    Estrategias de comercialización y percepción de marca de una empresa de conservas de pescado, Trujillo 2021

    El siguiente estudio se planteó como principal objetivo establecer la relación entre las estrategias de comercialización y la percepción de marca de una empresa de conservas de pescado en la provincia de Trujillo en el año 2021, por lo que se ejecutó una investigación de tipo aplicada, con enfoque cuantitativo, de nivel correlacional y con diseño no experimental – transversal. En ese sentido, se tomó en cuenta como población a las personas que consumen conservas de pescado en la ciudad de Trujillo, que por ser una población indeterminada se consideró a una muestra aleatoria simple de 385 clientes consumidores de conservas. Como resultados se obtuvo que el nivel de gestión de las estrategias de comercialización es alto de acuerdo al 44,4 %; mientras que el 52,2 % de ellos considera que el grado de percepción de marca es medio. Concluyendo que, sí existe una relación significativa entre las estrategias de comercialización y la percepción de marca de una empresa de conservas de pescado en la provincia de Trujillo en el año 2021, con valores de significancia de 0,000 y grado alto de 0,780, por lo que, al mejorar las estrategias de comercialización en la empresa, aumentará también la percepción de marca

    Environmental sustainability of alternative marine propulsion technologies powered by hydrogen - a life cycle assessment approach

    Shipping is a very important source of pollution worldwide. In recent years, numerous actions and measures have been developed trying to reduce the levels of greenhouse gases (GHG) from the marine exhaust emissions in the fight against climate change, boosting the Sustainable Development Goal 13. Following this target, the action of hydrogen as energy vector makes it a suitable alternative to be used as fuel, constituting a very promising energy carrier for energy transition and decarbonization in maritime transport. The objective of this study is to develop an ex-ante environmental evaluation of two promising technologies for vessels propulsion, a H2 Polymeric Electrolytic Membrane Fuel Cell (PEMFC), and a H2 Internal Combustion Engine (ICE), in order to determine their viability and eligibility compared to the traditional one, a diesel ICE. The applied methodology follows the Life Cycle Assessment (LCA) guidelines, considering a functional unit of 1 kWh of energy produced. LCA results reveal that both alternatives have great potential to promote the energy transition, particularly the H2 ICE. However, as technologies readiness level is quite low, it was concluded that the assessment has been conducted at a very early stage, so their sustainability and environmental performance may change as they become more widely developed and deployed, which can be only achieved with political and stakeholder's involvement and collaboration.This work was supported by the European Union through the Project HYLANTIC EAPA_204/201; “The Atlantic Network for Renewable Generation and Supply of Hydrogen to promote High Energy Efficiency”

    Hospital emergency room diagnosis of acute appendicitis in patients aged 2 to 20 years: the INFURG-SEMES score from the emergency infections study of the Spanish Society of Emergency Medicine

    Objetivo. Derivar una escala clínico-analítica diagnóstica de apendicitis aguda (AA) en pacientes entre 2 y 20 años atendidos por dolor abdominal con sospecha de AA en servicios de urgencias hospitalarios (SUH), y comparar su capacidad diagnóstica con la Escala de Alvarado (EA). Métodos. Estudio observacional de cohorte prospectivo que incluyó de forma consecutiva pacientes entre 2 y 20 años con dolor abdominal sospechoso de AA de menos de 72 horas de evolución atendidos en 4 SUH españoles entre junio y diciembre de 2014. Se recogieron datos demográficos, clínicos, analíticos (recuento leucocitario, fórmula y proteína C reactiva) y radiológicos (ecografía y/o TC) y, si procedía, quirúrgicos. La variable resultado principal fue el diagnóstico final de AA en los 14 días desde la visita índice. Resultados. Se incluyeron 331 pacientes con edad media de 11,8 (DE 3,8) años, siendo 175 (52,9%) hombres. Ciento dieciséis (35,0%) tuvieron diagnóstico final de AA. La escala INFURG-SEMES incluye sexo masculino, dolor en fosa ilíaca derecha a la exploración, dolor a la percusión, dolor al caminar, presencia de neutrofilia y proteína C reactiva elevada. El área bajo la curva (ABC) de la característica operativa del receptor (COR) de dicha escala fue 0,84 (IC 95% 0,79-0,88) y para la EA 0,77 (IC95% 0,72-0,82) siendo la diferencia estadísticamente significativa (p = 0,002). Conclusiones. La escala INFURG-SEMES podría ser una herramienta de ayuda para el diagnóstico de AA en los pacientes entre 2 y 20 años atendidos con dolor abdominal sospechoso de apendicitis en los SUH, y ha mostrado una mayor capacidad discriminativa que la EA

    IL-6 serum levels predict severity and response to tocilizumab in COVID-19: An observational study

