    Clinical Manifestations Vary with Different Age Spectrums in Infants with Kawasaki Disease

    Background. Kawasaki disease (KD) is an acute systemic vasculitis with unknown etiology. The diagnosis of KD depends on clinical manifestations. The prevalence of coronary artery abnormality (CAA) is 11.0% and results in cardiac sequelae, such as myocardial infarction or coronary aneurysm, which are the most serious complications in KD. Methods. We divided KD's children into different age groups: ≤6 months old, 7 months to 1 year old, and >1 year old, respectively. Different parameters were compared in each group. Results. Infants ≤6 months old are less likely to fulfill KD's major diagnostic criteria within 10 days, are prone to develop incomplete KD with the lowest cholesterol level, and have the greatest chance to have CAA and the laboratory features associated with CAA, such as the longest time needed to confirm CA diagnosis, lower hemoglobin level, lower albumin level, and higher platelet count. Infants <1 year old develop higher percentage of leukocytosis and sterile pyuria. But this group has fewer patients with neck lymphadenopathy

    Establishment, Regeneration, and Succession of Korean Red Pine (Pinus densiflora S. et Z.) Forest in Korea

    Seed production of Korean red pine (Pinus densiflora Siebold & Zucc.) was ranging from 25 to 27 seeds/m2 with a viability averaging between 42 and 44%. Seed dispersal reaches about 80 m. Germination rate of seed varied from 19 to 90%, and survival rate of seedling varied from 0 to 30% depending on moisture condition in field experiment. Survivorship curve of the pine population showed type III. Species composition of the pine forest was characterized by possessing plants with resistant capacity to water deficit such as Rhododendron micranthum, Vaccinium hirtum var. koreanum, Spodiopogon sibiricus, and Lespedeza cyrtobotrya. Ecological longevity of the pine was about 140 years based on mean age of gap makers. Natural maintenance of the pine forest depended on disturbance regime, which is dominated by endogenous factor. Natural regeneration of the pine forest is possible only in a very restricted site such as ridgetop with thin and infertile soil condition. Therefore, active and systematic management is required for artificial regeneration of the forest as is known in silivicultural method.     The Specific Characteristics in Children with Obstructive Sleep Apnea and Cor Pulmonale

    Background. The prevalence of obstructive sleep apnea (OSA) in the pediatric population is currently estimated at 1-2% of all children. The purpose of this study was to investigate the clinical and hemodynamic characteristics in pediatric patients with cor pulmonale and OSA. Methods. Thirty children with the diagnosis of OSA were included. These patients consisted of 26 male and 4 female children with a mean age of 7 ± 4 years old. Five of those children were found to be associated with cor pulmonale, and 25 had OSA but without cor pulmonale. Results. The arousal index was much higher in children with OSA and cor pulmonale. The children with OSA and cor pulmonale had much lower mean and minimal oxygen saturation and a higher incidence of bradycardia events. All 5 patients with OSA and cor pulmonale underwent an adenotonsillectomy, and the pulmonary arterial pressure dropped significantly after the surgery. Conclusion. This study demonstrated that the OSA pediatric patients with cor pulmonale had the different clinical manifestations and hemodynamic characteristics from those without cor pulmonale. The adenotonsillectomy had excellent results in both the OSA pediatric patients with and without cor pulmonale

    Peritoneal Dialysis in Infants and Children After Open Heart Surgery

    BackgroundInfants and children who undergo surgical repair of complex congenital heart diseases are prone to developing renal dysfunction. The purpose of this study was to investigate the risk factors associated with prolonged peritoneal dialysis (PD) and the mortality of pediatric patients with acute renal failure (ARF) after open heart surgery.MethodsFrom June 1999 to May 2007, a total of 542 children underwent open heart surgery for congenital heart disease. Fifteen (2.8%) experienced ARF and seven (1.3%) required PD. The clinical and laboratory variables were compared between the survivor and non-survivor groups of ARF patients that needed PD.ResultsThe non-survivors (n = 3, 43%) had a longer cardiopulmonary bypass time (154 ± 21 vs. 111 ± 8 minutes, p = 0.012) and longer aorta clamping time (92 ± 40 vs. 66 ± 15 minutes, p = 0.010) than the survivors (n = 4, 57%). Before the PD, the pH and base excess of the arterial blood gas analysis in the survivors was much higher than that non-survivors (7.30 ± 0.04 vs. 7.16 ± 0.10, p = 0.039; −5.15 ± 3.13 vs. −12.07 ± 2.9 mmol/L, p = 0.031). Furthermore, the survivors had a shorter interval between the onset of ARF and the day the PD was begun (1.2 ± 0.4 vs. 4.3 ± 1.2 days, p = 0.001), and shorter duration of PD (6.6 ± 2.7 vs. 13.0 ± 3.5 days, p= 0.036) than non-survivors.ConclusionEarly intervention with PD is a safe and effective method for managing patients with ARF after open heart surgery. The cardiopulmonary bypass and aortic clamping duration, time of initiating PD, duration of the PD, sepsis, and relative complications may predict the prognosis of these patients
