    Effects of alcohol consumption on mortality in patients with Type 2 diabetes mellitus

    Aims/hypothesis: Moderate alcohol intake has been associated with increased life expectancy due to reduced mortality from cardiovascular disease. We prospectively examined the effects of alcohol consumption on mortality in Type 2 diabetic patients in Switzerland. Methods: A total of 287 patients with Type 2 diabetes mellitus (125 women, 162 men), recruited in Switzerland for the WHO Multinational Study of Vascular Disease in Diabetes, were included in this study. After a follow-up period of 12.6±0.6 years (means ± SD), mortality from CHD and from all causes was assessed. Results: During the follow-up, 70 deaths occurred (21 from CHD, 49 from other causes). Compared with non-drinkers, alcohol consumers who drank alcohol 1 to 15g, 16 to 30g and 30g or more per day had the following risk rates of death from CHD: 0.87 (95% CI: 0.25 to 2.51, NS), 0.00 (95% CI: 0.00 to 0.92, p less than 0.05) and 0.37 (95% CI, 0.01 to 2.42, NS), respectively. The corresponding risk rates of death from all causes were 1.27 (95% CI: 0.68 to 2.28, NS), 0.36 (95% CI: 0.09 to 0.99, p less than 0.05) and 1.66 (95% CI: 0.76 to 3.33, NS). Conclusions/interpretation: In Swiss Type 2 diabetic patients moderate alcohol consumption of 16 to 30g per day was associated with reduced mortality from CHD and from all causes.     Aims/hypothesis: We evaluated the association of QT interval corrected for heart rate (QTc) and resting heart rate (rHR) with mortality (all-causes, cardiovascular, cardiac, and ischaemic heart disease) in subjects with type 1 and type 2 diabetes. Methods: We followed 523 diabetic patients (221 with type 1 diabetes, 302 with type 2 diabetes) who were recruited between 1974 and 1977 in Switzerland for the WHO Multinational Study of Vascular Disease in Diabetes. Duration of follow-up was 22.6 ± 0.6years. Causes of death were obtained from death certificates, hospital records, post-mortem reports, and additional information given by treating physicians. Results: In subjects with type 1 diabetes QTc, but not rHR, was associated with an increased risk of: (1) all-cause mortality (hazard ratio [HR] 1.10 per 10ms increase in QTc, 95% CI 1.02-1.20, p = 0.011); (2) mortality due to cardiovascular (HR 1.15, 1.02-1.31, p = 0.024); and (3) mortality due to cardiac disease (HR 1.19, 1.03-1.36, p = 0.016). Findings for subjects with type 2 diabetes were different: rHR, but not QTc was associated with mortality due to: (1) all causes (HR 1.31 per 10 beats per min, 95% CI 1.15-1.50, p < 0.001); (2) cardiovascular disease (HR 1.43, 1.18-1.73, p < 0.001); (3) cardiac disease (HR 1.45, 1.19-1.76, p < 0.001); and (4) ischaemic heart disease (HR 1.52, 1.21-1.90, p < 0.001). Effect modification of QTc by type 1 and rHR by type 2 diabetes was statistically significant (p < 0.05 for all terms of interaction). Conclusions/interpretation: QTc is associated with long-term mortality in subjects with type 1 diabetes, whereas rHR is related to increased mortality risk in subjects with type 2 diabete

    Psychosocial adaptation of adolescent migrants in a Swiss community survey

    OBJECTIVE: The aim of this study was to compare psychosocial adaptation in adolescent (first generation) migrants, double-citizens (mainly second generation with one migrant parent), and native Swiss, and to compare migrants from various European regions. METHOD: Data from a community survey were based on 1,239 participants (mean age 13.8, SD = 1.6 years) with 996 natives, 55 double-citizens, and 188 migrants. The adolescents completed the youth self-report measuring emotional and behavioural problems, and various questionnaires addressing life events, personality variables, perceived parental behaviour (PPB), family functioning, school environment, and social network. RESULTS: Adolescent migrants had significantly higher scores for internalizing and externalizing problems. There was a pattern of various unfavourable psychosocial features including life events, coping, self-related cognitions, and PPB that was more common among adolescent migrants than natives. Double-citizens were similar to natives in all domains. Young adolescents from South and South-East Europe differed from natives in terms of more unfavourable psychosocial features. Migrant status was best predicted by adverse psychosocial features rather than emotional and behavioural problems. CONCLUSION: There is some indication that certain migrant adolescents are at risk of psychosocial mal-adaptation. Obviously, ethnic origin is an important moderator

