Operative Management of Avascular Necrosis of the Femoral Head in Skeletally Immature Patients: A Systematic Review
Purpose: Osteonecrosis of the femoral head (ONFH) is common in skeletally immature patients. The management of ONFH is controversial, with limited evidence and unpredictable results. This study systematically reviewed the current operative modalities and clinical outcomes of surgical management for ONFH in skeletally immature patients. Methods: The present study was conducted according to the PRISMA 2020 guidelines. PubMed, Google Scholar, Embase, and Web of Science databases were accessed in October 2021. All the published clinical studies reporting data concerning the surgical management of ONFH in skeletally immature patients were included. Results: This review included 122 patients (127 hips). 38.2% (46 of 122) were female. The mean age of the patients was 14.2 +/- 2.3 years. The mean duration of the follow-up was 55.3 +/- 19.6 months. The Harris Hip Score improved from 68.8 +/- 11.9 at baseline to 90.5 +/- 6.5 at last follow-up (p < 0.0001). Femoral head collapse and secondary hip degeneration were the most common complications. Conclusion: Several surgical techniques are available and effective for the management of ONFH in skeletally immature patients. This study evidenced high heterogeneity of the surgical procedures and eligibility criteria. Further high-quality investigations are required to establish proper indications and surgical modalities
The Objective Buttocks Assessment Scale (OBAS): a new and complete method to assess the gluteal region
Introduction: New treatment methods to improve and enhance buttocks appearance require globally accepted scales for aesthetic research and patient evaluation. The purpose of our study was to develop a set of grading scales for objective assessment of the gluteal region and assess their reliability and validity.
Materials and methods: Twelve photonumeric grading scales were created. Eleven aesthetic experts rated photographs of 650 women in 2 validation sessions. Responses were analyzed to assess inter-rater and intra-rater reliability. The Rasch model was used as part of the validation process.
Results: All the scales exceeded criteria for acceptability, reliability and validity. Overall inter-rater reliability and intra-rater reliability were both “almost perfect” (p=0.15 and p=0.16 respectively).
Conclusion: Consistent outcomes between raters and by individual raters at 2 time points confirm the reliability of the Objective Buttocks Assessment Scale in female patients and suggest it will be a valuable tool for use in research and clinical practice
Necrotizing Fasciitis of the Upper Limb: Optimizing Management to Reduce Complications
Background: Necrotizing fasciitis (NF) is a severe, potentially life-threatening condition.
The aim of this study is to identify strategies aimed at reducing complications in patients with
NF of the upper limb. Methods: We conducted a retrospective study on patients admitted to our
Unit for suspected NF of the upper limb. The analyzed data included patient characteristics, delay
before primary care, clinical and biological signs upon arrival, pathogens involved, and the rate of
amputations and mortality. Results: A total of 21 patients presented with confirmed necrotizing
bacterial dermohypodermitis-NBDH with NF (NBDH-NF) affecting the upper limb. The mean delay
between the onset of symptoms and the clinical examination in the Emergency Dermatology Unit was
48 h (range: 6 to 72 h). The mean delay between admission and primary surgery was 150 min (range:
60 min to 280 min). No amputations were performed. All patients were alive one year after the first
surgical procedure. Conclusions: Our study demonstrated that it is possible to reduce mortality and
morbidity rates in NF of the upper limb. Timely diagnosis and early treatment and a multidisciplinary
medico-surgical dedicated team providing care can significantly modify the outcomes. Early surgical
debridement is the most important factor affecting the prognosis of these infections
RGDS peptide induces caspase 8 and caspase 9 activation in human endothelial cells
