Dynamic study of blood-brain barrier closure after its disruption using ultrasound: a quantitative analysis.
International audienceDelivery of therapeutic or diagnostic agents to the brain is majorly hindered by the blood-brain barrier (BBB). Recently, many studies have demonstrated local and transient disruption of the BBB using low power ultrasound sonication combined with intravascular microbubbles. However, BBB opening and closure mechanisms are poorly understood, especially the maximum gap that may be safely generated between endothelial cells and the duration of opening of the BBB. Here, we studied BBB opening and closure under magnetic resonance (MR) guidance in a rat model. First, MR contrast agents (CA) of different hydrodynamic diameters (1 to 65 nm) were employed to estimate the largest molecular size permissible across the cerebral tissues. Second, to estimate the duration of the BBB opening, the CA were injected at various times post-BBB disruption (12 minutes to 24 hours). A T(1) mapping strategy was developed to assess CA concentration at the ultrasound (US) focal point. Based on our experimental data and BBB closure modeling, a calibration curve was obtained to compute the half closure time as a function of CA hydrodynamic diameter. ACR Appropriateness Criteria® Locoregional therapy for resectable oropharyngeal squamous cell carcinomas
BackgroundThere are no level I studies to guide treatment for resectable oropharyngeal squamous cell carcinoma (SCC). Treatment toxicities influence management recommendations. Ongoing investigations are examining deintensified treatments for human papillomavirus (HPV)‐associated oropharyngeal SCC.MethodsThe Appropriateness Criteria panel, using modified Delphi methodology, produced a literature summary, an assessment of treatment recommendations, and cases to illustrate their use.ResultsA multidisciplinary team produces optimum results. Based on HPV status, smoking history, and staging, patients are divided into groups at low, intermediate, and high‐risk of death. In the future, treatment recommendations may be influenced by HPV status, which has changed the epidemiology of oropharyngeal SCC.ConclusionT1 to T2N0M0 resectable oropharyngeal SCC can be treated with surgery or radiation without chemotherapy. Patients with T1‐2N1‐2aM0 disease can receive radiation, chemoradiation, or transoral surgery with neck dissection and appropriate adjuvant therapy. Patients with T1‐2N2b‐3M0 disease should receive chemoradiation or transoral surgery with neck dissection and appropriate adjuvant therapy. Concurrent chemoradiation is preferred for T3 to T4 disease. © 2016 Wiley Periodicals, Inc. International audienceThis study looks at the conditions under which Rostand wrote and published the poems that were collected in Le Vol de la Marseillaise soon after his death. Starting with an analysis of the discursive situation that was furnished for him during the world war and based on readings of several plays, it shows how Rostand was able to appropriate war discourse and how he found the means to bring it before the public in such a way that he came to be seen as a real national poet.Cette étude s’intéresse aux conditions dans lesquelles Rostand a rédigé et publié les poèmes qui ont été réunis dans Le Vol de la Marseillaise au lendemain de sa disparition. Partant d’une analyse de la situation discursive qui lui a été faite durant le conflit mondial et s’appuyant sur la lecture de plusieurs pièces, l’on montre comment Rostand est parvenu à s’approprier un discours de guerre et s’est donné le moyen de le porter devant le public au point de faire figure de véritable poète national