    Background: Patients with coronavirus disaese 2019 (COVID-19) can develop a cytokine release syndrome that eventually leads to acute respiratory distress syndrome requiring invasive mechanical ventilation (IMV). Because IL-6 is a relevant cytokine in acute respiratory distress syndrome, the blockade of its receptor with tocilizumab (TCZ) could reduce mortality and/or morbidity in severe COVID-19. Objective: We sought to determine whether baseline IL-6 serum levels can predict the need for IMV and the response to TCZ. Methods: A retrospective observational study was performed in hospitalized patients diagnosed with COVID-19. Clinical information and laboratory findings, including IL-6 levels, were collected approximately 3 and 9 days after admission to be matched with preadministration and postadministration of TCZ. Multivariable logistic and linear regressions and survival analysis were performed depending on outcomes: need for IMV, evolution of arterial oxygen tension/fraction of inspired oxygen ratio, or mortality. Results: One hundred forty-six patients were studied, predominantly males (66%); median age was 63 years. Forty-four patients (30%) required IMV, and 58 patients (40%) received treatment with TCZ. IL-6 levels greater than 30 pg/mL was the best predictor for IMV (odds ratio, 7.1; P < .001). Early administration of TCZ was associated with improvement in oxygenation (arterial oxygen tension/fraction of inspired oxygen ratio) in patients with high IL-6 (P = .048). Patients with high IL-6 not treated with TCZ showed high mortality (hazard ratio, 4.6; P = .003), as well as those with low IL-6 treated with TCZ (hazard ratio, 3.6; P = .016). No relevant serious adverse events were observed in TCZ-treated patients.     Spread of a SARS-CoV-2 variant through Europe in the summer of 2020

    [EN] Following its emergence in late 2019, the spread of SARS-CoV-21,2 has been tracked by phylogenetic analysis of viral genome sequences in unprecedented detail3,4,5. Although the virus spread globally in early 2020 before borders closed, intercontinental travel has since been greatly reduced. However, travel within Europe resumed in the summer of 2020. Here we report on a SARS-CoV-2 variant, 20E (EU1), that was identified in Spain in early summer 2020 and subsequently spread across Europe. We find no evidence that this variant has increased transmissibility, but instead demonstrate how rising incidence in Spain, resumption of travel, and lack of effective screening and containment may explain the variant’s success. Despite travel restrictions, we estimate that 20E (EU1) was introduced hundreds of times to European countries by summertime travellers, which is likely to have undermined local efforts to minimize infection with SARS-CoV-2. Our results illustrate how a variant can rapidly become dominant even in the absence of a substantial transmission advantage in favourable epidemiological settings. Genomic surveillance is critical for understanding how travel can affect transmission of SARS-CoV-2, and thus for informing future containment strategies as travel resumes.S

    Mortality and pulmonary complications in patients undergoing surgery with perioperative SARS-CoV-2 infection: an international cohort study

    Background: The impact of severe acute respiratory syndrome coronavirus 2 (SARS-CoV-2) on postoperative recovery needs to be understood to inform clinical decision making during and after the COVID-19 pandemic. This study reports 30-day mortality and pulmonary complication rates in patients with perioperative SARS-CoV-2 infection. Methods: This international, multicentre, cohort study at 235 hospitals in 24 countries included all patients undergoing surgery who had SARS-CoV-2 infection confirmed within 7 days before or 30 days after surgery. The primary outcome measure was 30-day postoperative mortality and was assessed in all enrolled patients. The main secondary outcome measure was pulmonary complications, defined as pneumonia, acute respiratory distress syndrome, or unexpected postoperative ventilation. Findings: This analysis includes 1128 patients who had surgery between Jan 1 and March 31, 2020, of whom 835 (74·0%) had emergency surgery and 280 (24·8%) had elective surgery. SARS-CoV-2 infection was confirmed preoperatively in 294 (26·1%) patients. 30-day mortality was 23·8% (268 of 1128). Pulmonary complications occurred in 577 (51·2%) of 1128 patients; 30-day mortality in these patients was 38·0% (219 of 577), accounting for 81·7% (219 of 268) of all deaths. In adjusted analyses, 30-day mortality was associated with male sex (odds ratio 1·75 [95% CI 1·28–2·40], p\textless0·0001), age 70 years or older versus younger than 70 years (2·30 [1·65–3·22], p\textless0·0001), American Society of Anesthesiologists grades 3–5 versus grades 1–2 (2·35 [1·57–3·53], p\textless0·0001), malignant versus benign or obstetric diagnosis (1·55 [1·01–2·39], p=0·046), emergency versus elective surgery (1·67 [1·06–2·63], p=0·026), and major versus minor surgery (1·52 [1·01–2·31], p=0·047). Interpretation: Postoperative pulmonary complications occur in half of patients with perioperative SARS-CoV-2 infection and are associated with high mortality. Thresholds for surgery during the COVID-19 pandemic should be higher than during normal practice, particularly in men aged 70 years and older. Consideration should be given for postponing non-urgent procedures and promoting non-operative treatment to delay or avoid the need for surgery.     Assessment design decisions in practice: Profile identification in approaches to assessment design

    This study aims to identify different profiles of higher education teachers based on the way they design their assessment methods. It also explores differential characteristics in each profile and differences in the assessment methods preferred by the teachers and those implemented in their subjects. Seventeen teachers from four universities participated carrying out a think-aloud simulation task: designing the assessment methods for a set of learning outcomes. Teachers’ testimony during the task was transcribed and categorized using content analysis and an open-coding procedure. Three different patterns were identified: (a) focus on the feasibility of the assessment tasks, (b) on the alignment with the learning outcomes, or (c) alignment with teaching methods. Most of the participants focused only in one of the three elements. Teachers also designed different assessment methods in the simulation task in comparison with the ones they use in their subjects, despite the guidelines of assessing the same learning outcomes. A lack of resources is claimed as the reason of these differences.
