Radioiodine plus recombinant human thyrotropin do not cause acute airway compression and are effective in reducing multinodular goiter
Recombinant human thyrotropin (rhTSH) reduces the activity of radioiodine required to treat multinodular goiter (MNG), but acute airway compression can be a life-threatening complication. In this prospective, randomized, double-blind, placebo-controlled study, we assessed the efficacy and safety (including airway compression) of different doses of rhTSH associated with a fixed activity of 131I for treating MNG. Euthyroid patients with MNG (69.3 ± 62.0 mL, 20 females, 2 males, 64 ± 7 years) received 0.1 mg (group I, N = 8) or 0.01 mg (group II, N = 6) rhTSH or placebo (group III, N = 8), 24 h before 1.11 GBq 131I. Radioactive iodine uptake was determined at baseline and 24 h after rhTSH and thyroid volume (TV, baseline and 6 and 12 months after treatment) and tracheal cross-sectional area (TCA, baseline and 2, 7, 180, and 360 days after rhTSH) were determined by magnetic resonance; antithyroid antibodies and thyroid hormones were determined at frequent intervals. After 6 months, TV decreased significantly in groups I (28.5 ± 17.6%) and II (21.6 ± 17.8%), but not in group III (2.7 ± 15.3%). After 12 months, TV decreased significantly in groups I (36.7 ± 18.1%) and II (37.4 ± 27.1%), but not in group III (19.0 ± 24.3%). No significant changes in TCA were observed. T3 and free T4 increased transiently during the first month. After 12 months, 7 patients were hypothyroid (N = 3 in group I and N = 2 in groups II and III). rhTSH plus a 1.11-GBq fixed 131I activity did not cause acute or chronic changes in TCA. After 6 and 12 months, TV reduction was more pronounced among patients treated with rhTSH plus 131I
Reliability of maximal respiratory nasal pressure tests in healthy young adults
[Abstract] Introduction: Sniff nasal inspiratory (SNIP) and expiratory pressure (SNEP) may complement the assessment of respiratory muscle strength. Thus, specifying their reliability is relevant to improving the clinical consistency of both tests.
Objective: To assess the reliability of SNIP and SNEP in healthy young adults.
Methods: This cross-sectional study included self-reported healthy aged 18 to 29 years. SNIP was performed using a plug to occlude one nostril, while SNEP was conducted using a facemask. Participants performed 20 SNIP and SNEP maneuvers with 30-second intervals in between. The intraclass correlation coefficient (ICC), standard error of measurement (SEM), and minimum detectable change (MDC) assessed the reliability of SNIP and SNEP. Analyses were conducted between the highest peak pressure and the first reproducible maneuver in men and women.
Results: The total sample comprised 32 participants: 16 men and 16 women. The ICC, SEM, and MDC for SNIP maneuvers were 0.994 (95%CI 0.988 to 0.997), 1.820 cmH2O, and 5.043 cmH2O, respectively. For SNEP, these parameters were 0.950 (95%CI 0.897 to 0.976), 6.03 cmH2O, and 16.716 cmH2O. The SNIP and SNEP in men showed ICC of 0.992 (95%CI 0.977 to 0.997) and 0.877 (95%CI 0.648 to 0.957), SEM of 2.07 and 7.66 cmH2O, and MDC of 5.74 and 21.23 cmH2O. In women, SNIP and SNEP presented ICC of 0.992 (95%CI 0.977 to 0.997) and 0.957 (95%CI 0.878 to 0.985), SEM of 1.15 and 6.11 cmH2O, and MDC of 3.19 and 16.95 cmH2O. Also, 60% of the highest SNIPs occurred among the 11th and 20th maneuvers in men and women. In men, 55% of the highest SNEPs occurred among the 11th and 20th maneuvers; this value was 50% in women.
Conclusion: SNIP and SNEP showed excellent reliability. The reliability of SNIP and SNEP in men was good and excellent, respectively, whereas both tests had excellent reliability in women. Structural optimisation of random discontinuous fibre composites: Part 2 - case study
This is the second paper in a two part series presenting the development of a stiffness optimisation algorithm to intelligently optimise the fibre architecture of discontinuous fibre composites. A Multi-Criteria Decision Making (MCDM) strategy is used to select parameters associated with the fibre architecture, to produce components that satisfy stiffness, cost and mass criteria. The model has been successfully demonstrated using an automotive spare wheel well geometry, which shows that a highly optimised discontinuous fibre composite solution can compete against a continuous fabric counterpart in terms of specific stiffness, whilst presenting an opportunity for significant cost reduction. This could potentially lead to the application of composite materials into new areas where cost has previously been prohibitive
Environment-Induced Decoherence and the Transition From Quantum to Classical
We study dynamics of quantum open systems, paying special attention to those
aspects of their evolution which are relevant to the transition from quantum to
classical. We begin with a discussion of the conditional dynamics of simple
systems. The resulting models are straightforward but suffice to illustrate
basic physical ideas behind quantum measurements and decoherence. To discuss
decoherence and environment-induced superselection einselection in a more
general setting, we sketch perturbative as well as exact derivations of several
master equations valid for various systems. Using these equations we study
einselection employing the general strategy of the predictability sieve.
