    Methodological Frameworks and Dimensions to Be Taken Into Consideration in Digital Health Technology Assessment: protocol for a Scoping Review

Although the existing HTA frameworks are useful tools for the evaluation of a wide range of health technologies, more and more experts, organizations across the world, and HTA agencies are highlighting the need to update or develop specific methodological frameworks for the evaluation of digital health technologies in order to take into account additional domains that cover these technologies’ intrinsic characteristics. Objective: The purpose of our scoping review is to identify the methodological frameworks that are used worldwide for the assessment of digital health technologies; determine what dimensions and aspects are being considered; and generate, through a thematic analysis, a proposal for a methodological framework that is based on the most frequently described dimensions in the literature. Methods: The scoping review will be performed in accordance with the guidelines established in the updated statement of the PRISMA-ScR (Preferred Reporting Items for Systematic Reviews and Meta-Analyses extension for Scoping Reviews). We will search for peer-reviewed and grey literature published between 2011 and the date of the search execution. The retrieved references will be reviewed in a single-blind manner by 2 independent authors, and their quality will be assessed by using the Critical Appraisal Skills Program tool. The ATLAS.ti software (Scientific Software Development GmbH) will be used for data extraction and to perform the thematic analysis. Results: The scoping review is currently (May 2022) in progress. It is expected to be completed in October 2022, and the final results of the research will be presented and published by November 2022. Conclusions: To our knowledge, no studies have been published to date that identify the existing methodological frameworks for digital HTA, determine which dimensions must be evaluated for correct decision-making, and serve as a basis for the development of a methodological framework of reference that health care systems can use to carry out this kind of assessment. Els programes de cribratge de lesions precursores de càncer anal amb citologia poden reduir la incidència i prevalença d’aquest càncer en HSH que viuen amb VIH. No obstant això, no hi ha estudis de cost-efectivitat realitzats al nostre entorn. Els estudis detectats mostren resultats contradictoris i amb limitacions en la modelització i selecció de paràmetres. Pel que fa a les recomanacions no sembla haver-hi un consens i l’evidència és sustentada per opinions d’experts. Els estudis futurs haurien de ser poder avaluar íntegrament els programes de cribratge.Se han identificado nueve guías de práctica clínica con recomendaciones para el cribado de lesiones precursoras de cáncer anal, no obstante no había un consenso con el uso exclusivo de la citología, ni en la frecuencia ni en la derivación. Los programas de cribado de lesiones precursoras de cáncer anal con citología pueden reducir la incidencia y prevalencia de este cáncer en HSH que viven con VIH. No obstante, no hay estudios de coste-efectividad realizados en nuestro entorno. Los estudios detectados muestran resultados contradictorios y con limitaciones en la modelización y selección de parámetros. En cuanto a las recomendaciones no parece haber un consenso y la evidencia es sustentada por opiniones de expertos. Los estudios futuros deberían ser poder evaluar íntegramente los programas de cribado.Nine clinical practice guidelines have been identified with recommendations for screening precursor lesions of cancer, however there was no consensus on the exclusive use of cytology, neither in frequency nor referral. Screening programs for cancer precursor lesions with cytology can reduce the incidence and prevalence of this cancer in MSM living with HIV. However, there are no cost-effectiveness studies carried out in our environment. The studies detected show contradictory results, limitations in the modeling and selection of parameters. Regarding the recommendations, there seems to be no consensus and the evidence is supported by expert opinions. Future studies should be able to fully evaluate screening programs

    Effectiveness of technology-based interventions in detection, prevention, monitoring and treatment of patients at risk or diagnosed with mild cognitive impairment: protocol for a systematic review

    Introduction: The gradual changes over the decades in the longevity and aging of European society as a whole can be directly related to the prolonged decline in the birth rate and increase in life expectancy. According to the WHO, there is an increased risk of dementia or other cognitive disorders as the population ages, which have a major impact on public health. Mild cognitive impairment (MCI) is described as a greater than expected cognitive decline for an individual's age and level of education, but that does not significantly interfere with activities of daily living. Patients with MCI exhibit a higher risk of dementia compared with others in the same age group, but without a cognitive decline, have impaired walking, and a 50% greater risk of falling. The urban lifestyle and advent of smartphones, mobility, and immediate access to all information via the internet, including health information, has led to a totally disruptive change in most general aspects. This systematic review protocol is aimed at evaluating the effectiveness of technology-based interventions in the detection, prevention, monitoring, and treatment of patients at risk or diagnosed with MCI. Methods and analysis: This review protocol follows the recommendations of the Preferred Reporting Items for Systematic Review and Meta-Analysis Protocols reporting guidelines. The search will be performed on MEDLINE (PubMed), CENTRAL, CINAHL Plus, ISI Web of Science, and Scopus databases from 2010 to 2020. Studies of interventions either randomized clinical trials or pre-post non-randomized quasi-experimental designs, published in English and Spanish will be included. Articles that provide relevant information on the use of technology and its effectiveness in interventions that assess improvements in early detection, prevention, follow-up, and treatment of the patients at risk or diagnosed with MCI will be included. Ethics and dissemination: Ethics committee approval is not required. The results will be disseminated in publications and congresses.
