    Nd:YAG Laser Photodisruption for Multilevel Premacular Hemorrhage due to Isolated Retinal Venous Macroaneurysm

    A 55-year-old man presented with sudden deterioration of vision in the right eye. His visual acuity was reduced to hand motion because of a large multilevel premacular hemorrhage. Nd:YAG laser was performed to drain the entrapped hemorrhage under the internal limiting membrane (ILM) and posterior hyaloid face in the macula into the vitreous. Immediately after laser treatment, streaming of red blood cells into the vitreous gel through the perforation site was observed. At the first-month follow-up, BCVA improved to 20/25 and ILM wrinkling was observed at the macula where the preretinal hemorrhage cleared. Fluorescein angiography revealed an isolated retinal venous macroaneurysm located on the macular branch of the superotemporal vein at the bifurcation site. In contrast to retinal arterial macroaneurysms, retinal venous macroaneurysms are quite rare. To the best of our knowledge, this is the first case reported with multilevel premacular hemorrhage caused by an isolated retinal venous macroaneurysm

    Abstract Background To report clinical features of bilateral angle-closure glaucoma in a patient with nanophthalmic eyes associated with ocular cystinosis, foveoschisis and pigmentary retinal dystrophy. This is probably the first published report of the possible association of all these five entities in the same patient. Case presentation A 50-year-old white male was referred for uncontrolled glaucoma in both eyes. He was previously diagnosed with angle-closure glaucoma in association with ocular cystinosis. Ocular examination revealed high hyperopia (+13.5 OD and +14 OS diopters) with reduced axial length (16.27 mm OD and 15.93 mm OS). Despite being on 3 topical medications, his IOP measured 37 mmHg OD and 35 mm Hg OS. Slit-lamp biomicroscopy showed refractile, polychromatic crystalline deposits throughout the cornea and conjunctiva in both eyes. Gonioscopy revealed an extremely narrow angle with peripheral anterior synechiae (PAS). Anterior chamber depths were shallow. Fundus examination disclosed punctate hypopigmentation of the retinal pigment epithelium mainly at the posterior pole. Optical coherence tomography showed foveal schisis appearing as small retinal cysts. The patient did not display any systemic abnormalities. Conclusions This case brings into discussion a new clinical entity of angle closure glaucoma in nanophthalmos accompanied by ocular cystinosis-foveoschisis-pigmentary retinal dystrophy complex.</p

    Evaluation of Plasma Amino Acid Levels in Preterm Infants and Their Potential Correlation with Retinopathy of Prematurity

    Aim. The aim of this study is to ascertain whether the level of circulating amino acids (aa) is associated with retinopathy of prematurity (ROP). Methods. This is a randomized controlled study of 55 infants born at gestational age (GA) ≤32 weeks or birth weight (BW) ≤1500 grams. Serum samples were obtained from two groups: Group A comprised of 26 preterm infants with ROP and Group B comprised of 29 preterm infants without ROP. Plasma aa levels were analyzed using liquid chromatography with tandem mass spectrometry (LC-MS/MS). Correlation test and multivariate regression analysis were used to evaluate the relationship between plasma aa levels and variables. Results. The mean serum arginine and glutamine levels were significantly higher, but the mean lysine and aspartic acid levels were significantly lower in Group A, compared to Group B (p=0.04, p=0.002, p=0.029, and p=0.002, respectively). In multivariate analysis, the mean arginine and lysine levels were significantly associated with the stage of the disease (p=0.03 and p=0.01, respectively). No significant differences were determined between the groups in terms of alanine, asparagine, valine, leucine, phenylalanine, tyrosine, serine, proline, citrulline, cysteine, ornithine, tryptophan, methionine, threonine, taurine, and isoleucine amino acids (p>0.05, respectively). Conclusions. These results indicate a significant association between high arginine and glutamine, with low lysine and aspartic acid serum concentrations with ROP. Due to higher serum concentrations in ROP, extra arginine-glutamine supplementation in hyperoxic conditions may be unfavorable through pathways involving reactive oxygen, particularly in patients with ROP

