    HPLC analysis of biomarkers of Toluene and Xylene in human urine samples

    Introduction: Urinary Hippuric acid (HA) and Methylhippuric acid (MHA) are considered reliable biomarkers for monitoring exposure to toluene and xylenes. A simple and robust HPLC method for simultaneous determination of HA and MHA in urine samples was developed and validated as per Internation Conference on Harmonization (ICH) Q2 guidelines. Methods: Mobile phase consisting of two solvents A & B, solvent A was 0.1% TFA in water, and solvent B was 0.1% TFA in acetonitrile.  Separation was performed in gradient elution mode and the time program was as follows - Time (min)/%B: 0/20, 4/20, 11/80, 11.1/90, 16/90, 16.1/20, 25/20. The detection wavelength was 205 nm. Results: The limits of detection (LOD) and quantification (LOQ) of HA in urine were 0.3 and 1.0 mg/L, and for MHA were 0.25 mg/L and 1.0 mg/L respectively and the recovery for HA and MHA were 95% and 98.9% respectively.  Conclusion: The developed method was successfully applied for the analysis of urine samples

    Efficacy of vijaysar, aloevera alone and their combination in the treatment of newly diagnosed cases of type 2 diabetes mellitus: a randomized single blind prospective study

    Background: Diabetes mellitus is a disease was known since ancient time and all system of medicine over world were tried to cure this disease. Unfortunately the numbers of diabetes patients are increasing day by day due to many risk factors such as sedentary life, obesity etc.Methods: A total of 120 patients with newly diagnosed type 2 diabetes mellitus attending OPD of UPUMS, Saifai, Etawah, Uttar Pradesh, India, were included in the study. Patients were divided into 4 groups. Group-1, 2, 3 and 4 received Aloevera, Vijaysar, Aloevera + Vijaysar and Glimepiride respectively for thirteen weeks. Sample for fasting blood sugar and postprandial blood sugar were measured at baseline, 1st, 2nd, 3rd, 5th, 8th and at 13th week. HbA1c and Lipid profile were measured at baseline and at thirteen week.Results: Significant decrease in FPG, 2hPG, and HbA1c level were achieved in all groups but effects was maximum in Glimepiride group. Vijaysar had shown better glucose control than Aloevera as well as Aloevera + Vijaysar group. Synergism was shown by both herbal drugs for FPG control but not for 2hPG. The Effect of Vijaysar alone on 2hPG was similar to Glimepiride.Conclusions: Vijaysar could be a promising herbal drug for the treatment of mild uncomplicated cases of type-2 diabetes mellitus; however both drugs have shown synergism for FPG control. Both herbal drugs were safe during our study, only one patient of Vijaysar group had complaint of diarrhoea, which was subsided one week later

    Outcome of substitutional urethroplasty by using dorsal onlay buccal mucosal graft: our experience of 10 years

    Background: The term urethral stricture is anterior urethral disease, or a scarring process involving the spongy erectile tissue of the corpus spongiosum. Methods: Total 127 patients were included in the study. Patient selection criteria for urethroplasty by using buccal mucosal graft was length of stricture more than 2 cm, deep spongiofibrosis, failed optical urethrotomy for 3 times, adequate oral hygiene and proper buccal mucosa. Results: Length of stricture varied from 3.2 to 14 cm. In first postoperative week, 20.47% (n=26) patients developed minor wound infection culture negative seroma formation in stitch line in the skin. Donor site complications like eating and drinking difficulty, dysguesia, pain, sensitivity, speaking disorders were not found in any patient while oral tightness was noted in 43.30% (n=55) of patients. On postoperative follow-up mean peak urinary flow rate (Qmax) was 28.0 ml/sec (range 20.0-30.6 ml/sec). After a mean follow-up of 8.8 months range (1 month to 33 months) overall success rate was 90.55% (n=115). Conclusions: Buccal mucosa is an excellent graft material for substitution free graft urethroplasty in case of long anterior urethral stricture with excellent success rate. Success rate of dorsal onlay substitution free buccal mucosal graft urethroplasty is affected by length of stricture and aetiology of strictures. Lichen sclerosus having moderate success rate of urethroplasty and higher rate of complication and failure rate in 1-stage buccal mucosal urethroplasty and can be considered for two stage urethroplasty in case of very long stricture of anterior urethra of lichen sclerosus origin

    Evaluation of antidepressant and analgesic activity of tapentadol with mirtazapine: an experimental study

