Vástagos cementados y no cementados en artroplastias totales de cadera por coxopatías mecánicas
En la controversia sobre la cementación o no del componente femoral se
presenta el análisis clínico, radiológico y de supervivencia de los vástagos de una serie
de 121 artoplastias totales de cadera (61 cementadas y 60 sin cementar). El estudio no evidencia
diferencias significativas a los 5 años de seguimiento, aunque es destacable una
mayor incidencia de dolor en el muslo entre los vástagos no cementados (35%) (p < 0,1) y
una tasa de reabsorción ósea proximal más elevada (70%) que en los cementados (23%).
Estos fenómenos son diseño dependientes. Un mayor número de cirugías de revisión por
aflojamiento aséptico entre las prótesis cementadas nos hacen preferir la fijación sin cemento
del componente femoral.We discuss the cemented or non-cement fixation of the femoral components in total hip arthroplasty on a serie of 121 femoral stem (61 cemented stem and 60 non-cemented stem). Survival analysis, clinical and radiographic data showed no significant differences between the two groups at 5 years follow-up. However the incidence of pain thigh phenomenon was higher for the non-cemented stem (35%) (p < 0.1). Stress schielding was evidently higher between the non-cemented implants (70%) too, as compared with cemented stem (23%). These findings are design-dependent. The cemented arthroplasties had a higher rate of revision procedures because of aseptic loosening (13%). In this way we prefer the non-cement fixation of the femoral implant
Componentes de la corteza del olmo y su posible influencia en la alimentación de los escolítidos
In order to better understand the chemical factors involved in the host feeding selectivity by Scolytus, components were extracted from elm bark and studied. Sampled trees were located in an elm stand that survived the Dutch twig elm disease pandemia, and from elm clones from throughout the Iberian Peninsula. The main samples studied were from Ulmus minor, and the other samples from U. pumila, U. laevis and several hybrids. The main compounds in the extracts obtained from current-year twigs were aliphatic hydrocarbons (particularly heptacosane and nonacosane) and from 2- to 4-year-old stems were triterpenes. Three of the triterpenes were identified for the first time in Ulmus (lupenol, alnulin, and ilexol) and two in Ulmaceae (moretenol and betulin). Differences occurred in the compounds isolated from U. minor from the two locations, and between species.Se analizaron los componentes de la corteza de ramillas de olmos, a fin de determinar los factores químicos involucrados en la selección, por parte de los escolítidos del olmo (Scolytus), de árboles para su alimentación. Las muestras fueron recogidas en árboles de una olmeda que ha sobrevivido a la pandemia de grafiosis y de ejemplares de distintas procedencias de la península Ibérica en un banco clonal. La principal especie estudiada fue Ulmus minor. Se tomaron además muestras de U. pumila, U. laevis y diversos híbridos. Los compuestos más abundantes en los extractos obtenidos de ramillas del año en curso fueron hidrocarburos alifáticos (principalmente heptacosano y nonacosano) y en las ramillas de 2º a 4º año triterpenos pentacíclicos. Tres de los triterpenos (lupenol, alnulina e ilexol) fueron identificados por primera vez en Ulmus y dos (moretenol y betulina) en Ulmaceae. Amaru: plug&play resilient in-band control for SDN
Software-Defined Networking (SDN) is a pillar of next-generation networks. ImplementingSDN requires the establishment of a decoupled control communication, which might be installed either as anout-of-band or in-band network. While the benefits of in-band control networks seem apparent, no standardprotocol exists and most of setups are based on ad-hoc solutions. This article defines Amaru, a protocolthat provides plug&play resilient in-band control for SDN with low-complexity and high scalability. Amarufollows an exploration mechanism to find all possible paths between the controller and any node of thenetwork, which drastically reduces convergence time and exchanged messages, while increasing robustness.Routing is based on masked MAC addresses, which also simplifies routing tables, minimizing the numberof entries to one per path, independently of the network size. We evaluated Amaru with three differentimplementations and diverse types of networks and failures, and obtained excellent results, providing almoston-the-fly rerouting and low recovery time.Comunidad de MadridUniversidad de Alcal
First universal newborn screening program for severe combined immunodeficiency in Europe: two-years' experience in Catalonia (Spain)

