Oral health and systemic inflammatory, cardiac and nitroxid biomarkers in hemodialysis patients
Periodontal diseases have systemic inflammatory effects and have been adversely associated with cardiovascular diseases, which are also the most frequent cause of death in the end-stage renal disease. The aim of this cross-sectional study was to investigate the oral health and serum biomarkers among the hemodialysis (HD) patients in Slovenia. 111 HD patients were periodontally examined and their sera were assayed for C reactive protein (CRP), cardiac troponin T (TnT), nitrite/nitrate (NOx) and antibody levels to A. actinomycetemcomitans and P. gingivalis. The association of oral health with systemic response was analyzed with Kruskal-Wallis test, Fisher?s exact test and multivariate linear regression. Bleeding on probing without periodontal pockets was present in 5.2%, calculus without periodontal pockets in 42.1%, shallow periodontal pockets in 39.5% and deep periodontal pockets in 13.2% of dentate patients. There were 28.8% edentulous participants. 63.1% of the patients had CRP levels higher than 3 mg/L and 34.2% higher than 10 mg/L. TnT was detectable in all participants, with 25.2% exhibiting levels higher than 100 ng/L. The median level of NOx was 43.1 Āµmol/L. Participants with higher CRP were more likely to be edentulous and have higher TnT levels. A direct association of oral health with TnT or NOx was not detected. HD patients in Slovenia have compromised oral health and increased serum inflammatory and cardiac biomarkers. Edentulousness was an independent predictor for the increased CRP, indicating a need for improved dental care to retain the teeth as long as possible
Smjernice za farmakoloÅ”ko lijeÄenje epilepsije
SAŽETAK
MeÄunarodne smjernice za farmakoloÅ”ko lijeÄenje epilepsija opÄenite su, sveobuhvatne i ne prepoznaju lokalne specifiÄnosti poput ekonomskih i tehniÄkih moguÄnosti u pojedinim državama, dostupnosti pojedinih antiepileptika ili drugih metoda lijeÄenja i sliÄno. Stoga se nameÄe potreba izrade nacionalnih smjernica, Äiji su zapravo temelj meÄunarodne smjernice Internacionalne lige protiv epilepsije. Hrvatske smjernice za farmakoloÅ”ko lijeÄenje epilepsija plod su suradnje svih relevantnih struÄnih druÅ”tava i referentnih centara u RH, na Äelu s Hrvatskom ligom protiv epilepsije te Hrvatskim neuroloÅ”kim druÅ”tvom i Hrvatskim druÅ”tvom za djeÄju neurologiju Hrvatskoga lijeÄniÄkog zbora, a odražavaju aktualne socioekonomske i regulatorne specifiÄnosti u naÅ”oj zemlji, najnovije spoznaje farmakoloÅ”kih profila i uÄinkovitosti pojedinih antiepileptika kao i ekspertna miÅ”ljenja. AntiepileptiÄka terapija se uvodi nakon postavljanja dijagnoze epilepsije, stoga profilaktiÄka primjena nije opravdana. Nakon postavljanja dijagnoze potrebno je bolesnika informirati o prognozi bolesti, moguÄnostima lijeÄenja i samopomoÄi, životnim ograniÄenjima te moguÄim neželjenim dogaÄajima. Ciljevi farmakoterapije epilepsija su potpuna kontrola napada uz izbjegavanje nuspojava te održavanje ili poboljÅ”anje kvalitete života. Zlatni standard lijeÄenja je monoterapija odnosno primjena adekvatnog antiepileptika u adekvatnoj dozi. Izbor i titracija lijeka su individualni, a temelje se na smjernicama za lijeÄenje pojedinih vrsta napada, karakteristikama bolesnika i regulatorno specifiÄnim Äimbenicima. Nakon neuspjeha inicijalne monoterapije, potrebna je reevalucija anamnestiÄkih i dijagnostiÄkih podataka te potom postupna i spora zamjena antiepileptika. Racionalna politerapija podrazumijeva kombinaciju dvaju antiepileptika razliÄitih mehanizama djelovanja, prvog ili eventualno drugog izbora za postavljenju dijagnozu, niskoga interakcijskog potencijala, razliÄitog profila nuspojava i sinergistiÄkog ili aditivnog djelovanja. Zamjena generiÄkih ili originalnog i generiÄkog oblika lijeka nije preporuÄljiva, a poglavito nakon postizanja remisije ili prilikom uzimanja visokih doza lijeka. Ukidanje antiepileptiÄke terapije treba biti postupno i sporo, u sluÄaju politerapije jedan po jedan lijek, a u donoÅ”enju odluke o ukidanju, kao i o uvoÄenju antiepileptika, mora biti ukljuÄen bolesnik i njegova obitelj

