    Transient unexpected improvement of AV conduction: What is the mechanism?

    This ECG was recorded from a 93 year old patient with a previously documented third degree AV block and an underlying LBBB. The twelve lead ECG demonstrates sinus bradycardia at a rate of 52 beats per minute, a PR interval of 230 milliseconds, and a left bundle branch block (Figure 1). In figure 2, a rhythm strip of leads V1, II and V5 shows sinus rhythm with AV block. Note that only P waves in the T wave of the paced beat are conducted. The non-conducted P waves are followed by ventricular paced beats at an escape interval of 1200 milliseconds

    The past three decades have been witness to a nascent but compelling body of literature on lessons in leadership for business derived from biblical narratives. The aim of this paper is to advance that effort. Specifically, this study considers the leadership of Zerubbabel, the governor of Judah, who built the Second Temple on the ruins of the First. When he arrived in Judah from Babylonia, the walls of Jerusalem were breached and the entire country was filled with people hostile to constructing the Temple. One of the mysteries of the Bible is the disappearance of Zerubbabel from the biblical record. This paper discusses mistakes made by Zerubbabel as a leader, how Ezra and Nehemiah rectified these errors, and demonstrates what leaders of today can learn from the issues involved in the construction of the Second Temple

    Investigation of spiral blood flow in a model of arterial stenosis

    The spiral component of blood flow has both beneficial and detrimental effects in human circulatory system [Stonebridge PA, Brophy CM. Spiral laminar flow in arteries? Lancet 1991; 338: 1360–1]. We investigate the effects of the spiral blood flow in a model of three-dimensional arterial stenosis with a 75% cross-sectional area reduction at the centre by means of computational fluid dynamics (CFD) techniques. The standard κ–ω model is employed for simulation of the blood flow for the Reynolds number of 500 and 1000. We find that for Re = 500 the spiral component of the blood flow increases both the total pressure and velocity of the blood, and some significant differences are found between the wall shear stresses of the spiral and non-spiral induced flow downstream of the stenosis. The turbulent kinetic energy is reduced by the spiral flow as it induces the rotational stabilities in the forward flow. For Re = 1000 the tangential component of the blood velocity is most influenced by the spiral speed, but the effect of the spiral flow on the centreline turbulent kinetic energy and shear stress is mild. The results of the effects of the spiral flow are discussed in the paper along with the relevant pathological issues
