    Living in a fringe environment: three Late Roman settlements in the Kharga Oasis (Egypt's Western Desert)

    This article presents a comparative analysis of three Late Roman sites located at the northern outskirts of the Kharga Oasis in Egypt's Western Desert: Umm al-Dabadib, Ayn al-Labakha, and the Gib/Sumayra Complex. These were part of the district of the Oasis Magna, which included the oases of Dakhla and Kharga. An analysis of their layout, including both shape and extent, is followed by an evaluation of their absolute and relative positions. These data are then compared to the administrative and historical contexts within which the three sites flourished. Both administrative and economic aspects are considered, as well as the presence of the army. The complex picture that emerges suggests that these three sites played several roles at the same time and were part of a large-scale strategic design that encompassed not only the Kharga Oasis but the entire Western Desert

    Safety and efficacy of vacuum assisted thrombo-aspiration in patients with acute lower limb ischaemia: The INDIAN Trial

    Objective: The aim was to evaluate the short term safety and effectiveness of the Penumbra/Indigo aspiration thrombectomy Systems (Penumbra Inc.) in patients with acute lower limb ischaemia. (ALLI). Recently, endovascular vacuum assisted thrombectomy devices, similar to those used in the management of acute ischaemic stroke, have become available for peripheral arteries, but data are still scarce.Methods: To assess vessel patency, a modified Thrombolysis in Myocardial Infarction (TIMI) classification, called TIPI (Thrombo-aspiration In Peripheral Ischaemia), is proposed. The TIPI flow is assessed at presentation, immediately after treatment with the study device, and after all adjuvant procedures. The primary outcome is the technical success of the thrombo-aspiration with the investigative system, defined as near complete or complete revascularisation TIPI 2 - 3. Safety and clinical success rate were collected at one month.Results: One hundred and fifty patients were enrolled. The mean age was 72.4 years and 73.3% were male. Rutherford grade on enrolment was I in 16%, IIa in 40.7%, and IIb in 43.3% with a mean ankle brachial index of 0.19. Primary technical success (TIPI 2 - 3 flow) was achieved in 88.7% of patients. Adjunctive procedures included angioplasty/stenting of chronic atherosclerotic lesions (n = 39), thrombolysis (n = 31), covered stenting (n = 15), and supplementary Fogarty embolectomy (n = 6). After all interventions, assisted primary technical success was 95.3% (TIPI 2 - 3 in 143/150). No systemic bleeding complications or device related serious adverse events were reported. At one month follow up, one death, and one below the knee amputation were recorded. Primary patency was 92% (138/150), and the re-intervention rate was 7.33%, resulting in an assisted primary and secondary patency of 94% and 99.33%, respectively.Conclusion: Results from the INDIAN registry reveal that mechanical thrombectomy using the Indigo system is safe and effective for revascularisation of ALLI as a primary therapy

    Juno/JIRAM: Planning and commanding activities

    In the context of space missions, where science is the most important goal, careful planning and detailed commanding are fundamental. The planning and commanding phases are activities whose complexity depends on the instrument characteristics, environmental constraints and scientific goals. The purpose of this work is to describe in detail these activities for the Jovian Infrared Auroral Mapper (JIRAM) on board the Juno spacecraft, a NASA mission to Jupiter. To maximize the scientific return, we fully employ the flexibility offered by the JIRAM operational modes to efficiently plan observations of various Jovian targets, in spite of the harsh Jovian radiation environment and the spinning state of the Juno spacecraft. Moreover, the JIRAM observations are limited by the challenging pointing and timing scheme of the mission, which impose constraints on both the observation planning and instrumental commanding

    How future surgery will benefit from SARS-COV-2-related measures: a SPIGC survey conveying the perspective of Italian surgeons

    COVID-19 negatively affected surgical activity, but the potential benefits resulting from adopted measures remain unclear. The aim of this study was to evaluate the change in surgical activity and potential benefit from COVID-19 measures in perspective of Italian surgeons on behalf of SPIGC. A nationwide online survey on surgical practice before, during, and after COVID-19 pandemic was conducted in March-April 2022 (NCT:05323851). Effects of COVID-19 hospital-related measures on surgical patients' management and personal professional development across surgical specialties were explored. Data on demographics, pre-operative/peri-operative/post-operative management, and professional development were collected. Outcomes were matched with the corresponding volume. Four hundred and seventy-three respondents were included in final analysis across 14 surgical specialties. Since SARS-CoV-2 pandemic, application of telematic consultations (4.1% vs. 21.6%; p < 0.0001) and diagnostic evaluations (16.4% vs. 42.2%; p < 0.0001) increased. Elective surgical activities significantly reduced and surgeons opted more frequently for conservative management with a possible indication for elective (26.3% vs. 35.7%; p < 0.0001) or urgent (20.4% vs. 38.5%; p < 0.0001) surgery. All new COVID-related measures are perceived to be maintained in the future. Surgeons' personal education online increased from 12.6% (pre-COVID) to 86.6% (post-COVID; p < 0.0001). Online educational activities are considered a beneficial effect from COVID pandemic (56.4%). COVID-19 had a great impact on surgical specialties, with significant reduction of operation volume. However, some forced changes turned out to be benefits. Isolation measures pushed the use of telemedicine and telemetric devices for outpatient practice and favored communication for educational purposes and surgeon-patient/family communication. From the Italian surgeons' perspective, COVID-related measures will continue to influence future surgical clinical practice

    L'orizzonte dei re. Studio sul mundus imaginalis delle sepolture egizie

    Studiare una cultura come quella dell’Antico Egitto con strumenti propri della contemporaneità non permette di comprendere una civiltà che continua a parlarci attraverso le sue rovine. In un universo culturale dove assume una valenza fondamentale il concetto di mundus imaginalis – il mondo delle immagini – si rende necessario uno sguardo che sappia andare oltre l’analisi formale e strutturale. Nel volume vengono studiati i principali tipi di tombe risalenti al periodo compreso tra l’inizio del Nuovo Regno e la fine della XIX dinastia, per sottolineare un aspetto chiave dell’architettura antica, in particolare di quella egizia: il suo essere fondata su archetipi non solo formali quanto soprattutto religiosi, tradotti in forme architettoniche. Così se stanze, pozzi e corridoi delle sepolture reali seguono una disposizione in linea con la narrazione mitica, lo spazio stesso diventa liturgia. Attraverso una panoramica delle costruzioni dell’Antico Egitto l’autore porta alla luce l’elemento unificante di queste strutture, che condividono un comune substrato concettuale volto alla sacralizzazione degli spazi in quanto reali mundi imaginalis, emanazioni dell’ordine cosmico chiamato ma’at
