Association Between Aldosterone and Hypertension Among Patients With Overt and Subclinical Hypercortisolism
INTRODUCTION: Hypertension is one of the most common clinical features of patients with overt and subclinical hypercortisolism. Although previous studies have shown the coexistence of autonomous cortisol and aldosterone secretion, it is unclear whether aldosterone plays a role in hypertension among patients with hypercortisolism. Therefore, we examined the associations of plasma aldosterone concentrations (PACs) with hypertension among patients with overt and subclinical hypercortisolism. METHODS: This single-center retrospective cohort study included patients with adrenal tumor and serum cortisol levels after 1-mg dexamethasone suppression test >1.8 µg/dL (50 nmol/L). Using multivariable regression models adjusting for baseline characteristics, we investigated the association of PACs with systolic blood pressure and postoperative improvement of hypertension after the adrenalectomy. RESULTS: Among 89 patients enrolled in this study (median age, 51 years), 21 showed clinical signs of Cushing syndrome (overt hypercortisolism) and 68 did not show clinical presentations (subclinical hypercortisolism). We found that higher PACs were significantly associated with elevated systolic blood pressure among patients with subclinical hypercortisolism (adjusted difference [95% CI] = +0.59 [0.19-0.99], P = 0.008) but not among those with overt hypercortisolism. Among 33 patients with subclinical hypercortisolism and hypertension who underwent adrenalectomy, the postoperative improvement of hypertension was significantly associated with higher PACs at baseline (adjusted risk difference [95% CI] = +1.45% [0.35-2.55], P = 0.01). CONCLUSION: These findings indicate that aldosterone may contribute to hypertension among patients with subclinical hypercortisolism. Further multi-institutional and population-based studies are required to validate our findings and examine the clinical effectiveness of the intervention targeting aldosterone for such patients
Partial purification of Simian virus 40 large T antigen by immunoaffinity chromatography and its detection by enzyme-linked immunosorbent assay.
Simian virus 40 (SV40) large T antigen was partially purified from small amounts of SV40-infected and SV40-transformed cells by immunoaffinity chromatography with high recovery. T antigen, in both crude and partially purified states, was detected rapidly by a sensitive and quantitative enzyme-linked immunosorbent assay (ELISA). Stability of the partially purified T antigen was found to increase by addition of 0.01% bovine serum albumin (BSA).</p
A System for Worldwide COVID-19 Information Aggregation
The global pandemic of COVID-19 has made the public pay close attention to
related news, covering various domains, such as sanitation, treatment, and
effects on education. Meanwhile, the COVID-19 condition is very different among
the countries (e.g., policies and development of the epidemic), and thus
citizens would be interested in news in foreign countries. We build a system
for worldwide COVID-19 information aggregation
(http://lotus.kuee.kyoto-u.ac.jp/NLPforCOVID-19 ) containing reliable articles
from 10 regions in 7 languages sorted by topics for Japanese citizens. Our
reliable COVID-19 related website dataset collected through crowdsourcing
ensures the quality of the articles. A neural machine translation module
translates articles in other languages into Japanese. A BERT-based
topic-classifier trained on an article-topic pair dataset helps users find
their interested information efficiently by putting articles into different

近年、抗がん薬の曝露対策の重要性が周知され、職業性曝露対策ガイドラインに基づく対策が、各施設で実施されている。CSTD 使用、PPE 装着、患者への曝露対策のオリエンテーションを実施している外来化学療法関連施設で、曝露対策効果の評価を目的とし、揮発性薬剤のシクロホスファミド・在宅で投薬をするフルオロウラシル・使用頻度の高いゲムシタビン・パクリタキセル(アルブミン懸濁型も含む)の4種類の薬剤を指標として、調製から投薬、患者の排泄までの一連の流れでの環境曝露を調査した。薬剤調製エリア、点滴準備エリアにおいては薬剤の検出があったが、治療環境では検出されず、ガイドラインに基づく薬剤師と看護師の曝露対策の効果として評価できた。患者の在宅物品である在宅ポンプ表面・廃棄袋の汚染、および患者用トイレからの薬剤検出があり、今後の曝露対策の強化の必要性が示唆された。資料Information

Vascular Aldosterone Production at the Pre-Diabetic Stage of Young Otsuka Long-Evans Tokushima Fatty (OLETF) Rats, Compared with Long-Evans Tokushima Otsuka (LETO) Rats
We examined the ability of aortic smooth muscle cells (AoSMC) prepared from spontaneously diabetic rats to produce aldosterone (Aldo) and the regulatory mechanism that controls their Aldo production. AoSMC of 6 week-old Long-Evans Tokushima Otsuka (LETO: the control group) and 6 week-old Otsuka Long-Evans Tokushima Fatty (OLETF: the type 2 diabetes group) rats were used in the present experiments. CYP11B2 (Aldo synthetase) mRNA expression was detected in both the LETO and OLETF AoSMC. Basal Aldo production was significantly greater (4–5 fold higher) in the OLETF AoSMC culture medium than in the LETO AoSMC culture medium. When AoSMC were co-incubated with high-density lipoproteins (HDL), supplying cholesterol as a substrate for steroidogenesis in rats, angiotensin II (AII) significantly increased greater Aldo production in the OLETF AoSMC than in the LETO AoSMC. The present data suggested that future onset of diabetic vascular dysfunction is partly caused by excess Aldo production by AoSMC in young OLETF rats. Concomitant stimulation by HDL and AII resulted in elevated Aldo production in the OLETF and the LETO AoSMC, and also demonstrated that AII-induced Aldo production is greatly enhanced by HDL in OLETF, rather than in LETO. In conclusion, our data clearly demonstrated that Aldo production in the OLETF AoSMC was significantly higher than in the LETO AoSMC, suggesting possible future onset of vascular dysfunction in diabetes, induced by local Aldo production in the AoSMC
