    Cementoblastoma Arising in the Maxilla of an 8-Year-Old Boy: A Case Report

    Cementoblastoma is an uncommon disease, representing only 1–8% of all odontogenic tumours. Furthermore, this tumour is especially uncommon in children, as only five cases have been reported in this age group. Here, we describe a case of cementoblastoma arising in the maxilla of an 8-year-old boy, that was treated with a partial maxillectomy. The patient's facial appearance has remained satisfactory, and the tumour has not recurred in the 9 years after the operation

    Küttner's tumor of the sub-mandibular gland associated with fibrosclerosis and follicular hyperplasia of regional lymph nodes: a case report

    <p>Abstract</p> <p>Introduction</p> <p>Küttner's tumor is characterized through histology by peri-ductal fibrosis, dense lymphocytic infiltration with lymphoid follicles, loss of acini, and occasional marked sclerosis of the salivary gland. On occasion, Küttner's tumor can be difficult to distinguish from malignant neoplasm.</p> <p>Case presentation</p> <p>A 58-year-old Japanese man was referred to our hospital with a three-month history of a painless swollen mass in the right sub-mandibular region. Histological findings revealed both lymphoid follicles with reactive germinal centers and variously sized lymphoid follicle-like nodules without definitive germinal centers or mantle zones. B-cells of similar size and shape occupied the lymphoid follicle-like nodules and stained positive for B-cell lymphoma. These cells were detected in the polyclonal B-cells by flow cytometric analysis and tested negative for CD10. Unusual B-cell proliferation was observed, but as there was no definitive evidence of B-cell lymphoma, the lesion was diagnosed as Küttner's tumor.</p> <p>Conclusion</p> <p>We report on a rare case of Küttner's tumor associated with fibrosclerosis and atypical lymphoid hyperplasia in both the sub-mandibular gland and regional lymph nodes. Although more cases need to be investigated, our findings might be helpful to further studies seeking to clarify the etiology of idiopathic sclerosing lesions arising in the organs and regional lymph nodes.</p

    Multicenter phase II trial of preoperative chemoradiotherapy with S-1 for locally advanced oral squamous cell carcinoma

    PURPOSE: We evaluated whether preoperative chemotherapy with S-1 and concurrent radiotherapy is feasible and efficacious in the treatment of advanced oral squamous cell carcinoma. METHODS: Participants comprised 39 patients with oral carcinoma (stage III, n = 15; stage IVA, n = 24). All patients received a total radiation dose of 40 Gy, in once-daily 2-Gy fractions, and received S-1 at 65 mg/m(2)/day for 5 consecutive days, over 4 consecutive weeks with concurrent radiotherapy. RESULTS: Hematological toxicity was mild and reversible. The most common non-hematological toxicity was grade 3 mucositis, but this was transient and tolerable. Radical surgery was performed for 37 patients, with the remaining 2 patients declining the surgery. Postoperatively, local failure developed in 1 patient, and neck failure in 2 patients. Distant metastases were identified in 4 patients. At a median follow-up of 38.0 months (range 23–88 months), locoregional control, disease-specific survival, and overall survival rates at 3 years were 91.5, 83.8, and 83.8 %, respectively. CONCLUSION: Concurrent administration of S-1 and radiotherapy combined with surgery offers a well-tolerated method of successfully treating advanced oral squamous cell carcinoma. The locoregional control rate remains high even at 3 years of follow-up, and no serious adverse effects have been encountered