    Background: Data comparing tapentadol with an antidepressant is limited. A comparison of tapentadol with mirtazapine at different dose has not been performed, the other antidepressant in the same therapeutic class with a significant market share, has been undertaken. In the absence of relevant data to assess the place that tapentadol should occupy in the therapeutic arsenal, indirect comparisons are the most rigorous way to go. We conducted a study evaluate antidepressant and analgesic activity of tapentadol with mirtazapine at different doses in Swiss albino mice.Methods: Tapentadol was administered at 10, 20 and 40 mg/kg (i.p) once daily for 14 days to swiss albino mice of either sex. The immobility period for antidepressant activity of mice were recorded in forced swim test and reaction time for analgesic activity of mice were recorded in tail flick test of the control and drug treated group. The antidepressant and analgesic activity of tapentadol (10, 20, 40 mg/kg i.p) was compared with that of mirtazapine (3, 5, 7 mg/kg i.p), administered for 14 days.Results: Tapentadol produced better antidepressant at (20, 40 mg/kg), but less at 10 mg/kg and significant analgesic activity at all the three doses, as indicated by reduction in immobility times and increase in reaction time as compared to control. Mirtazapine produced no antinociceptive activity at 3 mg/kg, but significant at 5, 7 mg/kg and showed better antidepressant activity at all the three doses in mice. The result of this study indicates the better analgesic activity of tapentadol at all the doses and least antidepressant activity at 10 mg/kg, as compared to mirtazapine which has shown better antidepressant activity at all the three doses but no analgesic activity at 3 mg/kg.Conclusion: It can be concluded that tapentadol is a better drug in case of depression associated with pain compared to mirtazapine in mice

    Assessment of oral health among seafarers in Mundra Port, Kutch, Gujarat: a cross-sectional study

    Background: Seafarer is a person who navigates waterborne vessels or assists as a crewmember in their operation and maintenance in all tough weather, but little research has been done to identify conditions that may lead to assess seafarer general health as well as oral health. Aim: To assess oral diseases including dental caries and periodontal conditions among seafarer’s population arrived in Mundra Port, Kutch, Gujarat, India. Materials and methods: A descriptive cross-sectional survey was conducted to assess oral health condition of seafarer community of Mundra Taluka of Kutch District, Gujarat, India, from July 2014 to September 2014. Results: Total of 385 subjects participated in the survey. Adverse habits show the overall 72.3% prevalence among the study population. Occurrence rate of caries, periodontal disease and prosthetic status were 88%, 75.1% and 6.5%, respectively. The best predictors for Decayed Missing Filled Teeth (DMFT), Community Periodontal Index (CPI) and prosthetic status were oral hygiene practices, adverse habit and educational status. Conclusions: Findings of the present study suggest that oral health condition of seafarer community was relatively poor, with high caries prevalence and poor periodontal health. This epidemiological survey has provided baseline information to underpin the implementation of oral health programmes

    Effect of Salinity and Sodicity on Vegetable Production and Remedial Measures: A Review

    The word salinity comes from the Latin word salinium which means "salt cellar" and it means "position or quality of being". Sodicity indicates the amount of sodium converted to calcium and magnesium in soil. High sodicity suppresses plant growth due to sodium toxicity and nutrient imbalance in plants, as well as low availability of mineral nutrients in the soil. Salt stress is the cause of the slow growth and growth of plants and leads to changes in yield and quality in a variety of crops. Plants provide a complex response to salt and changes in the morphology, physiology and metabolism of plants are observed. The effect of salt on various vegetable plants namely beetroot, cabbage, capsicum, kabuli chana, coriander, fenugreek, lettuce, onion, tomato, potato was reviewed. Salinity was adversely damaged as a result of salinity: Seed formation, survival percent, phonological attribute, growth and yield, its components, dry and fresh weight were affected. Photosynthesis and respiratory rates of plants were reduced. Salinity reduced total carbohydrate, fatty acid, and protein content but notably increased amino acid levels. The growth of asparagus and tomatoes was more concentrated in sodic soils than saline soils, indicating that asparagus and tomatoes are sensitive to sodicity. Beans also die in sodic soils, indicating that the beans are very sensitive to sodicity. The negative effects of alkaline water on the addition of gypsum and FYM have shown a significant increase in plant growth and yield limits

    Prenatal detection of Kaposiform Hemangioendothelioma with Kasabach–Merritt phenomenon: a case report

    Abstract Background Kaposiform Hemangioendothelioma is a rare vascular neoplasm that typically presents in neonates and infancy. Most of the cases of Kaposiform Hemangioendothelioma are reported in neonates (60%) and infancy (93%), and very few cases have been reported prenatally. We describe here a case which was diagnosed prenatally in the 3rd trimester with rapid growth of upper limb and appearance of non-immune hydrops. Case presentation A 25-year-old primigravida, an antenatally booked case, presented for routine third trimester level III scan at 34-week gestation. Level I scan for nasal bone and nuchal translucency at 12 weeks and level II anomaly scan at 20 weeks were normal. Level III ultrasound scan revealed increased echogenicity and hypertrophy of soft tissues of right upper limb and trunk involving both the subcutaneous and muscular compartments. Fetal middle cerebral artery Doppler, echocardiography and magnetic resonance imaging revealed fetal anemia, cardiac failure, and non-immune hydrops. These findings with hemodynamic changes and rapid evolution were suggestive of Kaposiform Hemangioendothelioma with Kasabach–Merritt phenomenon. Conclusion The diagnosis of Kaposiform Hemangioendothelioma should be suspected in prenatal period if there is unilateral hypertrophy of limb which has evolved rapidly over a short interval of time and shows findings suggestive of Kasabach–Merritt Phenomenon. Prenatal presentation is life threatening if it is associated with poor fetal hemodynamics. Prenatal counseling in such cases should include advice on termination of pregnancy with emphasis on neonatal management
