A New Model For In-Service Education In A Public Hospital: Web-Based Distance Education

The unprecedented developments in information and communication technologies, together with other developments, such as increased prevalence of the internet, limited time of individuals allocated to face-to-face health trainings, and the enhanced availability of accessing web-based information have increased the indispensability of web-based educations. Web-based educations, compared to traditional educations, are more flexible and interactive for adult training, and therefore become preferable alternatives for education. In this study, within the scope of life-long learning, it is aimed to evaluate the effectiveness of web-based in-service education given to the nurses. The sample of this study was based on the nurses working in a research and education hospital in Sakarya. After obtaining the necessary permissions, between June 2013 and December 2013, it was planned to give the subjects of "Clinical Microbiology, Hospital Infections, Hand Hygiene, BloodBorne Infection, and Hospital Cleaning and Waste Management", under the main title of "Infection Protection", which was involved in the in-house education program. The participants in the study were given standard username and password by the program managers. Username was set to the first five digits of national identity number. In order to solve user problems and respond to interface page, in which the technical problems are reported, researchers were given to one-day education in distance education center (DEC). The participants were asked to answer four questions both before the each education subject and after the each education subject, and participants got score of 50 or above were evaluated as successful. It was seen that the number of correct answers given by majority of the nurses was increased after the education, and this difference was found to be statistically significant (p<0.05) (Table 1). Distance education, which dates back to the first quarter of 20 th century, has created competition in traditional education through combining different education materials in different settings. Today, health education is also transferred to web-environment through the increasing developments in information technologies. In this direction, it is possible to suggest that in-service education might also be transferred to web-environment due to the potential benefits offered by information technologies

Smoking-Cessation Success Prediction Scale: Development, Validity, and Reliability Study

This study has been conducted with the aim of testing the validity and reliability of the Smoking Cessation Success Prediction Scale (SCSPS). The study was carried out between March 2017 to March 2018 with individuals who had applied to the smoking and quitting polyclinic of an education and research hospital after approval from the institution and the ethics committee. The sample of the study is comprised of 148 adult individuals. The test-retest method was conducted with 34 people. The data have been collected using a questionnaire prepared by the researchers and the SCSPS, which has a valid scope. The validity and reliability of the scale have been assessed using the programs IBM SPSS 23 and AMOS 23. In the study, the validity-rate test result for the scope is 0.77, and Bartlett's sphericity test is significant (p < .001). Explanatory factor and confirmatory factor analyses have been performed to determine the structural validity of the SCSPS. The result of the exploratory factor analysis concludes the scale to have 10 items and two factors. The scale is shown to be reliable due to the results of Cronbach's alpha coefficient and the test-retest. SCSPS which developed was determined as a valid and reliable measurement tool

Morpho-physiological combining ability among tropical and temperate maize germplasm for drought tolerance

In this study, seven maize inbred lines representing different tropical, drought tolerant populations and two adapted temperate maize inbred lines were crossed in a half-diallel mating design to determine combining abilities. The genotypes were tested in well-watered (WW) and managed water stressed (WS) conditions in 2013 and 2014. General combining ability (GCA) and specific combining ability (SCA) mean squares were significant for all investigated traits and demonstrated both additive and non-additive genetic effects in both conditions. Higher desired leaf rolling, leaf senescence, stomatal conductance and leaf chlorophyll content GCA effects of tropical inbreds under WS conditions showed the presence of the valuable allels related to drought stress. SCA analysis revealed that the best hybrids for water use efficiency and irrigation water use efficiency were tropical x temperate crosses. Stress tolerance index and drought resistance index identified G5 x G9, a tropical x temperate hybrid, as the most tolerant hybrid to drought. Our study suggests that tropical drought tolerant germplasm has the potential to contribute useful genetic diversity to temperate maize breeding programs

Triple-negative breast cancer: immunohistochemical correlation with basaloid markers and prognostic value of survivin

Objective Among the subgroups of breast cancer, basaloid type has the shortest disease-free survival. Survivin is an apoptosis inhibitor and its prognostic and predictive value in breast cancer is under investigation. In this study, we examined the basaloid markers CK5/6, CK14, CK17, and EGFR in triplet-negative patients and evaluated the impact of survivin on survival. Materials and methods Thirty patients with breast cancer in triplet-negative form admitted to Erciyes University Medical Oncology Department between 2001 and 2005 were included in the study. Median follow up and age were 45 months (range 5-76 months) and 47 years (range 2376), respectively. Eighteen patients (60%) were premenopausal and 12 (40%) were postmenopausal. In total, 2, 12, and 14 patients had stage I, II, and III disease, respectively. When cytokeratines and survivin were analyzed independently, association between CK5/6 positivity and lymph node involvement was statistically significant (P = 0.014). In 70% of patients, CK5/6 or EGFR was found positive, and positive results were only had statistically significant correlation with age and menopausal status (P = 0.049 and 0.049, respectively). Ten patients (33.3%) totally and nine patients (42.8%) in the basaloid subgroup had positive staining for survivin. Survivin was not correlated with any of the clinical or histopathological features. While correlation between the number of involved lymph nodes, lymphovascular invasion, histopathological grade, and disease-free survival was statistically significant (P = 0.036, 0.002, and 0.035, respectively), this is not valid for CK5/6, EGFR, and survivin. Conclusion CK5/6 or EGFR was accepted as determinants of basaloid breast cancer. The correlation between basaloid form and other histopathological markers did not reveal any significant difference with respect to prognostic and clinical parameters. We were unable to demonstrate the prognostic impact of survivin in patients with basaloid form or triplet-negative breast cancer

Hypoglycemia in a Patient With Metastatic Gastrointestinal Stromal Tumor: Is Chemotherapy a Reasonable Option for Symptom Control?

Tumor induced hypoglycemia is a rare paraneoplastic phenomenon. However, hypoglycemia may also occur in solid tumors of epithelial or mesenchymal origin. Hypoglycemia caused by these types of tumours is referred to as non-islet cell tumour hypoglycemia (NICTH). Sunitinib is a drug that novel, oral, multitargeted tyrosine kinase. The biologic bases of the activity of sunitinib in patients with GISTs that are target multiple signaling pathways in tumor, stromal, and endothelial compartments. In this report, we describe a case of metastatic gastrointestinal stromal tumor (GIST) presented with loss of consciousness due to hypoglycemia caused or worsened by sunitinib. In our opinion, who the patients with imatinib mesylate and sunitinib malate intolerant GIST, combination chemotherapy consisting of cisplatin and etoposid can be useful in the palliative setting, like our case
