months. The pneumological evaluation confirmed for all three cases the asthma diagnoses. Conclusion: It is known that patients who are treated with drugs including Mesalazine can develop adverse health effect such as asthma. However occupational asthma in workers exposed to Mesalazine powder inhalation is until now not described in the literature. Immunologic investigations in order to know if the occupational asthma caused by Mesalazine is of allergic or mechanical irritation nature are still ongoing. Concerning the three workers with asthma, inability to work with Mesalazine was pronounced. Furthermore, the SUVA recognized the three patients with asthma as occupational respiratory diseases. Following our results and recommendations, the company undertook some measures to reduce the exposure to Mesalazine. A new health evaluation of the employees in the Mesalazine production is hence planned in 2009. As each year new causes of occupational asthma are described, the possible work relation of new asthma onset has to be carefully investigated as the consequences for the patient e.g. removal from exposure and for the exposed co-workers are of substantial importance

    Les Suisses travaillent... et consultent leur médecin. La Suisse est un pays de travailleurs: un des taux de chômage les plus bas du monde (moins de 5 %), 4,4 millions de personnes actives professionnellement sur 7,5 millions d'habitants... Et, heureusement, de travailleurs sains pour la plupart: selon une enquête de 2002, 85 % des Suisses de quinze ans et plus s'estimaient en bonne santé. Malgré cela, 77 % d'entre eux avaient tout de même eu recours à un médecin dans les douze mois précédant l'enquête. Si l'on ne retient que les personnes de 15 à 64 ans (donc professionnellement actives pour la plupart), la proportion se monte même à 81 %, ce qui représente près de deux millions de personnes ayant consulté au minimum une fois dans l'année. The notion of a cooperative effect between these two alterations has been demonstrated in respective mouse brain tumor models including our own. Here, we investigated underlying molecular mechanisms in early passage cortical astrocytes deficient for p16(INK4a)/p19(Arf) or p53, respectively, with or without ectopic expression of DeltaEGFR(2-7). Targeting these cells with the specific EGFR inhibitor tyrphostin AG1478 revealed that phosphorylation of ERK was only abrogated in the presence of an intact INK4a/Arf gene locus. The sensitivity to inhibit ERK phosphorylation was independent of ectopic expression of DeltaEGFR(2-7) and independent of the TP53 status. This resistance to downregulate the MAPK pathway in the absence of INK4a/Arf was confirmed in cell lines derived from our mouse glioma models with the respective initial genetic alterations. Thus, deletion of INK4a/Arf appears to keep ERK in its active, phosphorylated state insensitive to an upstream inhibitor specifically targeting EGFR/DeltaEGFR(2-7). This resistance may contribute to the cooperative tumorigenic effect selected for in human glioblastoma that may be of crucial clinical relevance for treatments specifically targeting EGFR/DeltaEGFR(2-7) in glioblastoma patients
