The influence of chemotherapy on bone marrow in patients with chronic lymphocytic leukaemia
In chronic lymphocytic leukaemia (CLL) bone marrow trephine biopsy (BMT) is not
required for diagnosis but can have a significant prognostic value and can be used
for the detection of the minimal residual disease (MRD) and for assessment of the
effectiveness of the treatment applied. The aim of the study was to evaluate the
morphological changes in bone marrow after treatment with purine nucleoside
analogues cladribine and fludarabine. Bone marrow trephine biopsy was taken
routinely from 15 patients with CLL. Bone marrow trephine biopsy was performed
on every patient before as well as after chemotherapy. The number of cell elements
of the marrow (the degree of atrophy), the patterns of bone marrow infiltration,
the presence of reticulin and collagen fibres and the disturbances in bone
marrow stroma were assessed. The infiltration of bone marrow by neoplastic cells
was observed in all the patients before administration of chemotherapy. The infiltration
was followed by an increase in the number of reticulin fibres. After the
treatment a regression of the reticulin fibres was observed with the lessening of
the infiltration. After the treatment the levels of marrow infiltrate were decreased.

Assessment of expression of selected Bcl-2 family proteins in lymphoid infiltration in patients with B-cell chronic lymphocytic leukaemia treated with nucleoside analogues.
B-cell chronic lymphocytic leukaemia (B-CLL) is characterized by clonal growth and accumulation of mature lymphoid cells due to disturbance in genetically regulated form of cell death called apoptosis. The intrinsic mechanism of apoptosis is controlled by Bcl-2 family proteins. Purine nucleoside analogues induce the apoptosis in cells in a state of quiescence. The aim of the study was to assess expression of selected Bcl-2 family proteins in neoplastic infiltration in bone marrow in patients with B-CLL treated with nucleoside analogues. The study comprised examination of bone marrow obtained routinely by trephine biopsy from 18 patients with B-CLL diagnosed before administration of purine nucleoside analogues treatment and after its completion. Expression of Bcl-2, Bcl-x and Bax proteins was examined. Lymphoid cells in bone marrow were present in all patients before administration of treatment. After treatment in two patients bone marrow was infiltrated in diffuse pattern, whereas other patients presented nodular pattern of infiltration. Układ antykoagulacyjny białka C w ostrych białaczkach
Acute leukaemias development is connected with subclinical activation of coagulation. Haemostatic disturbances – as a result of endothelium damage and/or impaired synthetic function of the liver – not always manifest as an overt haemorrhage or thrombotic disease. Compensatory mechanisms competence, in so far as it affect protein C anticoagulant system, assures haemostatic balance between prothrombotic and prohaemorrhage factors.Investigation was performed in 20 newly diagnosed patients with acute leukaemias. At the moment of diagnosis, plasma concentrations decrease of anticoagulation proteins, synthesised by the liver was observed. There was also noted an increase of sTM and significant decrease of sEPCR concentrations – markers of endothelial damage.Rozwojowi ostrych białaczek już w momencie diagnozy towarzyszy subkliniczna aktywacja krzepnięcia. Zaburzenia hemostazy, wynikające m.in. z uszkodzenia śródbłonka i osłabionej funkcji syntetycznej wątroby, nie zawsze manifestują się jawną skazą krwotoczną lub chorobą zakrzepową. Wydolność mechanizmów kompensacyjnych na poziomie układu antykoagulacyjnego białka C – zapewnia równowagę hemostatyczną między czynnikami prozakrzepowymi i prokrwotocznymi. Badaniami objęto 20 chorych z nowo rozpoznanymi ostrymi białaczkami. Colorectal cancers in chronic lymphocytic leukemia patients
Przewlekła białaczka limfocytowa (CLL) jest najczęstszym rodzajem białaczki w krajach zachodnich.
U chorych na CLL obserwuje się zwiększone ryzyko wystąpienia innych nowotworów.
W ich rozwoju istotną rolę odgrywają zaburzenia układu odpornościowego, uwarunkowania
genetyczne oraz wcześniejsze leczenie cytostatyczne. W niniejszej pracy przedstawiono
przypadki dwóch chorych z CLL, u których stwierdzono raka gruczołowego dolnego odcinka
przewodu pokarmowego.

western countries. Performed population study showed increased risk of second malignancy in
this group of patients. The role of immunodeficiency, genetic predisposition and received anti-
-cancer treatment in the development of second neoplasms may be important. This paper
presents two cases of CLL patients with coexisting colorectal adenocarcinoma.

