Temporal trends in primary care-recorded self-harm during and beyond the first year of the COVID-19 pandemic: Time series analysis of electronic healthcare records for 2.8 million patients in the Greater Manchester Care Record
Summary: Background: Surveillance of temporal trends in clinically treated self-harm is an important component of suicide prevention in the dynamic context of COVID-19. There is little evidence beyond the initial months following the onset of the pandemic, despite national and regional restrictions persisting to mid-2021. Methods: Descriptive time series analysis utilizing de-identified, primary care health records of 2.8 million patients from the Greater Manchester Care Record. Frequencies of self-harm episodes between 1st January 2019 and 31st May 2021 were examined, including stratification by sex, age group, ethnicity, and index of multiple deprivation quintile. Findings: There were 33,444 episodes of self-harm by 13,148 individuals recorded during the study period. Frequency ratios of incident and all episodes of self-harm were 0.59 (95% CI 0.51 to 0.69) and 0.69 (CI 0.63 to 0.75) respectively in April 2020 compared to February 2020. Between August 2020 and May 2021 frequency ratios were 0.92 (CI 0.88 to 0.96) for incident episodes and 0.86 (CI 0.84 to 0.88) for all episodes compared to the same months in 2019. Reductions were largest among men and people living in the most deprived neighbourhoods, while an increase in all-episode self-harm was observed for adolescents aged 10–17. Interpretation: Reductions in primary care-recorded self-harm persisted to May 2021, though they were less marked than in April 2020 during the first national lockdown. The observed reductions could represent longer term reluctance to seek help from health services. Our findings have implications for the ability for services to offer recommended care for patients who have harmed themselves
Exploring the Consistency, Quality and Challenges in Manual and Automated Coding of Free-text Diagnoses from Hospital Outpatient Letters
Coding of unstructured clinical free-text to produce interoperable structured
data is essential to improve direct care, support clinical communication and to
enable clinical research.However, manual clinical coding is difficult and time
consuming, which motivates the development and use of natural language
processing for automated coding. This work evaluates the quality and
consistency of both manual and automated clinical coding of diagnoses from
hospital outpatient letters. Using 100 randomly selected letters, two human
clinicians performed coding of diagnosis lists to SNOMED CT. Automated coding
was also performed using IMO's Concept Tagger. A gold standard was constructed
by a panel of clinicians from a subset of the annotated diagnoses. This was
used to evaluate the quality and consistency of both manual and automated
coding via (1) a distance-based metric, treating SNOMED CT as a graph, and (2)
a qualitative metric agreed upon by the panel of clinicians. Correlation
between the two metrics was also evaluated. Comparing human and
computer-generated codes to the gold standard, the results indicate that humans
slightly out-performed automated coding, while both performed notably better
when there was only a single diagnosis contained in the free-text description.
Automated coding was considered acceptable by the panel of clinicians in
approximately 90% of cases
Findability of UK health datasets available for research: a mixed methods study
Objective How health researchers find secondary data to analyse is unclear. We sought to describe the approaches that UK organisations take to help researchers find data and to assess the findability of health data that are available for research.Methods We surveyed established organisations about how they make data findable. We derived measures of findability based on the first element of the FAIR principles (Findable, Accessible, Interoperable, Reproducible). We applied these to 13 UK health datasets and measured their findability via two major internet search engines in 2018 and repeated in 2021.Results Among 12 survey respondents, 11 indicated that they made metadata publicly available. Respondents said internet presence was important for findability, but that this needed improvement. In 2018, 8 out of 13 datasets were listed in the top 100 search results of 10 searches repeated on both search engines, while the remaining 5 were found one click away from those search results. In 2021, this had reduced to seven datasets directly listed and one dataset one click away. In 2021, Google Dataset Search had become available, which listed 3 of the 13 datasets within the top 100 search results.Discussion Measuring findability via online search engines is one method for evaluating efforts to improve findability. Findability could perhaps be improved with catalogues that have greater inclusion of datasets, field-level metadata and persistent identifiers.Conclusion UK organisations recognised the importance of the internet for finding data for research. However, health datasets available for research were no more findable in 2021 than in 2018
The Absolute Magnitude of RRc Variables From Statistical Parallax
We present the first definitive measurement of the absolute magnitude of RR
Lyrae c-type variable stars (RRc) determined purely from statistical parallax.
We use a sample of 247 RRc selected from the All Sky Automated Survey (ASAS)
for which high-quality light curves, photometry and proper motions are
available. We obtain high-resolution echelle spectra for these objects to
determine radial velocities and abundances as part of the Carnegie RR Lyrae
Survey (CARRS). We find that M_(V,RRc) = 0.52 +/- 0.11 at a mean metallicity of
[Fe/H] = -1.59. This is to be compared with previous estimates for RRab stars
(M_(V,RRab) = 0.75 +/- 0.13 and the only direct measurement of an RRc absolute
magnitude (RZ Cephei, M_(V, RRc) = 0.27 +/- 0.17). We find the bulk velocity of
the halo to be (W_pi, W_theta, W_z) = (10.9,34.9,7.2) km/s in the radial,
rotational and vertical directions with dispersions (sigma_(W_pi),
sigma_(W_theta), sigma_(W_z)) = (154.7, 103.6, 93.8) km/s. For the disk, we
find (W_pi, W_theta, W_z) = (8.5, 213.2, -22.1) km/s with dispersions
(sigma_(W_pi), sigma_(W_theta), sigma_(W_z)) = (63.5, 49.6, 51.3) km/s.
Finally, we suggest that UCAC2 proper motion errors may be overestimated by

Full Circle or Spiralling Out of Control?: State Violence and the Control of Urbanisation in Papua New Guinea
There is an administrative reluctance to recognise the permanency of urban settlement in Papua New Guinea. This reluctance, evident since the 1960s, has been characteristic of both the colonial and post-colonial administrations. Opposition to some facets of urbanisation continues today, despite growing population and land pressures in most rural areas and real problems of landlessness emerging in particular rural areas. Colonial control of urban populations has been replicated in contemporary times, often in more draconian form. Eviction of urban settlers has been tied to issues of crime and urban respectability, and lingering perceptions that Melanesians should be rural residents. The growth of informal settlements and urbanisation are not seen as issues of urban planning, nor is the context of urban migration linked to socioeconomic inequality, hence other forms of urban policy are largely absent. Strengthening alliances between land-owners and the state (especially police and provincial administrations) have thus emphasised intraurban inequality and hampered national development
Does Foreign Ownership Contribute to Sounder Banks in Emerging Markets? The Latin American Experience
Foreign bank entrants into emerging markets are usually thought to improve the condition and performance of acquired institutions, and more generally to enhance local financial stability. We use bank-specific data for a range of Latin American countries since the mid-1990s to address elements of this claim. Across the seven largest countries, we find that the financial strength ratings of local banks acquired by foreign entities generally show a slight improvement relative to their domestic counterparts. Our more in-depth case studies of Chile, Colombia, and Argentina do not indicate striking differences in health between larger foreign and domestic retail-oriented banks (although state banks are noticeably weaker). However, foreign banks often have higher average loan growth, higher average provisioning expense, and greater loss-absorption capacity. These results suggest that foreign ownership may provide important positive influences on the stability and development of emerging market banking systems