Early detection of this condition speeds up the establishment of appropriate treatment and increases the patient's life expectancy. Newborn screening for SCID started in January 2017 in Catalonia, the first Spanish and European region to universally include this testing. The results obtained in the first 2 years of experience are evaluated here. All babies born between January 2017 and December 2018 were screened. TREC quantification in DBS (1.5 mm diameter) was performed with the Enlite Neonatal TREC kit from PerkinElmer (Turku, Finland). In 2018, the retest cutoff in the detection algorithm was updated based on the experience gained in the first year, and changed from 34 to 24 copies/μL. This decreased the retest rate from 3.34 to 1.4% (global retest rate, 2.4%), with a requested second sample rate of 0.23% and a positive detection rate of 0.02%. Lymphocyte phenotype (T, B, NK populations), expression of CD45RA/RO isoforms, percentage and intensity of TCR αβ and TCR γδ, presence of HLA-DR+ T lymphocytes, and in vitro lymphocyte proliferation were studied in all patients by flow cytometry. Of 130,903 newborns screened, 30 tested positive, 15 of which were male. During the study period, one patient was diagnosed with SCID: incidence, 1 in 130,903 births in Catalonia. Thirteen patients had clinically significant T-cell lymphopenia (non-SCID) with an incidence of 1 in 10,069 newborns (43% of positive detections). Nine patients were considered false-positive cases because of an initially normal lymphocyte count with normalization of TRECs between 3 and 6 months of life, four infants had transient lymphopenia due to an initially low lymphocyte count with recovery in the following months, and three patients are still under study. The results obtained provide further evidence of the benefits of including this disease in newborn screening programs. Longer follow-up is needed to define the exact incidence of SCID in Catalonia
Development of a novel splice array platform and its application in the identification of alternative splice variants in lung cancer
<p>Abstract</p> <p>Background</p> <p>Microarrays strategies, which allow for the characterization of thousands of alternative splice forms in a single test, can be applied to identify differential alternative splicing events. In this study, a novel splice array approach was developed, including the design of a high-density oligonucleotide array, a labeling procedure, and an algorithm to identify splice events.</p> <p>Results</p> <p>The array consisted of exon probes and thermodynamically balanced junction probes. Suboptimal probes were tagged and considered in the final analysis. An unbiased labeling protocol was developed using random primers. The algorithm used to distinguish changes in expression from changes in splicing was calibrated using internal non-spliced control sequences. The performance of this splice array was validated with artificial constructs for <it>CDC6</it>, <it>VEGF</it>, and <it>PCBP4 </it>isoforms. The platform was then applied to the analysis of differential splice forms in lung cancer samples compared to matched normal lung tissue. Overexpression of splice isoforms was identified for genes encoding <it>CEACAM1</it>, <it>FHL-1</it>, <it>MLPH</it>, and <it>SUSD2. </it>None of these splicing isoforms had been previously associated with lung cancer.</p> <p>Conclusions</p> <p>This methodology enables the detection of alternative splicing events in complex biological samples, providing a powerful tool to identify novel diagnostic and prognostic biomarkers for cancer and other pathologies.</p
Evaluation of Natural Language Processing for the Identification of Crohn Disease-Related Variables in Spanish Electronic Health Records:A Validation Study for the PREMONITION-CD Project
Background: The exploration of clinically relevant information in the free text of electronic health records (EHRs) holds the potential to positively impact clinical practice as well as knowledge regarding Crohn disease (CD), an inflammatory bowel disease that may affect any segment of the gastrointestinal tract. The EHRead technology, a clinical natural language processing (cNLP) system, was designed to detect and extract clinical information from narratives in the clinical notes contained in EHRs. Objective: The aim of this study is to validate the performance of the EHRead technology in identifying information of patients with CD. Methods: We used the EHRead technology to explore and extract CD-related clinical information from EHRs. To validate this tool, we compared the output of the EHRead technology with a manually curated gold standard to assess the quality of our cNLP system in detecting records containing any reference to CD and its related variables. Results: The validation metrics for the main variable (CD) were a precision of 0.88, a recall of 0.98, and an F1 score of 0.93. Regarding the secondary variables, we obtained a precision of 0.91, a recall of 0.71, and an F1 score of 0.80 for CD flare, while for the variable vedolizumab (treatment), a precision, recall, and F1 score of 0.86, 0.94, and 0.90 were obtained, respectively. Conclusions: This evaluation demonstrates the ability of the EHRead technology to identify patients with CD and their related variables from the free text of EHRs. To the best of our knowledge, this study is the first to use a cNLP system for the identification of CD in EHRs written in Spanish. © 2022 JMIR Medical Informatics. All rights reserved
Clinicopathological and Genomic Identification of Breast Cancers with No Impact on Mortality