Assumptions that are usually made in the discussion of decoherence are
critically reexamined along with the ``standard lore'' to which they lead.
Restoration of quantum-classical correspondence in systems that are classically
chaotic is discussed. The dynamical second law -it is shown- can be traced to
the same phenomena that allow for the restoration of the correspondence
principle in decohering chaotic systems (where it is otherwise lost on a very
short time-scale). Quantum error correction is discussed as an example of an
anti-decoherence strategy. Implications of decoherence and einselection for the

A survey of solution techniques for the partially observed Markov decision process
We survey several computational procedures for the partially observed Markov decision process (POMDP) that have been developed since the Monahan survey was published in 1982. The POMDP generalizes the standard, completely observed Markov decision process by permitting the possibility that state observations may be noise-corrupted and/or costly. Several computational procedures presented are convergence accelerating variants of, or approximations to, the Smallwood-Sondik algorithm. Fitting the integrated Spectral Energy Distributions of Galaxies
Fitting the spectral energy distributions (SEDs) of galaxies is an almost
universally used technique that has matured significantly in the last decade.
Model predictions and fitting procedures have improved significantly over this
time, attempting to keep up with the vastly increased volume and quality of
available data. We review here the field of SED fitting, describing the
modelling of ultraviolet to infrared galaxy SEDs, the creation of
multiwavelength data sets, and the methods used to fit model SEDs to observed
galaxy data sets. We touch upon the achievements and challenges in the major
ingredients of SED fitting, with a special emphasis on describing the interplay
between the quality of the available data, the quality of the available models,
and the best fitting technique to use in order to obtain a realistic
measurement as well as realistic uncertainties. We conclude that SED fitting
can be used effectively to derive a range of physical properties of galaxies,
such as redshift, stellar masses, star formation rates, dust masses, and
metallicities, with care taken not to over-interpret the available data. Yet
there still exist many issues such as estimating the age of the oldest stars in
a galaxy, finer details ofdust properties and dust-star geometry, and the
influences of poorly understood, luminous stellar types and phases. The
challenge for the coming years will be to improve both the models and the
observational data sets to resolve these uncertainties. The present review will
be made available on an interactive, moderated web page (sedfitting.org), where
the community can access and change the text. The intention is to expand the

Resíduos madeireiros do alburno de pau-rainha (Brosimum rubescens): investigação de metabólitos secundários e alguns aspectos tecnológicos
Estudos fitoquímicos prévios com resíduos do cerne de pau-rainha (Brosimum rubescens), identificaram um alto teor de xantiletina, uma cumarina com potencial biológico. Dando prosseguimento aos estudos com serragens desta espécie, este estudo relata a densidade básica bem como o isolamento e identificação do triterpeno 3β-acetoxi-olean-12-eno-28-al e do β- sitosterol nos extratos hexânico e metanólico do alburno da planta. A estrutura do triterpeno foi determinada com base nos espectros de RMN em 1D (¹H e 13C) e 2D (HSQC e HMBC) além de comparação com dados da literatura. A densidade básica encontrada para o alburno foi de 0,58 g cm-3 que, embora seja inferior a do cerne, poderá ser utilizada na confecção de vários produtos, inclusive em técnicas de marchetaria.Previous phytochemical studies on residues of pau-rainha’s heartwood (Brosimum rubescens) showed a high content of xanthyletin, a coumarin with biological potential. Continuing our studies with sawdust of this species, this work relates the basic density, isolation and identification of the triterpene 3β-acetoxy-olean-12-ene-28-al and β-sitosterol in n-hexane and MeOH extracts of the plant´s sapwood. The structure of the triterpene was determined on the basis of NMR spectra in 1D (¹H and 13C) and 2D (HSQC and HMBC) and comparison with literature data. The basic density found for sapwood was 0.58 g cm-3, that even so is inferior the one of heartwood, could be used in the confection of some products, also in marquetry techniques