AbstractPeptides containing the Arg-Gly-Asp (RGD) motif inhibit cell adhesion and exhibit a variety of other biologic effects including anticoagulant and antimetastatic activities. The aim of the present study was to examine the anchorage-independent effects of an RGD-containing peptide, Arg-Gly-Asp-Ser (RGDS), on human umbilical vein endothelial cells (HUVECs). Assays were performed on HUVECs seeded onto collagen IV; under these experimental conditions RGDS did not exert antiadhesive effects but significantly reduced FGF-2-dependent chemotaxis after 4 hours of treatment and reduced proliferation after 24 hours of treatment. Experiments carried out with caspase-specific inhibitors indicated that the observed antichemotactic effects required caspase 8 and caspase 9 activation. RGDS activated both caspase 8 and caspase 9 after 4 hours of treatment and caspase 3 after 24 hours of treatment, and markedly enhanced HUVEC apoptosis by transferase-mediated deoxyuridine triphosphate nick-end labeling (TUNEL)/Hoechst staining and fluorescence-activated cell sorting (FACS) analysis. Finally, confocal microscopy showed that RGDS localizes in the cytoplasm of live HUVECs within 4 hours and in vitro experiments showed that RGDS directly interacts with recombinant caspases 8 and 9 in a specific way. In summary, these results indicate that RGDS directly binds and activates caspases 8 and 9, inhibits chemotaxis, and induces apoptosis of HUVECs with a mechanism independent from its antiadhesive effect
Microvascular free-flap transfer for head and neck reconstruction in elderly patients
BACKGROUND: With the increase in life expectancy, the incidence of head and neck cancer has grown in the elderly population. Free tissue transfer has become the first choice, among all the reconstructive techniques, in these cases. The safety and success of micro vascular transfer have been well documented in the general population, but its positive results achieved in elderly patients have received less attention. METHODS: We retrospectively studied 28 patients over the age of 60 years. The aim of this paper was to study the success rate of free tissue transfer and investigate the complication incidence in this patient population. RESULTS: Twenty-eight free flaps were performed to reconstruct medium to large cervico-facial surgical defects in six years. No difference was noted between success and complication rates observed between general and elderly population. CONCLUSION: This study indicates that free-flap technique for head and neck reconstruction could be considered a safe option in elderly patients when a good pre-operative general status is present
The effects of primary care monitoring strategies on COVID-19 related hospitalisation and mortality: a retrospective electronic medical records review in a northern Italian province, the MAGMA study
Background: Most symptomatic SARS-CoV-2 infections produce mild to moderate symptoms. Although most patients are managed in the outpatient setting, little is known about the effect of general practitioners' (GP) management strategies on the outcomes of COVID-19 outpatients in Italy. Objectives: Describe the management of Italian GPs of SARS-CoV-2 infected adult patients and explore whether GP active care and monitoring are associated with reducing hospitalisation and death. Methods: Retrospective observational study of SARS-CoV-2 infected adult outpatients managed by GPs in Modena (Italy) from March 2020 to April 2021. Information on management and monitoring strategies, patients' socio-demographic characteristics, comorbidities, and outcomes (hospitalisation and death due to COVID-19) were retrieved through an electronic medical record review and analysed descriptively and through multiple logistic regression. Results: Out of the 5340 patients from 46 GPs included in the study, 3014 (56%) received remote monitoring, and 840 (16%) had at least one home visit. More than 85% of severe or critical patients were actively monitored (73% daily) and 52% were visited at home. Changes over time in patients' therapeutic management were observed in concordance with the guidelines' release. Active daily remote monitoring and home visits were strongly associated with reduced hospitalisation rate (OR 0.52, 95% CI 0.33-0.80 and OR 0.50, 95% CI 0.33-0.78 respectively). Conclusion: GPs effectively managed an increasing number of outpatients during the first waves of the pandemic. Active monitoring and home visits were associated with reduced hospitalisation in COVID-19 outpatients
Argonaute 2 as novel molecular determinant for myeloid differentiation
