    Clinical Support through Telemedicine in Heart Failure Outpatients during the COVID-19 Pandemic Period: Results of a 12-Months Follow Up

    Background: Heart failure (HF) patients are predisposed to recurrences and disease destabilizations, especially during the COVID-19 outbreak period. In this scenario, telemedicine could be a proper way to ensure continuous care. The purpose of the study was to compare two modalities of HF outpatients’ follow up, the traditional in-person visits and telephone consultations, during the COVID-19 pandemic period in Italy. Methods: We conducted an observational study on consecutive HF outpatients. The follow up period was 12 months, starting from the beginning of the COVID-19 Italy lockdown. According to the follow up modality, and after the propensity matching score, patients were divided into two groups: those in G1 (n = 92) were managed with traditional in-person visits and those in G2 (n = 92) were managed with telephone consultation. Major adverse cardiovascular events (MACE) were the primary endpoints. Secondary endpoints were overall mortality, cardiovascular death, cardiovascular hospitalization, and hospitalization due to HF. Results: No significant differences between G1 and G2 have been observed regarding MACE (p = 0.65), cardiovascular death (p = 0.39), overall mortality (p = 0.85), hospitalization due to acute HF (p = 0.07), and cardiovascular hospitalization (p = 0.4). Survival analysis performed by the Kaplan–Meier method also did not show significant differences between G1 and G2. Conclusions: Telephone consultations represented a valid option to manage HF outpatients during COVID-19 pandemic, comparable to traditional in-person visits

    The second half of the XX Century saw the birth of a process of returning to civil cohabitation in the mark of universal values which are encapsulated in the dignity of the person and the legal-ethical imperative: humana dignitas servanda est. This essay discusses how processes of globalisation and de-territorialisation of politics converge in driving governance to express itself with reference to a common normative paradigm and in the perspective of an institutional architecture which fosters the (re)distribution of political processes and government functions over various territorial levels: from that of a local municipality to the United Nations system. The universal Law of human dignity is the compass whose power of guidance is directly proportionate to the order of magnitude of the needs of multi-level governance in the whole world-space. In this essay we propose interpreting the current highly complex transition towards a more just and peaceful world order through the metaphor of a mosaic. For a mosaic to be recognisable, it is not enough to have the tesserae, but they must be put together following a predetermined design. As concerns world order, the essay suggests that all the tesserae are there, but the mosaic cannot be made if no-one takes the trouble to put it together. It further posits that the \u201ctesserae\u201d which are available for composing, advancing and completing the building of positive peace are: international human rights law, international democracy, multi-level governance, human security, the United Nations and the other legitimate multilateral institutions

Non ripropone dunque l\u2019ennesima diagnosi sulla situazione della politica internazionale e sulle disavventure del multilateralismo, intende piuttosto aggiornare la riflessione propositiva in materia di riforma delle Nazioni Unite quale si va conducendo, ormai da molti anni ma con recente forte accelerazione, nel \u2018cantiere\u2019 progettuale di societ\ue0 civile globale, all\u2019interno del quale si segnalano il World Federalist Movement, il World Social Forum, lo International Network for a United Nations Second Assembly , INFUSA, il South Centre, pi\uf9 di recente UBUNTU, senza dimenticare l\u2019organica riflessione condotta dal \u201cWorld Order Models Project\u201d, WOMP, la \u201cCommission on Global Governance\u201d, il Civil Society Millennium Forum alle Nazioni Unite nell\u2019anno 2000 e, in Italia a partire dal 1992, le iniziative collegate alla Marcia della pace Perugia-Assisi e all'Assemblea dell'ONU dei Popoli. In questo cantiere di nuova, autentica cultura di global governance emerge con chiarezza una strategia progettuale caratterizzata dal collegamento tra l\u2019obiettivo del rafforzamento e l\u2019obiettivo della democratizzazione delle Nazioni Unite all\u2019insegna dei diritti umani, della pace e dei principi di un\u2019economia di giustizia

    L'Annuario italiano dei diritti umani 2015, quinto della serie, offre dati aggiornati su come l\u2019Italia opera nell\u2019adattare la propria legislazione e le proprie politiche agli obblighi derivanti dal diritto internazionale dei diritti umani e dagli altri impegni che il Governo ha volontariamente assunto di fronte alla comunita\u300 internazionale. Sono presi in esame i piu\u300 significativi atti posti in essere dalle istituzioni nazionali e locali a livello interno e internazionale, compresi quelli delle istituzioni educative e accademiche e delle organizzazioni di societa\u300 civile.     The Italian Yearbook of Human Rights 2015 provides a dynamic and up-to-date overview of the measures Italy has taken to adapt its legislation and policies to international human rights law and to comply with commitments voluntarily assumed by the Italian Government at the international level. The 2015 Yearbook surveys the activities of the relevant national and local Italian actors, including governmental bodies, civil society organisations and universities. It also presents reports and recommendations that have been addressed to Italy in 2014 by international monitoring bodies within the framework of the United Nations, the Council of Europe and the European Union. Finally, the Yearbook provides a selection of international and national case-law that casts light on Italy's position vis-\ue0-vis internationally recognised human rights. \u201cItaly and Human Rights in 2014: the Challenge of National/International Constitutional Synergism\u201d is the focus of the introductory section of the Yearbook. The complex network of monitoring actions carried out by the supranational bodies, and the relative reporting requirements Italy must meet, can only be viewed in the context of reciprocal exchange and strengthening between the provisions enshrined in the national Constitution and international human rights law. The Italian Agenda of Human Rights 2015 represents an updated orientation tool intended to support the commitment taken by the Italian Government in the framework of the second Universal Periodic Review (October 2014) before the UN Human Rights Council