Effectiveness and safety of ixazomib–lenalidomide–dexamethasone in high-cytogenetic-risk relapsed/refractory multiple myeloma — results of the Polish Myeloma Group observational study
The survival of patients with multiple myeloma (MM) has significantly improved in recent years due to the introduction of new drugs such as proteasome inhibitors (PI) or immunomodulatory drugs (ImiDs). However, MM is still an incurable condition, with very variable clinical course.
The group of patients with especially poor prognosis are individuals with relapsed/refractory multiple myeloma (RRMM) with specific cytogenetic disorders — del(17p), t(4;14), t(14;16). Among the therapies that are currently in use the ixazomib–lenalidomid–dexamethasone (IRd) is considered as a candidate to improve outcome.
In this study, we analyzed the cases of patients diagnosed with high-risk molecular RRMM, who have been treated with the IRd chemotherapy regimen. An aggressive case report with no known cytogenetic data was also added. The data was collected from four centers in Poland as part of the Polish Myeloma Group observational study.
The results suggest high efficacy and good safety profile of IRd therapy in patients with RRMM and unfavorable cytogenetics
Treatment of patients with Philadelphia positive acute lymphoblastic leukemia
Translokacja (9;22)(q34:q11.2), zwana chromosomem Filadelfia (Ph), jest najczęstszym zaburzeniem
cytogenetycznym stwierdzanym u dorosłych chorych na ostrą białaczkę limfoblastyczną (ALL). Przed wprowadzeniem inhibitorów kinazy tyrozynowej (TKI) podtyp ALL Ph+
był obciążony szczególnie złym rokowaniem. Wprowadzenie imatynibu (IM), stosowanego
w skojarzeniu z chemioterapią indukującą i konsolidującą, pozwoliło na zwiększenie odsetka
całkowitych remisji do ponad 90% i zwiększenie szansy na allogeniczne przeszczepienie krwiotwórczych komórek macierzystych. Pierwsze doniesienia dotyczące wyników odległych wskazują na poprawę prawdopodobieństwa przeżycia, które w perspektywie 5-letniej wynosi obecnie
około 50%. Konieczne jest przeprowadzenie dalszych badań klinicznych zmierzających do
ustalenia optymalnego sposobu stosowania IM oraz określenia ewentualnej roli TKI II generacji
w leczeniu I linii.

cytogenetic aberration among adults with acute lymphoblastic leukemia (ALL). Before the era
of tyrosine kinase inhibitors (TKI), ALL Ph+ was associated with particularly poor prognosis.
Introduction of imatinib (IM) in combination with induction-consolidation chemotherapy
increased complete remission rate to over 90% and allowed application of allogeneic hematopoietic
stem cell transplantation to greater proportion of patients. First reports on long term
results indicate that 5-year probability of the overall survival is now approximately 50%.
Further improvement requires prospective clinical trials aimed to optimize the protocols of IM
administration and evaluate potential role of 2nd generation TKI in the up-front therapy of
adults with ALL Ph+.

Prophylaxis and treatment of the central nervous system involvement in lymphoid malignancies
W przebiegu nowotworów układu chłonnego może dojść do pierwotnego lub wtórnego zajęcia
ośrodkowego układu nerwowego (OUN) z obecnością komórek chłoniaka w płynie mózgowo-rdzeniowym i/lub ze zmianami o charakterze oponowym, miąższowym lub mieszanym. Ryzyko
takiego umiejscowienia choroby jest duże, zwłaszcza w przebiegu ostrej białaczki limfoblastycznej
(ALL)/chłoniaka limfoblastycznego (LBL), chłoniaka Burkitta (BL), u niektórych
chorych na chłoniaka rozlanego z dużych komórek B (DLBCL) oraz w przebiegu chłoniaków
nieklasyfikowalnych o cechach pośrednich między BL i DLBCL. Rozpoznanie zajęcia OUN
implikuje konieczność intensywnej terapii obejmującej chemioterapię dokanałową, stosowanie
dużych dawek cytostatyków przekraczających barierę krew–mózg, a w niektórych przypadkach
- radioterapię. U chorych, u których doszło do nawrotu w obrębie OUN, rokowanie jest złe.
Z tego względu u osób obciążonych wysokim ryzykiem niezbędna jest odpowiednia profilaktyka. W niniejszej pracy przedstawiono zalecenia sformułowane przez Polską Grupę ds. Leczenia
Białaczek u Dorosłych (PALG) oraz Polską Grupę Badawczą Chłoniaków (PLRG).

nervous system (CNS). The risk is particularly high in course acute lymphoblastic leukemia
(ALL)/lymphoblastic lymphoma (LBL), Burkitt lymphoma (BL), selected patients with diffuse
large B cell lymphoma (DLBCL) and unclassifiable large B cell lymphoma with features
between DLBCL and BL. The diagnosis of CNS involvement implicates the need for intensive
treatment including intrathecal chemotherapy as well as intravenous infusion of high doses of
cytostatics crossing the blood-brain barrier. As the prognosis of patients with CNS relapse is
poor, appropriate prophylaxis is required. In this paper we present recommendations of the
Polish Adult Leukemia Group (PALG) and the Polish Lymphoma Research Group (PLRG).

Ponatynib w leczeniu przewlekłej białaczki szpikowej z obecnością mutacji T315I
Przewlekła białaczka szpikowa (PBSz), zaliczana do grupy nowotworów mieloproliferacyjnych, jest chorobą nowotworową, u podłoża której leży charakterystyczna translokacja t(9;22)(q34;q11) i związane z tym powstanie skróconego chromosomu 22. pary, określanego jako chromosom Philadelphia (Ph). Poczynione w ostatnich dwóch dekadach postępy terapeutyczne oraz wprowadzenie do terapii nowych inhibitorów kinazy tyrozynowej (IKT) znacząco poprawiły rokowanie chorych. W niniejszej pracy przedstawiono przypadek pacjenta z rozpoznaną PBSz z obecnością mutacji punktowej T315I, leczonej ponatynibe