    Unmanned aerial vehicle-based mapping of turf-banked solifluction lobe movement and its relation to material, geomorphometric, thermal and vegetation properties

    Solifluction is one of the most widespread periglacial processes with low annual movement rates in the range of —millimeters to centimeters. Traditional methods to assess solifluction movement usually have low spatial resolution, which hampers our understanding of spatial movement patterns and the factors controlling them. In this study, we (a) test the applicability of unmanned aerial vehicle (UAV)-based structure-from-motion photogrammetry in comparison to a traditional total station survey to map surface movement of a turf-banked solifluction lobe (TBL) in the Turtmann Valley (Switzerland). We then (b) relate the detected movement patterns to potential geomorphometric, material, thermal and vegetation controls, which we assessed using geomorphic and vegetation mapping, electrical resistivity surveys and temperature loggers. Our results show that (a) UAV-based mapping can detect solifluction movement with high spatial resolution (one point per m2, total > 900 points) and rates and patterns consistent with a total station survey, but requires careful measurement set-up and analysis; and (b) movement rates differ between lobe tread, riser and a ridge feature. Differences can be explained by heterogeneous material, geomorphometric, thermal and vegetation properties of the TBL, which promote different solifluction processes. Our study demonstrates the applicability of UAV-based mapping in solifluction research and improves our understanding of solifluction processes and landform development

    Decoding the complex internal chemical structure of garnet porphyroblasts from the Zermatt area, Western Alps

    Garnet is a prototypical mineral in metamorphic rocks because it commonly preserves chemical and textural features that can be used for untangling its metamorphic development. Large garnet porphyroblasts may show extremely complex internal structures as a result of a polycyclic growth history, deformation, and modification of growth structures by intra- and intercrystalline diffusion. The complex internal structure of garnet porphyroblasts from garnet–phengite schists (GPS) of the Zermatt area (Western Alps) has been successfully decoded. The centimetre-sized garnet porphyroblasts are composed of granulite facies garnet fragments overgrown by a younger generation of grossular-rich eclogite facies garnet. The early granulite facies garnet (G-Grt) formed from low-P, high-T metamorphism during a pre-Alpine orogenic event. The late garnet (E-Grt) is typical of high-pressure, low-temperature (HPLT) metamorphism and can be related to Alpine subduction of the schists. Thus, the garnet of the GPS are polycyclic (polymetamorphic). G-Grt formation occurred at ~670 MPa and 780°C, E-Grt formed at ~1.7 GPa and 530°C. The G-Grt is relatively rich in Prp and poor in Grs, while E-Grt is rich in Grs and poor in Prp. The Alm content (mol.%) of G-Grt is 68 of E-Grt 55. After formation of E-Grt between and around fragmented G-Grt at 530°C, the GPS have been further subducted and reached a maximum temperature of 580°C before exhumation started. Garnet composition profiles indicate that the initially very sharp contacts between the granulite facies fragments of G-Grt and fracture seals of HPLT garnet (E-Grt) have been modified by cation diffusion. The profiles suggest that Ca did not exchange at the scale of 1 µm, whereas Fe and Mg did efficiently diffuse at the derived maximum temperature of 580°C for the GPS at the scale of 7–8 µm. The Grt–Grt diffusion profiles resulted from spending c. 10 Ma at 530–580°C along the P–T–t path. The measured Grt composition profiles are consistent with diffusivities of log DMgFe = −25.8 m2/s from modelled diffusion profiles. Mg loss by diffusion from G-Grt is compensated by Fe gain by diffusion from E-Grt to maintain charge balance. This leads to a distinctive Fe concentration profile typical of uphill diffusion
