Une étude exploratoire. Interaction entre les dyades mère-nourrisson et père-nourrisson chez les couples à faible revenu
Vingt-neuf parents à faible revenu et leurs nourrissons ont participé à une recherche exploratoire portant sur les interactions parent-nourrisson et décrivant le niveau d'interactions, les similarités et les différences au sein des dyades mère-nourrisson et père-nourrisson. L'échelle de mesure Nursing Child Assessment Teaching Scale, un instrument d'observation standardisé de 73 items, a été utilisée pour mesurer les interactions. Les résultats ont montré que bien que la majorité (69%) des dyades parent-nourrisson n'ont pas démontré de comportements interactifs à risque, près de 31% de l'échantillon étaient désorganisés dans leurs interactions. De plus, les résultats ont démontré que bien que les deux parents étaient sensibles et répondaient aux signaux de leurs nourrissons, les mères étaient plus aptes que les pères à répondre à la détresse de leurs nourrissons alors que les nourrissons étaient plus clairs et répondaient mieux aux pères qu'aux mères. Avec cette information, les cliniciens et les chercheurs peuvent mieux comprendre les interactions au sein du contexte familial et renforcer les programmes d'intervention visant à maintenir et à améliorer les interactions parent-nourrisson.Interaction between low-income mother-infant and father-infant pairs: an exploratory study Twenty-nine low-income parents and their infants participated in an explanatory study about parent-infant interaction — describing the level of interaction and the similarities and differences in mother-infant and father infant pairs. The Nursing Child Assessment Teaching Scale, a 73-item standardized observation instrument, was used to measure interaction. Results showed that although the majority (69%) of parent-infant dyads did not demonstrate risky interactive behaviors, nearly 31% of the sample was disorganized in their interactions. Further, results showed that while both mothers and fathers were sensitive and responsive to their infants' cues, mothers were more apt to respond to their infants' distress than fathers, and infants were clearer and more responsive with their fathers than with their mothers. With this information clinicians and researchers can better understand interactions within a family context and strengthen intervention programs aimed at maintaining and improving parent-infant interactions.Una investigación exploratoria. Interacción entre los pares madre-niño lactante y padre-niño lactante en las parejas a ingreso modesto Veinte-nueve padres con ingreso modesto y sus niños lactantes participaron en una investigación exploratoria sobre las interacciones padre-niño lactante y el nivel de interacciones, las similitudes y las diferencias en el seno de las dyadas madre-niño lactante y padre-niño lactante. Para medir las interacciones se utilizó un instrumento de observación standardizado de 73 elementos, la escala de medida Nursing Child Assessment Teaching Scale. Los resultados indican que aunque la mayoridad (69%) de las dyadas padre-niño lactante no demuestran comportamientos interactivos con riesgo, cerca de 31% de la muestra estaban desorganisados en sus interacciones. Además, los resultados demuestraron que aunque los dos padres eran sensibles y respondian a los señales de sus niños lactantes, las madres estaban más aptas que los padres a responder al desemparo de sus niños cuando los niños estaban más tranquilos y respondian mejor a los padres que a las madres. Con esta información, los clínicos y los investigadores pueden comprender mejor las interacciones en el seno del contexto familial y reforzar programas de intervención aspirando mantener y mejorar las interacciones padre-niño lactante
Biloma secondary to blunt liver trauma in a pediatric patient: case report
Liver is the organ most frequently injured after blunt or penetrating abdominal trauma, being in pediatrics a pathology that has an increasing incidence; Non-operative management is the hallmark of treatment, however cases of secondary biliary leakage have been described, which may affect the intrahepatic or extrahepatic track. A male 8 years old, with blunt trauma in the right hypochondrium, presenting acute abdomen and hemodynamic instability, requiring exploratory laparotomy with a grade III liver injury in VI and VII hepatic segments. Four weeks after discharge, he presented as a complication a giant biloma in VII and VIII hepatic segments, performing percutaneous drainage guided by ultrasound. The incidence of biliary complications related to hepatic trauma is low, 4% in pediatric patients, dividing into bilomas or biliary fistulas; the presentation of biliary leakage is very non-specific and early diagnosis difficult; in the bilomas, the tomography allows to define precisely its size, nature, distribution and regional anatomy in relation to adjacent structures, as well as underlying cause. The approach of choice is percutaneous or endoscopic drainage, with surgical management being the last option
A case report on choledochoduodenal fistula: how to suspect this unusual entity?
