Rzadkie schorzenia hematologiczne u dzieci - histiocytoza komórek Langerhansa, mastocytoza. Możliwe obrazy kliniczne choroby
Histiocytoza komórek Langerhansa, a szczególnie mastocytoza, należą do rzadkich
schorzeń hematologicznych wieku dziecięcego. Właściwe rozpoznanie tych chorób często
bywa trudne.
Mnogość obrazów klinicznych histiocytozy związana jest z różnym stopniem proliferacji
i nagromadzenia w tkankach i narządach histiocytów Langerhansa. Cytokiny i prostaglandyny
uwalniane przez te komórki mogą doprowadzać do uszkodzenia zajętych procesem chorobowym
narządów. Najczęściej dotyczy to skóry, kości, wątroby, mózgu, krwi, śledziony,
grasicy, węzłów chłonnych, tkanek miękkich, szpiku kostnego i płuc.
W mastocytozie klonalny rozrost mastocytów w skórze lub narządach wewnętrznych może
powodować gwałtownie przebiegające reakcje z udziałem histaminy. Wstrząs anafilaktyczny
wywołany wieloma czynnikami stanowi bezpośrednie zagrożenie życia u chorych z mastocytozą.
Oba schorzenia, w agresywnej formie, wymagają leczenia cytoredukcyjnego
Ostatecznie gruźlica, a nie choroba nowotworowa. Prezentacja trzech przypadków pediatrycznych
Gruźlica wieku dziecięcego należy obecnie w krajach wysoko rozwiniętych do rzadkości.
W pracy zaprezentowano przypadki trojga dzieci z objawami nasuwającymi podejrzenie
choroby nowotworowej, u których ostatecznie dzięki interwencji chirurgicznej rozpoznano
gruźlicę (węzłowo-płucną, wielonarządową oraz ściany klatki piersiowej). Tylko
u jednej pacjentki uzyskano potwierdzenie bakteriologiczne. Objawy gruźlicy wymagają
różnicowania z chorobą nowotworową. Chirurg, który pobiera materiał diagnostyczny,
powinien pamiętać o wykonaniu badań bakteriologicznych w kierunku gruźlicy.

Nietypowy obraz kliniczny nowotworów wieku dziecięcego niejednokrotnie stwarza trudności
diagnostyczne i staje się przyczyną opóźnień w ustaleniu rozpoznania. Poniżej
przedstawiamy troje dzieci z rozpoznaniami guzów litych o nietypowym przebiegu klinicznym,
leczonych w Klinice Pediatrii, Hematologii, Onkologii i Endokrynologii Akademii

Ultrasound assessment of thrombotic complications in pediatric patients with tunneled central venous catheters
Central venous access consists in inserting a vascular catheter to the vena cava and placing
its tip in the vicinity of the opening to the right atrium. In the patients of the Clinic
of Pediatrics, Hematology and Oncology at the Academic Clinical Centre of the Medical
University in Gdańsk, such implantation procedures are conducted 40–50 times in a year
using Broviac/Hickman catheters that are placed in the subclavian vein. In the Ultrasound
and Biopsy Laboratory at the clinic mentioned above, approximately 200–250 examinations
have been conducted since 2005 to assess the central venous access. Implantation of
a catheter considerably increases the comfort of patients who require a long-term venous
access. Nevertheless, it is an invasive procedure, burdened with a risk of numerous, early
and late complications. The late complications are associated with implanted catheters and
include catheter-related thrombosis. The aim of this paper was to present three patients
of the Clinic of Pediatrics, Hematology and Oncology at the Academic Clinical Centre of
the Medical University in Gdańsk, in whom thrombotic complications occurred as a result
of long-term central venous catheters. The paper also discusses the possibilities of using
sonography in the assessment of such complications. In the presented patients, it was possible
to determine the size and localization of a thrombus which enabled effective treatment
in two cases. The pathomechanism of catheter-related thrombosis was explained and the
risk factors of such complications were discussed. The attention was paid to the necessity
of conducting ultrasound examinations in pediatric patients with inserted catheters as soon
as the fi rst symptoms of thrombosis appear. Based on own observations and despite the lack
of validation of ultrasound imaging in the assessment of central catheters, we believe that
this method is highly promising and can be recommended for the assessment of thrombotic
complications in pediatric patients with central venous catheters.Centralny dostęp dożylny polega na wprowadzeniu cewnika naczyniowego (kateteru) do
żyły głównej i zlokalizowaniu jego końcówki blisko ujścia do prawego przedsionka serca.
U pacjentów Kliniki Pediatrii, Hematologii i Onkologii Uniwersyteckiego Centrum Klinicznego
Gdańskiego Uniwersytetu Medycznego w ciągu roku przeprowadza się 40–50 zabiegów
implantacji cewników centralnych typu Broviac/Hickman przez żyłę podobojczykową.
W Pracowni Diagnostyki Ultrasonografi cznej i Biopsyjnej przy wyżej wymienionej Klinice
od 2005 roku wykonuje się rocznie około 200–250 badań, do których wskazaniem jest
ocena centralnego dostępu dożylnego. Zastosowanie kateteru znacznie poprawia komfort
pacjentów wymagających długotrwałego dostępu dożylnego. Jest jednak procedurą
inwazyjną, obarczoną ryzykiem wystąpienia szeregu powikłań, zarówno wczesnych, jak
i późnych. Powikłania późne dotyczą cewników już implantowanych i obejmują między
innymi zakrzepicę odcewnikową. Celem niniejszej pracy jest prezentacja trzech pacjentów
z Kliniki Pediatrii, Hematologii i Onkologii Uniwersyteckiego Centrum Klinicznego Gdańskiego
Uniwersytetu Medycznego, u których doszło do powikłań zakrzepowych w przebiegu
długotrwale utrzymywanego cewnika centralnego. Przedstawiono możliwości wykorzystania
ultrasonografi i w ocenie powstałych zaburzeń. W prezentowanych przypadkach
możliwe było określenie wielkości i lokalizacji skrzepliny, co pozwoliło na podjęcie skutecznego
leczenia u dwojga pacjentów. Omówiono patomechanizm powstania zakrzepicy
odcewnikowej oraz czynniki zwiększonego ryzyka takich zaburzeń. Zwrócono uwagę na
konieczność wykonywania badań ultrasonografi cznych u dzieci z kateterem już przy pierwszych
objawach sugerujących powikłania zakrzepowe. Na podstawie obserwacji własnych,
pomimo braku walidacji dla diagnostyki ultrasonografi cznej w ocenie cewników centralnych,
uważamy, że jest to metoda bardzo obiecująca i godna polecenia w ocenie powikłań
zakrzepowych u dzieci z centralnym dostępem dożylnym

