    Remodelación de los tejidos blandos alrededor de implantes inmediatos y diferidos.Estudio experimental en perros Beagle.

    Antecedentes: Los protocolos de implantes inmediatos (tipo 1) y diferidos (tipo 2) son considerados unánimemente como las dos alternativas principales para el reemplazo de un diente desahuciado. Sin embargo, todavía falta una comparación directa de los dos, sobre la organización de los tejidos perimplantarios, tanto duros como blandos. Objetivos: Investigar la presencia de diferencias significativas en la altura y el grosor del tejido blando alrededor de implantes inmediatos y precoces en fases de cicatrización temprana y tardía. Métodos: 8 perros de raza Beagle han recibido 32 implantes inmediatos (test) y 32 implantes diferidos (control). Los implantes inmediatos fueron puestos en los alvéolos mésiales de 3P3 y 4P4, mientras los diferidos fueron puestos en los alveolos mésialed de 1P1 y 2P2, 8 semanas después de la emi-sección de los dientes y la extracción de las raíces mésiales. Los implantes diferidos Los perros fueron sometidos a eutanasia después 2 y 8 semanas para el análisis histológico. Mitad de los sitios implantares fueron procesados con técnica calcificada y la otra mitad con técnica descalcificada. Resultados: En las secciones descalcificadas, la anchura biológica perimplantaria fue significativamente mayor a las 2 semanas (I-J = 1,5 mm; p = 0,028) y casi significativamente mayor a las 8 semanas (I-J = 1 mm; p = 0,088). Sin embargo, no se observaron diferencias significativas en las secciones calcificadas. Con respecto al grosor del tejido, se observaron valores consistentemente más altos en el grupo control a las 2 semanas (de 0,69 a 1,51 mm), pero esta diferencia se redujo abundantemente a las 8 semanas (0,2 a 0,6 mm) y casi siempre perdió significación estadística. Conclusiones: A las 8 semanas de cicatrización, la formación de la mucosa perimplantaria alrededor de implantes inmediatos y diferidos fue asociada a ninguna diferencia relevante en el espesor del tejido blando. En las secciones descalcificadas, se observó una anchura biológica approx. mayor de 1 mm en los implantes inmediatos. Dicha diferencia implicaba tanto el epitelio de unión como el tejido conectivo de aprox. 0,5 mm cada uno

    Techniques on vertical ridge augmentation: Indications and effectiveness.

    Vertical ridge augmentation techniques have been advocated to enable restoring function and esthetics by means of implant-supported rehabilitation. There are three major modalities. The first is guided bone regeneration, based on the principle of compartmentalization by means of using a barrier membrane, which has been demonstrated to be technically demanding with regard to soft tissue management. This requisite is also applicable in the case of the second modality of bone block grafts. Nonetheless, space creation and maintenance are provided by the solid nature of the graft. The third modality of distraction osteogenesis is also a valid and faster approach. Nonetheless, owing to this technique's inherent shortcomings, this method is currently deprecated.     Faecal microbiota of individuals with autism spectrum disorder

    Many children with autistic spectrum disorders (ASDs) suffer from gastrointestinal problems such as diarrhoea, constipation and abdominal pain. Such symptoms may be due to a disruption of the indigenous gut microbiota promoting the overgrowth of potentially pathogenic micro-organisms. These observations have stimulated investigations into possible abnormalities of intestinal microbiota in autistic patients. The purpose of the present study was to determine if a relationship exists between ASD severity (mild – severe) and GI microbial populations. The faecal microbiota of 22 male and 6 female participants with ASDs (aged 7 ± 6 years) were analyzed by standard microbial culture methods and compared within-group (based on ASD severity) and with a standard laboratory reference range. Comparisons between children with mild ASD and those with moderate to severe ASD, as well as comparisons to a neurotypical control group previously reported, revealed that no significant differences appear to exist in the composition of the gut microbiota. Nevertheless, examination of each individual’s gut microbial composition showed 10 cases of unusual findings witch means 1out of 3 cases have unusual microbiota. Our data do not support consistent GI microbial abnormalities in ASD children, but the findings do suggest that aberrations may be found in a minority subset of ASD children. Further studies are required to determine the possible association between the microbiota and gastrointestinal dysfunctions in a subset of children with both ASD and gastro-intestinal problems

