    Gene drive research is progressing towards future field evaluation of modified mosquitoes for malaria control in sub-Saharan Africa. While many literature sources and guidance point to the inadequacy of individual informed consent for any genetically modified mosquito release, including gene drive ones, (outside of epidemiological studies that might require blood samples) and at the need for a community-level decision, researchers often find themselves with no specific guidance on how that decision should be made, expressed and by whom. Target Malaria, the Kenya Medical Research Institute and the Pan African Mosquito Control Association co-organised a workshop with researchers and practitioners on this topic to question the model proposed by Target Malaria in its research so far that involved the release of genetically modified sterile male mosquitoes and how this could be adapted to future studies involving gene drive mosquito releases for them to offer reflections about potential best practices. This paper shares the outcomes of that workshop and highlights the remaining topics for discussion before a comprehensive model can be designed

    Balancing professional autonomy and authority at the margins of a fragile state:Front-line health workers' experiences in Burkina Faso

    The availability of diagnostic equipment, medical products and basic infrastructure is limited in most low-income societies. Poor motivation among health workers as well as recruitment and retention problems are key factors contributing to poor health care services in developing countries. The current paper describes how the front line cope with these difficult working conditions. Data for this study come from anthropological fieldwork in two districts of Burkina Faso and include a total of 27 unstructured and 40 semi-structured interviews with staff at dispensaries and medical centres in the two districts. Analytically, we make a distinction between their professional autonomy and their professional authority. We find that while the health workers experience a certain degree of professional autonomy, in the sense that they rely on their own clinical judgement and their discretion as decision makers and gatekeepers, their professional authority is constantly challenged when working at the margins of the state. Through improvisations and bricolaging, they compensate for the many shortcomings, but many of them feel that their skills are waning and that their professional identity threatened. Thus, massive strengthening of the front-line health worker’s capacity is imperative for future improvement of health care services in Burkina Faso

    Évaluation des déterminants socioculturels influençant l’adoption de la chimioprévention du paludisme saisonnier au Burkina Faso : une étude qualitative

    Résumé : La Chimioprévention du Paludisme Saisonnier (CPS) est une stratégie efficace de lutte contre le paludisme chez les enfants de 3 à 59 mois. Depuis 2012, sur la base des données factuelles, la CPS été recommandée par l’Organisation Mondiale de la Santé (OMS), aux pays d’Afrique subsaharienne dont le Burkina Faso où la transmission du paludisme est saisonnière. Dans ces pays d’Afrique subsaharienne, le paludisme constitue un problème majeur de santé publique d’où l’introduction de la CPS au Burkina Faso en 2014 dans les sites pilotes. Mais depuis 2018, les campagnes CPS couvrent tous les 70 districts sanitaires du territoire national. Cependant, en dépit de ces campagnes de distribution de masse, l’adoption de la CPS par les parents au profit de leurs enfants demeure faible sur le terrain à Bobo-Dioulasso. Cette étude vise à analyser les déterminants socioculturels qui influencent l’adoption de la CPS par les parents au profit de leurs enfants. Nous avons privilégié une recherche qualitative où plusieurs techniques comme l’observation directe en temps réel, la recherche documentaire, l’entretien individuel approfondi, le focus group et le récit de vie ont été mobilisées. Les données ont été collectées auprès de la population générale et des acteurs de la santé avant, pendant et après les campagnes CPS dans la commune de Bobo-Dioulasso, sur une période de huit (08) mois allant de mars à octobre 2023. Au total, 87 entretiens individuels, 08 focus groups et 04 récits de vie ont été réalisés. Il ressort des résultats que les principaux déterminants socioculturels entravant l’adoption de la CPS ont trait essentiellement aux représentations sociales du paludisme, aux perceptions populaires autour de la CPS, aux croyances, aux normes sociales et du genre dans la communauté.     Operationalizing stakeholder engagement for gene drive research in malaria elimination in Africa—translating guidance into practice

    Abstract Gene drive mosquitoes are increasingly considered a potential transformational tool for vector control of malaria mosquitoes. As part of efforts to promote responsible research in this field, a number of guidance documents have been published by the World Health Organization, National Academies and expert groups. While virtually all recent guidance documents on gene drive research stress the importance of stakeholder engagement activities, no specific guidelines on implementing them have been established. Target Malaria, a not-for-profit research consortium developing a vector-control gene drive approach to eliminate malaria, has reflected on how its stakeholder engagement strategy translates engagement guidance documents into practice. The project analysed and addressed the tension between the context specificities and the international recommendations. The engagement strategy combines published recommendations for responsible gene drive research, information collected from the local context where the project operates and a set of principles guiding the choices made. This strategy was first developed during the early phases of the project’s research, years ahead of any activities with gene drive mosquitoes in those countries of operations. These earlier activities, and their related engagement, allow the project to develop and adapt an engagement strategy appropriate for potential gene drive research in its field site countries. This paper offers a description of a stakeholder engagement strategy operationalization based on (1) adaptation to stakeholder preferences, (2) inclusiveness and (3) empowerment and accountability. The authors hope to offer concrete examples to support other projects with the development and implementation of their engagement strategies with particular attention to the co-development principle

    Factors Influencing Second and Third Dose Observance during Seasonal Malaria Chemoprevention (SMC): A Quantitative Study in Burkina Faso, Mali and Niger

    This study aims to evaluate the factors influencing the adherence to the 2nd and 3rd doses of Amodiaquine (AQ) during seasonal malaria chemoprevention (SMC) in Burkina Faso, Mali, and Niger. Overall, 3132 people were interviewed during surveys between 2019 and 2020 in 15 health districts. In Burkina Faso, Mali, and Niger, the proportions of non-adherence were 4.15%, 5.60%, and 13.30%, respectively, for the 2nd dose and 3.98%, 5.60% and 14.39% for the 3rd dose. The main cause of non-adherence to the 2nd and 3rd doses was other illnesses in 28.5% and 29.78%, respectively, in Burkina Faso, 5.35% and 5.35% in Mali and 1.6% and 0.75% in Niger. It was followed by vomiting in 12.24% and 10.63% for Burkina and 2.45% and 3.78% in Niger. The last cause was refusal in 6.12% and 4.25% in Burkina, 33.9% and 15.25% in Mali and 0.8% and 1.51% in Niger. Non-adherence of doses related to parents was primarily due to their absence in 28.5% and 27.65% in Burkina, 16.07% and 16.07% in Mali and 7.37% and 6.06% in Niger. Traveling was the second cause related to parents in 12.24% and 12.76% in Burkina, 19.64% and 19.64% in Mali and 0.81% and 0.75% in Niger. Non-adherence related to community distributors was mainly due to missing the doses in 4.08% and 4.25% in Burkina, 23.21% and 23.21% in Mali, 77.04% and 76.51% in Niger. Our study reported very small proportions of non-adherence to 2nd and 3rd doses of SMC and identified the main causes of non-adherence. These findings will provide helpful information for policymakers and public health authorities to improve adherence to SM