Potential role of PET-CT in chemotherapy efficacy assessment and recurrence diagnosis in a patient with a Wilms’ tumour
BACKGROUND: Wilms’ tumour is the most frequent renal
malignancy in children. There is no worldwide consensus regarding
treatment and PET-CT role in this neoplasm. The aim
of this report is to demonstrate the potential role of PET-CT in
chemotherapy efficacy assessment and recurrence diagnosis.
CASE description: a 7-year-old boy was diagnosed with blastemic
type Wilms’ tumour and underwent neoadjuvant chemotherapy,
nephrectomy, and adjuvant chemotherapy in compliance
with SIOP protocol. Three years later the patient underwent
surgical resection of the metastasis and chemoradiotherapy.
Nine months later tomography and PET-CT were performed
(during the third month of the treatment due to a second recurrence).
The results were equivocal but within four months the
boy underwent surgical resection of a third recurrence. Fourteen
months later a second PET-CT revealed an active disease with extensive involvement of the left lung and pleura. The patient
was referred to oral palliative chemotherapy.
DISCUSSION: Equivocal PET-CT results during chemotherapy
should be interpreted with caution. The first, during third line
chemotherapy, was equivocal; however, an early massive recurrence
three months later indicates that treatment was ineffective.
The second PET-CT examination fourteen months later, as the
only modality, depicted massive progression of the disease. This
suggests the value of this examination in recurrence diagnosis.
CONCLUSIONS: PET-CT seems to be valuable technique in
recurrence detection in patients with histologically unfavourable
tumours. Equivocal results of PET-CT should raise suspicion of
recurrence even in recently treated patients. The timing of CIM
and PET-CT should be considered individually — no universal
and reliable schedule exists.

Spinal pain syndrome and headache as initial manifestation of spinal cord tumour – a report of two cases
Two cases of intraspinal thoracolumbar tumours with meningeal involvement in boys aged 10.5 and 12 years are presented. Medical history of both patients comprised headaches and back pains lasting for some weeks and months before admission. Because of the significant extension of the neoplastic process, one patient was disqualified from tumour resection and was administered with neoadjuvant chemotherapy. The second patient received partial tumour resection and supplementary intense chemotherapy. Both children died of disease progression after 2 and 15 months after first symptoms occurrence, respectively. Autopsy revealed malignant gliomas with extensive meningeal involvement as well as neoplasm dissemination within central nervous system via cerebro-spinal fluid in both cases. Vertebral pain is a rare neurological symptom in children, requiring particular attention and thorough radiological examination. It needs to be stressed that headaches may be also the first symptom of the tumour located within vertebral column.Przedstawiamy dwa przypadki guzów śródrdzeniowych odcinka piersiowo-lędźwiowego z towarzyszącym zajęciem opon mózgowo-rdzeniowych u chłopców w wieku 10,5 i 12 lat. Klinicznie choroba zamanifestowała się bólami głowy i kręgosłupa w okresie kilku tygodni i kilku miesięcy przed hospitalizacją. U jednego dziecka wykonanie zabiegu operacyjnego było niemożliwe ze względu na rozległość procesu nowotworowego, poddano je chemioterapii. U drugiego chłopca przeprowadzono częściową resekcję guza i zastosowano intensywną chemioterapię. Obydwaj chłopcy zmarli w wyniku progresji choroby, odpowiednio - w 2 i 15 miesięcy od wystąpienia pierwszych objawów neurologicznych. Badanie sekcyjne ujawniło w obydwu przypadkach utkanie glejaków złośliwych z rozległymi naciekami opony miękkiej oraz rozsiew nowotworu drogą płynu mózgowo-rdzeniowego w obrębie układu nerwowego. Zespół bólowy kręgosłupa jest rzadkim zespołem neurologicznym w wieku dziecięcym, wymagającym wnikliwej diagnostyki, ze szczególnym wskazaniem do wykonania badań obrazowych. Pierwszym objawem procesu rozrostowego w kanale kręgowym mogą być również bóle głowy