    Genetic variation associated with hypersensitivity to mercury

    OBJECTIVES: Very little is known about mechanisms of idiosyncratic sensitivity to the damaging effects of mercury (Hg); however, there is likely a genetic component. The aim of the present study was to search for genetic variation in genes thought to be involved in Hg metabolism and transport in a group of individuals identified as having elevated Hg sensitivity compared to a normal control group. MATERIALS AND METHODS: Survivors of pink disease (PD; infantile acrodynia) are a population of clinically identifiable individuals who are Hg sensitive. In the present study, single nucleotide polymorphisms in genes thought to be involved in Hg transport and metabolism were compared across two groups: (i) PD survivors (n = 25); and (ii) age- and sex-matched healthy controls (n = 25). RESULTS: Analyses revealed significant differences between groups in genotype frequencies for rs662 in the gene encoding paraoxanase 1 (PON1) and rs1801131 in the gene encoding methylenetetrahydrofolate reductase (MTHFR). CONCLUSIONS: We have identified two genetic polymorphisms associated with increased sensitivity to Hg. Genetic variation in MTHFR and PON1 significantly differentiated a group formerly diagnosed with PD (a condition of Hg hypersensitivity) with age- and gender-matched healthy controls

    Evaluation of biogenic amines in the faeces of children with and without autism by LC-MS/MS

    Previous researchers have postulated that gastrointestinal bacteria may contribute to the development and maintenance of Autism Spectrum Disorders (ASD). There is evidence based on quantitative evaluation of the gastrointestinal bacterial population in ASD that this is unlikely and an alternate mechanism will be examined where the bacteria may contribute to the development of ASD via their metabolic products and the role of biogenic amines (BAs) will be investigated. In humans, BAs influence a number of physiological processes via their actions as neurotransmitters, local hormones and gastric acid secretion. Various amines have been implicated in several medical conditions such as schizophrenia and colon cancer. To date, the relationship between BAs and autism has not been explored. This study has been designed to identify differences (and/or similarities) in the level of Bas in faecal samples of autistic children (without gastrointestinal dysfunction: n = 14; with gastrointestinal dysfunction; n = 21) and their neurotypical siblings (n = 35) by LC-MS/MS. Regardless of the diagnosis, severity of ASD and gastrointestinal dysfunction there were no significant differences found between the groups. The findings suggest that BAs in the gastrointestinal tract do not play a role in the pathophysiology of gastrointestinal dysfunction associated with ASD

    Joint estimation of relaxation and diffusion tissue parameters for prostate cancer with relaxation-VERDICT MRI

    This work presents a biophysical model of diffusion and relaxation MRI for prostate called relaxation vascular, extracellular and restricted diffusion for cytometry in tumours (rVERDICT). The model includes compartment-specific relaxation effects providing T1/T2 estimates and microstructural parameters unbiased by relaxation properties of the tissue. 44 men with suspected prostate cancer (PCa) underwent multiparametric MRI (mp-MRI) and VERDICT-MRI followed by targeted biopsy. We estimate joint diffusion and relaxation prostate tissue parameters with rVERDICT using deep neural networks for fast fitting. We tested the feasibility of rVERDICT estimates for Gleason grade discrimination and compared with classic VERDICT and the apparent diffusion coefficient (ADC) from mp-MRI. The rVERDICT intracellular volume fraction fic discriminated between Gleason 3 + 3 and 3 + 4 (p = 0.003) and Gleason 3 + 4 and ≥ 4 + 3 (p = 0.040), outperforming classic VERDICT and the ADC from mp-MRI. To evaluate the relaxation estimates we compare against independent multi-TE acquisitions, showing that the rVERDICT T2 values are not significantly different from those estimated with the independent multi-TE acquisition (p > 0.05). Also, rVERDICT parameters exhibited high repeatability when rescanning five patients (R2 = 0.79–0.98; CV = 1–7%; ICC = 92–98%). The rVERDICT model allows for accurate, fast and repeatable estimation of diffusion and relaxation properties of PCa sensitive enough to discriminate Gleason grades 3 + 3, 3 + 4 and ≥ 4 + 3