    Pre-eclampsia is common in the region who are underserved to antenatal care services and constitute risk of anemia.To evaluate the anemia status and its contributing factors in pregnant women a diagnosis of Pre-Eclampsia. This cross-sectional study was conducted from October 2012 to January 2013. This study involved 128 pregnant women hospitalized with a diagnosis of Pre-Eclampsia who consented to being study subjects were included. Statistical analysis was performed using descriptive statistics, Chi-square test, Students t-test, ANOVA and Pearsons correlation, Binary logistic regression coefficient.     Investigation of Combined Effectiveness of Spiramycin and Beta-Glucan in Mice Models of Acute Toxoplasmosis and Determination of IL-10, IL-12 and TNF-alpha Levels

    Toxoplasmosis which is caused by Toxoplasma gondii, has a high risk of fetal infection development if the infection occurs during pregnancy. Treatment with oral spiramycin is recommended during pregnancy in order to prevent the transmission of protozoa to fetus and development of infection. Since beta-glucan is known to stimulate the immune system and increase the phagocytic activity of the cells, it has been shown to exhibit immunomodulatory effect on many infectious diseases. The objectives of this study were to investigate the effectiveness of beta-glucan alone and in combination with spiramycin and to determinate the levels of interlokin (IL)-10, IL-12 and tumor nekrosis factor (TNF)-alpha in mice experimentally infected with T.gondii. For this purpose, four experimental groups each consisting of eight BALB/c mice, were formed with the approval of Ethics Committee for the Animal Experiments. All the mice were infected with 2 ml of suspension containing 2 x 10(2)/ml of trophozoite prepared from T.gondii RH strain (Refik Saydam National Public Health Agency, Parasitology Laboratory of Communicable Diseases Research Department, Ankara, Turkey), by intraperitoneal injection. Twenty-four hours after the infection, beta-glucan (3 mg/day) was given to the beta-glucan group, spiramycin (200 mg/kg/day) to the spiramycin group, beta-glucan (3 mg/day) plus spiramycin (200 mg/kg/day) to the beta-glucan-spiramycin (BG-S) group by oral gavage. The fourth group which was the control group was infected but untreated. The above administration was carried out for seven days. On the 8th day, under anaesthesia, 1 ml normal saline was given into the peritoneum, drawn back later and the number of trophozoites in 1 ml of peritoneal fluid was determined by counting them on the Thoma slide. Moreover, by drawing the heart blood; IL-10, IL-12, TNF-alpha levels were determined in serum samples by ELISA method (eBioscience Platinum, Austria). The number of trophozoites in the BC-S group was found significantly lower than the number of trophozoites in control, beta-glucan and spiramycin groups (p<0.05). There was no significant difference between the beta-glucan and spiramycin groups, however the number of trophozoites in both groups was significantly lower than the number of trophozoites in the control group (p<0.05). There was a certain decrease in IL-10 level in spiramycin and BC-S groups, compared to the control group, in addition when IL-10 levels in spiramycin and BC-S groups were compared with BC group, a significant decrease was noticed (p<0.05). There was no difference in IL-12 levels between the groups, while there was a certain decrease in TNF-alpha level in beta-glucan, spiramycin, BC-S group in comparison to the control group. Within the reach of our literature survey, this study is the first research in which the effectiveness of the combination of beta-glucan and spiramycin in the treatment of acute toxoplasmosis was investigated. The results of our study suggested that there might be synergy between beta-glucan and spiramycin in the treatment of acute toxoplasmosis

The infragluteal sulcus, an important part of the gluteal region, determines the attractiveness of the buttocks. For that reason, any small change in the infragluteal sulcus makes a major cosmetic difference in the morphology of the buttock. In recent years, some flaps such as the infragluteal flap, the transverse upper gracilis flap, and the transverse musculocutaneous gracilis flap have become popular because of reduced donor area morbidity and their availability for hiding incision scars. In these cases, there may be distortion of the gluteal morphology and infragluteal sulcus. Management of AI is a challenge for the patient and the clinician. This case report presents the management of AI in a sixyear-old female patient. Considering the patient's age, we decided to make removable dentures in order to avoid growth and development problems. Conventional complete dentures were made, vertical dimension was increased, and the desired aesthetics and function were gained. Additionally, satisfaction with prosthodontic rehabilitation was evaluated using a questionnaire. A high level of patient and parent satisfaction was obtained. Treatment planning for patients with AI is related to many factors including the age and socioeconomic status of the patient, the type and severity of the disorder, the intraoral situation at the time the treatment is planned and most importantly, cooperation of the patient plays a major role
