Eating well, living well and weight management: A co-produced semi-qualitative study of barriers and facilitators experienced by adults with intellectual disabilities
Adults with intellectual disabilities in England experience health inequalities. They are more likely than their non-disabled peers to be obese and at risk of serious medical conditions such as heart disease, stroke and type 2 diabetes. This semi-qualitative study engaged adults with intellectual disabilities in a co-production process to explore their perceived barriers and facilitators to eating well, living well and weight management. Nineteen participants with intellectual disabilities took part in four focus groups and one wider group discussion. They were supported by eight of their carers or support workers. Several barriers were identified including personal income restrictions, carers’ and support workers’ unmet training needs, a lack of accessible information, inaccessible services and societal barriers such as the widespread advertising of less healthy foodstuffs. A key theme of frustration with barriers emerged from analysis of participants’ responses. Practical solutions suggested by participants included provision of clear and accessible healthy lifestyle information, reasonable adjustments to services, training, ‘buddying’ support systems or schemes and collaborative working to improve policy and practice
Populists in power and conspiracy theories
The populist use of tropes such as conspiracy theories play an increasingly important role in their politics. Populism and conspiracy theories present a number of common traits – Manicheanism, a sense of victimhood, and an ambivalence towards representative politics – and populists’ use of conspiracy theories is politically purposeful. Targeting a conspiring elite serves to vilify real or fictional opponents and/or shield populists from hostile attacks. Looking at three cases of populists in government – Orbán in Hungary, Trump in the United States, and Chávez in Venezuela – we examine the definition of conspiring elites (who), the circumstances under which conspiracy theories are propagated (when), and the ultimate purpose of conspiratorial framing (why). We demonstrate how populists in power use conspiracy theories to demonise and delegitimise their opponents, to promote or prolong a sense of crisis, and to rally support while distracting from possible failure
European populism before the pandemic: ideology, Euroscepticism, electoral performance, and government participation of 63 parties in 30 countries
This contribution is conceived as a resource on the state of European populist parties before the outbreak of the COVID-19 pandemic. It reports on cross-national comparative findings generated by data collected from 30 European countries as to the state of populist parties in one calendar year (2019) and provides an extensive qualitative overview of the national cases. The article shows that while populist parties are preponderantly on the right, there is a significant degree of ideological variation among European populism. The data show significant diversity in their electoral performance but also that populist party participation in government is no longer a marginal phenomenon. The article ultimately elaborates on the various types of positions on European integration – from soft/hard Euroscepticism to lack thereof – and discusses the implications of their affiliation in the European Parliament
A systematic review of the methodological and practical challenges of undertaking randomised-controlled trials with cognitive disability populations
Approximately 10% of the world's population have a cognitive disability. Cognitive disabilities can have a profound impact on a person's social, cognitive or mental functioning, requiring high levels of costly health and social support. Therefore, it is imperative that interventions and services received are based upon a sound evidence-base. For many interventions for this population, this evidence-base does not yet exist and there is a need for more Randomised Controlled Trials (RCTs). The process of conducting RCTs with disabled populations is fraught with methodological challenges. We need a better understanding of these methodological barriers if the evidence-bases are to be developed. The purpose of this study was to explore the methodological and practical barriers to conducting trials with adults with cognitive disabilities. As a case example, the literature regarding RCTs for people with intellectual disabilities (ID) was used to highlight these pertinent issues. A systematic literature review was conducted of RCTs with adults with ID, published from 2000 to 2017. A total of 53 papers met the inclusion criteria and were reviewed. Some of the barriers reported were specific to the RCT methodology and others specific to people with disabilities. Notable barriers included; difficulties recruiting; obtaining consent; resistance to the use of control groups; engaging with carers, staff and stakeholders; the need to adapt interventions and resources to be disability-accessible; and staff turnover. Conducting RCTs with people with cognitive disabilities can be challenging, however with reasonable adjustments, many of these barriers can be overcome. Researchers are not maximising the sharing of their experience-base. As a result, the development of evidence-bases remains slow and the health inequities of people with disabilities will continue to grow. The importance of the MRC guidelines on process evaluations, together with implications for the dissemination of ‘evidence-base’ and ‘experience-base’ are discussed
Stakeholder views on the barriers and facilitators of psychosocial interventions to address reduction in aggressive challenging behaviour in adults with intellectual disabilities
Success of psychosocial interventions in reducing aggressive challenging behaviour is likely to be related not only to mechanistic aspects but also to therapeutic and system factors. The study aims to examine the facilitators and barriers that influence whether psychosocial interventions for aggressive challenging behaviour in adults with intellectual disabilities lead to positive change. We conducted 42 semi-structured interviews with adults with intellectual disabilities who display aggressive challenging behaviour, family/paid carers, and professionals engaged in or delivering a psychosocial intervention across the UK. Data were analysed thematically using a framework approach. Stakeholders considered therapeutic and supportive relationships and personalised care as facilitating factors in addressing aggressive challenging behaviour. The operational structure of community intellectual disability services and conflicting expectations of professionals and carers were the main contextual barriers that impeded the implementation of psychosocial interventions in adults with intellectual disabilities. Findings highlight the valued components that maximise positive change in adults with intellectual disabilities who display aggressive challenging behaviour. Several operational adjustments including referral criteria, roles of professionals and workforce issues need to be addressed in services to maximise the implementation of psychosocial interventions leading to reduction in aggressive challenging behaviour in this population

Ethnopolitics Across Central and Eastern Europe in a State of Flux: Time for Updating and Upgrading?
This chapter illustrates why and how the existing theoretical models in the study of ethnopolitics need to be updated in light of the latest developments and the increasing impact of new catalysts. These are, namely, anti-immigration and the rise of the populist and radical right across Central and Eastern Europe. This chapter hints that the more systematic cooperation between academic experts in nationalism and academic experts in the populist and radical right will enable: (a) the former to assess more accurately the degree to which new variables such as Euroscepticism and anti-immigrant trends can reshape ethnopolitics, both as a living reality and a field of study, across Central and Eastern Europe; (b) the latter to formulate new interpretative models about how (right-wing) populist and Eurosceptic actors embed their agendas inside the pre-existing political cultures of nationalism and particularistic identity and memory politics. This chapter introduces and outlines the ethnosymbolic approach as well as the triadic and quadratic configurations of ethnopolitics. Then, it proceeds into a more empirical assessment of the applicability of these theoretical approaches in a series of case studies during the 1990s, as well as the more recent emergence of new catalysts and the ensuing necessity to update and upgrade the existing theoretical models