The composition and microstructure of historic tongues and shallots from reed pipes of various Baroque organs has been studied. They contain Cu-Zn solid solution (Ī±-brass with 23-29 wt. % Zn) and lead particles. Lead is mainly present as spherical bulk or lens-like grain boundary (GB) inclusions. However, in two samples Pb wets the brass GBs. In this case Pb forms the branched root-like structures. Grain size in brass scatters from 10 to 200 Ī¼m. Around 50% of all GBs in brass are Ī£=3 twin GBs. The high-indexed coincidence site lattice facets were observed in twin GBs. The increase of number of various facets roughly correlates with decreasing grain size. It may indicate the variation in annealing temperature used by organbuilders in Baroque Era. The annealing temperature has been estimated using the faceting phase diagram for twin GBs in Cu

Brain MRI post-processing with MAP07 in the preoperative evaluation of patients with pharmacoresistant epilepsy ā Croatian single centre experience
Objective: This study aimed to determine the role of brain MRI post-processing method MAP07 (Morphometric Analysis Program) in detecting epileptogenic brain lesions in patients with pharmacoresistant epilepsy (PE). MAP07 is a sophisticated diagnostic program that offers several morphometric maps and facilitates the detection and localization of hippocampal sclerosis (HS), focal cortical dysplasias (FCD), and other types of cortical malformations, which could be undetected by conventional visual MRI analysis (CVA). -----
Methods: 120 patients aged > 16 years with PE have been recruited. 3 T MRI was performed according to epilepsy imaging protocol followed by image postprocessing with a fully automated MATLAB script, MAP07, by applying SPM5 algorithms. Statistical analysis was performed in IBM SPSS Statistics, version 25.0.

Results: Analysis in our patients showed a high sensitivity of MAP07 with low specificity and with a high proportion of false-positive patients. After MRI analysis, out of 120 patients, 32 were found to have no structural abnormalities by conventional visual analysis in whom after MAP07 in 5 patients structural lesions were found (in one HS, in one FCD, in two perinatal vascular lesions, and in one hippocampal hyperintensity). There was a quite high overall coincidence of the findings of MAP07 and MRI for the detection of FCD, HS, perinatal ischemia/chronic vascular lesions, heterotopias, and polymicrogyria (kappa coefficient above 0.700).

Conclusions: MAP07 analysis is a useful, additional, and automated method that may guide re-evaluation of MRI by highlighting suspicious cortical regions, as a complementary method to CVA, by enhancing the visualization of cortical malformations and lesions
Single oral inoculation with Escherichia coli (ATCC 25922) stimulates generalised production of nitric oxide in mice
Nitric oxide (NO) production was investigated in the lungs, thoracic aorta, heart, liver, spleen, kidneys and brain of mice inoculated orally with
Escherichia coli
ATCC 25922. Detection of NO was performed by electron paramagnetic resonance (EPR) using diethyldithiocarbamate (DETC) spin trap. Nitric oxide synthase (NOS) inhibitors [nonselective: L-NAME and inducible NOS (iNOS) selective: 1400W] were used to determine the source of NO. Spin-trap only and untreated mice were included as controls. Within 2.5 hours (h) of a single oral inoculation with
E. coli
half of the animals had increased NO levels in all investigated organs. Thereafter the signals dropped before increasing again to reach maximal median values by 25 h in all organs of all inoculated mice. The most intense response occurred in livers, followed by aorta and lungs. Early (2.5 h) inhibition of the signal was achieved using both NOS inhibitors. L-NAME was also effective at 25 h, while 1400W-treated mice had increased NO levels beyond 7 h. The generalised increase in NO production in the short and longer term indicates a host response to