    Pinus mugo Krummholz dynamics during concomitant change in pastoralism and climate in the central Apennines

    The dynamics of Pinus mugo krummholz during concomitant change in pastoral land use and climate in central Italy since the mid-20th century was investigated. Krummholz dynamics were detected using sequential aerial photography and fitted to a logistic regression model with elevation, grazing, proximity to beech forest, and proximity to krummholz as explanatory variables. Dendrochronological series were correlated with temperature and precipitation and fitted to a linear model. During this period krummholz doubled in extent and migrated 35–65 m upslope. Expansion was positively associated with krummholz proximity, residual pastoral grazing, and proximity to beech forest beyond 10 m and negatively associated with elevation and beech forest closer than 10 m. The logistic regression model forecasts krummholz migration by an additional 30 m upslope by 2060. During the 20th century, winter and spring minimum temperatures increased but did not result in increased radial stem growth of P. mugo.     Reservoir-Excess Pressure Parameters Independently Predict Cardiovascular Events in Individuals With Type 2 Diabetes

    The parameters derived from reservoir-excess pressure analysis (RPA) have prognostic utility in several populations. However, evidence in type 2 diabetes (T2DM) remains scarce. We determined if these parameters were associated with T2DM, and whether they would predict cardiovascular events in individuals with T2DM.We studied 306people with T2DM and cardiovascular disease (CVD)(DMCVD:70.4±7.8yrs), 348people with T2DM but without CVD (DM:67.7±8.4yrs) and 178peoplewithout T2DM or CVD (CTRL:67.2±8.9yrs). RPA-derived parameters including reservoir pressure integral (INTPR), peak reservoir pressure (MAXPR), excess pressure integral (INTXSP), systolic rate constant (SRC) and diastolic rate constant (DRC) were obtained by radial artery tonometry. INTPR was lower in DMCVD and DM than CTRL. MAXPR was lower, and INTXSP was greater in DMCVD than DM and CTRL. SRC was lower in a stepwise manner among groups(DMCVD<DM<CTRL).DRC was greater in DMCVD than CTRL. In the sub group of individuals with T2DM (n=642), 14 deaths (6 cardiovascular and 9non-cardiovascular causes) and 108cardiovascular events occurred during a 3-yr follow-up period. Logistic regression analysis revealed that INTPR [odds ratio 0.59(95%CI:0.45-0.79)] and DRC [odds ratio 1.60(95%CI:1.25-2.06)] were independent predictors of cardiovascular events during follow-up after adjusting for conventional risk factors(both p<0.001). Further adjustments for potential confounders had no influence on associations. These findings demonstrate that altered RPA-derived parameters are associated with T2DM. Furthermore, baseline values of INTPR and DRC independently predict cardiovascular events in individuals with T2DM, indicating the potential clinical utility of these parameters for risk stratification in T2DM

    Predicting radiologists' gaze with computational saliency models in mammogram reading

    Previous studies have shown that there is a strong correlation between radiologists' diagnoses and their gaze when reading medical images. The extent to which gaze is attracted by content in a visual scene can be characterised as visual saliency. There is a potential for the use of visual saliency in computer-aided diagnosis in radiology. However, little is known about what methods are effective for diagnostic images, and how these methods could be adapted to address specific applications in diagnostic imaging. In this study, we investigate 20 state-of-the-art saliency models including 10 traditional models and 10 deep learning-based models in predicting radiologists' visual attention while reading 196 mammograms. We found that deep learning-based models represent the most effective type of methods for predicting radiologists' gaze in mammogram reading; and that the performance of these saliency models can be significantly improved by transfer learning. In particular, an enhanced model can be achieved by pre-training the model on a large-scale natural image saliency dataset and then fine-tuning it on the target medical image dataset. In addition, based on a systematic selection of backbone networks and network architectures, we proposed a parallel multi-stream encoded model which outperforms the state-of-the-art approaches for predicting saliency of mammograms