Background. Implementing mammogram screening means that clinicians are seeing many breast cancers that will never develop metastases. The purpose of this study was to identify subgroups of breast cancer patients who did not present events related to long-term breast cancer mortality, taking into account diagnosis at breast screening, absence of palpability and axillary involvement, and genomic analysis with PAM50. Patients and Methods. To identify them, a retrospective observational study was carried out selecting patients without any palpable tumor and without axillary involvement, and a genomic analysis was performed with PAM50. Results. The probability of distant metastasis-free interval (DMFI) of 337 patients was 0.92 (95% CI, 0.90–0.93) at 20 years and 0.96 (95% CI, 0.92–1.00) in 95 patients (28%) with available PAM50 tests. In 22 (23.15%) luminal A tumors and in 9 (9.47%) luminal B tumors smaller than 1 cm, and in HER2 and basal type tumors, there were no metastatic events (20-year DMFI of 1.00). Conclusion. Patients with nonpalpable breast cancer found at screening with negative nodes are at very low risk. It is possible to identify subgroups without metastatic events by determining the intrinsic subtype and tumor size less than 1 cm. Therefore, de-escalation of treatment should be considered
Prevalence and distribution of infectious and parasitic agents in roe deer from Spain and their possible role as reservoirs
In order to provide up-to-date information about the prevalence of infectious and parasitic agents in Spanish roe deer (Capreolus capreolus), samples from 93 animals hunted from January 2013 to April 2015 were collected and analysed by parasitological, serological and molecular techniques. Sampled animals came from four roe deer populations corresponding to Oceanic, Continental, Mediterranean and Mountainous ecosystems of Spain. Data regarding sex, age and year were also considered. A high percentage of roe deer (95.7%) resulted positive for at least one agent. Sarcocystis spp. was the most frequently diagnosed genus (88.8%), followed by gastrointestinal nematodes (62.9%) and Schmallenberg virus (53.5%). Varestrongylus capreoli (38%), Anaplasma phagocitophylum (34.2%), Eimeria spp. (29.2%), Toxoplasma gondii (25%) and Cephenemyia stimulator (23.8%) displayed medium prevalences and, finally low percentages were registered for Moniezia spp. (6.7%), Dictyocaulus noerneri (2.4%) and Mycobacterium avium subsp. paratuberculosis (1.5%). No infections by Neospora caninum, Bovine herpesvirus, pestivirus or Coxiella burnetii were found. Climate was significantly associated with the prevalence of T. gondii, C. stimulator and A. phagocitophylum, with higher prevalences in animals from Oceanic and Mediterranean areas. Our results suggest that infections affecting Spanish roe deer, especially those of parasitic aetiology, represent one of the causes of the descent in the abundance of this ungulate in the last years. The 2013 Balearic campaign GNSS position analysis of the 2013 will be performed with different softwares by different groups (similarly as it is being done in
the International GNSS Service for their different products), in order improve the high demanded accuracy for JASON2 and SARAL altimeters precise calibration.
In particular JPL GIPSY-OASIS software will be used, with the undifferenced PPP ambiguity fixing strategy. In order to improve the results accuracy, two similar
networks are being processed. The first network includes the deployed GNSS receivers and the reference stations. The second one is a control network, defined
by using the permanent receivers in the California dense network with a similar distribution as the main altimeter campaign network. In this case, the position of
the receivers plying the role of buoys are being processed in the same kinematic way than the actual buoys, in order to compare them with the very accurate

Identification of novel synthetic lethal vulnerability in non small cell lung cancer by co targeting TMPRSS4 and DDR1
Finding novel targets in non-small cell lung cancer (NSCLC) is highly needed and identification of synthetic lethality between two genes is a new approach to target NSCLC. We previously found that TMPRSS4 promotes NSCLC growth and constitutes a prognostic biomarker. Here, through large-scale analyses across 5 public databases we identified consistent co-expression between TMPRSS4 and DDR1. Similar to TMPRSS4, DDR1 promoter was hypomethylated in NSCLC in 3 independent cohorts and hypomethylation was an independent prognostic factor of disease-free survival. Treatment with 5-azacitidine increased DDR1 levels in cell lines, suggesting an epigenetic regulation. Cells lacking TMPRSS4 were highly sensitive to the cytotoxic effect of the DDR1 inhibitor dasatinib. TMPRSS4/DDR1 double knock-down (KD) cells, but not single KD cells suffered a G0/G1 cell cycle arrest with loss of E2F1 and cyclins A and B, increased p21 levels and a larger number of cells in apoptosis. Moreover, double KD cells were highly sensitized to cisplatin, which caused massive apoptosis (~40%). In vivo studies demonstrated tumor regression in double KD-injected mice. In conclusion, we have identified a novel vulnerability in NSCLC resulting from a synthetic lethal interaction between DDR1 and TMPRSS4