A choledochoduodenal fistula is an abnormal connection between the common bile duct and the duodenum, which are associated with a history of problems in the common bile duct. It has appeared in 0.74% of patients submitted for biliary tract surgery. The most frequent symptoms of non-obstructive enteric biliary fistulas are: epigastric pain, cholangitis (80.91%), jaundice (54,.26%), fever (50.69%), nausea and/or vomiting (10.30%), abdominal distension (0.39%), asymptomatic (0.11%), and diarrhea (0.11%). Diagnostic imaging methods provide the data of greatest interest in revealing the presence of air in the bile duct. This method, as well as barium reflux under the biliary tree in contrasted studies and in ERCP, reveal the fistulous tract and its location. Neither the prevalence, nor the clinical characteristics that pertain to its presentation, are well known among our population. Possible treatments for this illness include conservative treatment with medication, endoscopic sphincterotomy, and surgical therapy
Pedagogical strategies to develop citizenship competencies in law students
This article analyses the teaching strategies used in the process of
teaching and learning civic skills. To this end, developed a joint
methodological design that integrates techniques and instruments of
qualitative and quantitative cut such as interviews with focus groups and
surveys to teachers and students. The findings allowed to identify the lack
of congruence between the training and exercise in citizenship skills. Also,
pedagogical recommendations are formulated to facilitate the
apprehension of these competences.Este artículo analiza las estrategias pedagógicas empleadas en el proceso
de enseñanza-aprendizaje para desarrollar competencias ciudadanas. Para

Incidence of mindfulness and Qi Gong on the State of Health, Psychological Well-Being, life satisfaction and work stress
El presente estudio se realizó a través de un abordaje metodológico pre-experimental con medidas de tipo pre-test y post-test. Participaron 52 trabajadores de la Universidad de la Costa (Barranquilla, Colombia), quienes asistieron por invitación al taller de ocho horas de mindfulness y qi gong para el mejoramiento de la salud, bienestar psicológico, satisfacción vital y reducción del estrés. Los instrumentos utilizados fueron el Cuestionario de Salud sf-11, Escala de Bienestar Psicológico (Ryff & Keyes, 1995), Escala de Satisfacción Vital (Cortés, 2013) y Escala Numérica de Estrés Laboral. La combinación de mindfulness y qi gong produjo una mejoría en el estado de salud de los trabajadores —salud general, energía, eficiencia, actividades domésticas y disminución del dolor—, cambios importantes en el bienestar psicológico —adaptación, confianza, claridad, apertura y autoestima—, y mejoramiento de la satisfacción vital —salud y productividad—. No se encontraron diferencias significativas en los niveles de estrés laboralThis study was conducted through a pre-experimental methodological approach with pre-test and post-test measurements. 52 workers from the Universidad de la Costa participated were invited to attend the eight-hour mindfulness and qi gong workshop for improving health, psychological well-being, life satisfaction and stress reduction. The instruments used were the Health Survey sf-11, Scale of Psychological Well-Being (Ryff & Keyes, 1995), Life Satisfaction Scale (Cortés, 2013) and Occupational Stress Numerical Scale. The combination of mindfulness and qi gong produced an improvement in the health of the workers —general health, energy, efficiency, domestic activities and diminution of pain, important changes in the psychological well-being — adaptation, confidence, clarity, openness and self-esteem, and improvement in satisfaction— health and productivity. No significant differences in work stress levels were found.