microRNAs (miRNAs) are emerging as crucial factors for the establishment of complex regulatory circuitries involved in the regulation of hematopoietic cell fate determination. These small non-coding RNAs to exert their functional activity are assembled in RNA-induced silencing complexes (RISCs), where a member of Argonaute (Ago) family of proteins plays a central role in miRNA-mRNA target interaction and gene silencing. In human cells the miRNAs-Ago complex can also localize in the nucleus where Ago proteins can associate with promoter gene sequences to impact heterochromatin genomic structure and transcriptional silencing (Janowski BA et al., 2006; Meister G., 2013). By using human myeloid cell lines and acute myeloid leukemia (AML) primary blasts we highlight Ago2 as a new player in myeloid cell fate determination. We observed that: i) Ago2 protein levels are strongly increased during 1,25-dihydroxyvitamin D3 (D3)-induced monocyte differentiation, whereas are down-regulated during Retinoic Acid (RA)-induced granulocyte differentiation; ii) Ago2 depletion by shRNA or small chemical compounds disrupting both miRNA-Ago2 complex interaction and Ago2 chromatin localization, results in a strong improvement of the RA-dependent myeloid differentiation. These results are bringing out that the down-regulation of Ago2 expression/functional activity is required during RA-dependent myeloid differentiation and may represent a molecular determinant for the improvement of RA-treatment response in leukemic myeloid progenitors cells
Tissue Induction in Plastic and Maxillo-facial Surgery
Tissue induction is defined as the activation of cell regeneration to restore damaged tissue, which involves stimulating cell signaling and modifying the microenvironment. Tissue inducers therefore have the advantage of acting quickly and durably on treated tissues, alone or in combination with surgical procedures, in order to reduce iatrogeny and potentiate surgical results. The aim of this review was to detail the various current techniques for tissue regeneration in the field of plastic and maxillo-facial surgery. We conducted a systematic search on Pubmed, Google Scholar and Science Direct. Articles in English and French, published after 2012 and focusing on facial tissue induction were searched. Only prospective comparative studies assessing as many cases as possible were analyzed. The following keywords were used: “skin rejuvenation”, “skin regeneration”, “collagen induction”, “skin enhancer”, “aging rejuvenation”, “oral mucosa rejuvenation”, “oral mucosa regeneration”, “buccal mucosa rejuvenation”, “buccal mucosa regeneration”, “oral bone regeneration”, “alveolar bone regeneration”. Fifty innovative articles published since 2012 dealing with tissue induction techniques with an interest in plastic and maxillo-facial surgery were identified and then selected. The most effective tissue inducers for skin and mucosal regeneration were lasers, radiofrequency, pulsed light, hyaluronic acid and PRP. Tissue induction allows collagen self-production leading to tissue regeneration. Many techniques can be used for tissue induction that represent an additional tool in the therapeutic arsenal available to plastic and maxillofacial surgeons to improve patient management.These inducers can be used alone or in combination to achieve synergistic effects and better clinical outcomes
Antibacterial activity of Arthrobacter strains isolated from Great Gobi A Strictly Protected Area, Mongolia
Desert soil hosts many microorganisms, whose activities are essential from an ecological viewpoint. Moreover, they are of great anthropic interest. The knowledge of extreme environments microbiomes may be beneficial for agriculture, technology, and human health. In this study, 11 Arthrobacter strains from topsoil samples collected from the Great Gobi A Strictly Protected Area in the Gobi Desert, were characterized by a combination of different techniques. The phylogenetic analysis, performed using their 16S rDNA sequences and the most similar Arthrobacter sequences found in databases, revealed that most of them were close to A. crystallopoietes, while others joined a sister group to the clade formed by A. humicola, A. pascens, and A. oryzae. The resistance of each strain to different antibiotics, heavy -metals, and NaCl was also tested as well as the inhibitory potential against human pathogens (i.e., Burkholderia ssp., Klebsiella pneumoniae, Pseudomonas aeruginosa, and Staphylococcus ssp.) via cross -streaking, to check the production of metabolites with antimicrobial activity. Data obtained revealed that all strains were resistant to heavy metals and were able to strongly interfere with the growth of many of the human pathogens tested. The volatile organic compounds (VOCs) profile of the 11 Arthrobacter strains was also analyzed. A total of 16 different metabolites were found, some of which were already known for having an inhibitory action against different Grampositive and Gram-negative bacteria. Isolate MS -3A13, producing the highest quantity of VOCs, is the most efficient against Burkholderia cepacia complex (Bcc), K. pneumoniae, and coagulase-negative Staphylococci (CoNS) strains. This work highlights the importance of understanding microbial populations' phenotypical characteristics and dynamics in extreme environments to uncover the antimicrobial potential of new species and strains