Detection of Anopheles stephensi mosquitoes by molecular surveillance, Kenya
The Anopheles stephensi mosquito is an invasive malaria vector recently reported in Djibouti, Ethiopia, Sudan, Somalia, Nigeria, and Ghana. The World Health Organization has called on countries in Africa to increase surveillance efforts to detect and report this vector and institute appropriate and effective control mechanisms. In Kenya, the Division of National Malaria Program conducted entomological surveillance in counties at risk for An. stephensi mosquito invasion. In addition, the Kenya Medical Research Institute conducted molecular surveillance of all sampled Anopheles mosquitoes from other studies to identify An. stephensi mosquitoes. We report the detection and confirmation of An. stephensi mosquitoes in Marsabit and Turkana Counties by using endpoint PCR and morphological and sequence identification. We demonstrate the urgent need for intensified entomological surveillance in all areas at risk for An. stephensi mosquito invasion, to clarify its occurrence and distribution and develop tailored approaches to prevent further spread
Burnout among surgeons before and during the SARS-CoV-2 pandemic: an international survey
Background: SARS-CoV-2 pandemic has had many significant impacts within the surgical realm, and surgeons have been obligated to reconsider almost every aspect of daily clinical practice. Methods: This is a cross-sectional study reported in compliance with the CHERRIES guidelines and conducted through an online platform from June 14th to July 15th, 2020. The primary outcome was the burden of burnout during the pandemic indicated by the validated Shirom-Melamed Burnout Measure. Results: Nine hundred fifty-four surgeons completed the survey. The median length of practice was 10 years; 78.2% included were male with a median age of 37 years old, 39.5% were consultants, 68.9% were general surgeons, and 55.7% were affiliated with an academic institution. Overall, there was a significant increase in the mean burnout score during the pandemic; longer years of practice and older age were significantly associated with less burnout. There were significant reductions in the median number of outpatient visits, operated cases, on-call hours, emergency visits, and research work, so, 48.2% of respondents felt that the training resources were insufficient. The majority (81.3%) of respondents reported that their hospitals were included in the management of COVID-19, 66.5% felt their roles had been minimized; 41% were asked to assist in non-surgical medical practices, and 37.6% of respondents were included in COVID-19 management. Conclusions: There was a significant burnout among trainees. Almost all aspects of clinical and research activities were affected with a significant reduction in the volume of research, outpatient clinic visits, surgical procedures, on-call hours, and emergency cases hindering the training. The evolving SARS-CoV-2 epidemic in Africa: Insights from rapidly expanding genomic surveillance
INTRODUCTION
Investment in Africa over the past year with regard to severe acute respiratory syndrome coronavirus 2 (SARS-CoV-2) sequencing has led to a massive increase in the number of sequences, which, to date, exceeds 100,000 sequences generated to track the pandemic on the continent. These sequences have profoundly affected how public health officials in Africa have navigated the COVID-19 pandemic.
RATIONALE
We demonstrate how the first 100,000 SARS-CoV-2 sequences from Africa have helped monitor the epidemic on the continent, how genomic surveillance expanded over the course of the pandemic, and how we adapted our sequencing methods to deal with an evolving virus. Finally, we also examine how viral lineages have spread across the continent in a phylogeographic framework to gain insights into the underlying temporal and spatial transmission dynamics for several variants of concern (VOCs).
RESULTS
Our results indicate that the number of countries in Africa that can sequence the virus within their own borders is growing and that this is coupled with a shorter turnaround time from the time of sampling to sequence submission. Ongoing evolution necessitated the continual updating of primer sets, and, as a result, eight primer sets were designed in tandem with viral evolution and used to ensure effective sequencing of the virus. The pandemic unfolded through multiple waves of infection that were each driven by distinct genetic lineages, with B.1-like ancestral strains associated with the first pandemic wave of infections in 2020. Successive waves on the continent were fueled by different VOCs, with Alpha and Beta cocirculating in distinct spatial patterns during the second wave and Delta and Omicron affecting the whole continent during the third and fourth waves, respectively. Phylogeographic reconstruction points toward distinct differences in viral importation and exportation patterns associated with the Alpha, Beta, Delta, and Omicron variants and subvariants, when considering both Africa versus the rest of the world and viral dissemination within the continent. Our epidemiological and phylogenetic inferences therefore underscore the heterogeneous nature of the pandemic on the continent and highlight key insights and challenges, for instance, recognizing the limitations of low testing proportions. We also highlight the early warning capacity that genomic surveillance in Africa has had for the rest of the world with the detection of new lineages and variants, the most recent being the characterization of various Omicron subvariants.
CONCLUSION
Sustained investment for diagnostics and genomic surveillance in Africa is needed as the virus continues to evolve. This is important not only to help combat SARS-CoV-2 on the continent but also because it can be used as a platform to help address the many emerging and reemerging infectious disease threats in Africa. In particular, capacity building for local sequencing within countries or within the continent should be prioritized because this is generally associated with shorter turnaround times, providing the most benefit to local public health authorities tasked with pandemic response and mitigation and allowing for the fastest reaction to localized outbreaks. These investments are crucial for pandemic preparedness and response and will serve the health of the continent well into the 21st century