    Acute Angioedema and Urticaria Associated with Herpes Simplex Infection

    U radu je predstavljen akutni angioedem s urtikarijom koji je možda potaknula infekcija virusom herpes simplex. Japanac u dobi od 58 godina imao je na liječničkom pregledu urtikariju i eritematozni osip na udovima i torzu. Žalio se i na angioedem s oticanjem usana, jezika, lica te periorbitalnoga tkiva, pa je imao poteškoće u disanju i govoru, a osjećao je i ispunjenost ždrijela. Nakon šest dana na donjoj su se usni pojavili ulkusi s krastama. Od pretraga su liječnici tražili kompletnu krvnu sliku, biokemijski screening za titrove antitijela kod viralnih infekcija te test za preosjetljivost na lijekove. Zanimljiva je bila serumska fiksacija titra antitijela na HSV - iznosila je 1:32. Osim oralne ulceracije, pacijentu je bila postavljena i dijagnoza herpes labialis. Bio je primljen u bolnicu te intravenski liječen antihistaminom, penicilinom i kortikosteroidom, te su se nakon tri dana simptomi znatno popravili. Tijekom trogodišnjeg praćenja kod njega se nisu razvili ni angioedem ni urtikarija. U ovom radu opisani su klinička slika, liječenje i moguća povezanost angioedema i urtikarije s infekcijom virusom herpes simplex. Ipak, treba uzeti u obzir mogućnost slučajnog istodobnog pojavljivanja i virusa herpes simplex.We report a case of acute angioedema and urticaria, which is possibly induced by herpes simplex virus infection. A Japanese 58-year-old male patient presented urticaria including cutaneous erythematous rashes at extremities and trunk. He also developed angioedema which was characterized by swelling of lips, tongue, face, and periorbital tissue with difficulty in breathing, speaking, and fullness in throat. Six days later, crusted ulcers were detected on lower lip. Complete blood studies and biochemical screening examination, antibody titers for viral infections, urinalysis and scratch test for drug hypersensitivity have been investigated. Interestingly, serum complement fixation antibody titer for HSV howed high titer at 1:32. Accompanied by oral ulcerations, he was diagnosed as herpes labialis. The patient was admitted and treated intravenously with antihistamine, penicillin, and corticosteroid. After 3 days of treatment, his symptoms significantly improved. From 3-year followup, he had not developed angioedema and urticaria. This letter shows clinical pictures, treatment and possibility of the relation of angioedema and urticaria with herpes simplex virus infection. However, one should also consider the possibility of coincidence of herpes simplex virus infection

    A Case of Basal Cell Adenoma of the Upper Lip

    Basal cell adenoma is a rare type of benign salivary gland tumor found most commonly in the parotid gland. We present a rare case of basal cell adenoma arising in the minor salivary gland of the upper lip. The patient was a 59-year-old Japanese man who visited our department in December 2012 with a chief complaint of a mass in the upper lip, which had increased in size over several years. A mobile, elastic, and relatively soft mass without tenderness was palpable in the upper lip region. The mucosa of the upper lip covering the mass was normal. Tumor extirpation was performed under local anesthesia. Histologically, the tumor had a capsule and was composed of islands of relatively uniform, monotonous cells. Immunohistochemically, the inner tumor comprised tubuloductal structures that showed strong staining for CK7, while the outer tumor showed weak staining for CK7. The outer tumor cells also stained positively for CD10 and p63. The MIB-1 (Ki-67) labeling index was extremely low. Basal cell adenoma was diagnosed based on these results. The postoperative course was uneventful 12 months after surgery and there has been no recurrence

    Genetic Alterations in Gorlin Syndrome

    Gorlin syndrome (GS) is an autosomal dominant disorder that predisposes affected individuals to developmental defects and tumorigenesis, and caused mainly by heterozygous germline PTCH1 mutations. Despite exhaustive analysis, PTCH1 mutations are often unidentifiable in some patients; the failure to detect mutations is presumably because of mutations occurred in other causative genes or outside of analyzed regions of PTCH1, or copy number alterations (CNAs). In this study, we subjected a cohort of GS-affected individuals from six unrelated families to next-generation sequencing (NGS) analysis for the combined screening of causative alterations in Hedgehog signaling pathway-related genes. Specific single nucleotide variations (SNVs) of PTCH1 causing inferred amino acid changes were identified in four families (seven affected individuals), whereas CNAs within or around PTCH1 were found in two families in whom possible causative SNVs were not detected. Through a targeted resequencing of all coding exons, as well as simultaneous evaluation of copy number status using the alignment map files obtained via NGS, we found that GS phenotypes could be explained by PTCH1 mutations or deletions in all affected patients. Because it is advisable to evaluate CNAs of candidate causative genes in point mutation-negative cases, NGS methodology appears to be useful for improving molecular diagnosis through the simultaneous detection of both SNVs and CNAs in the targeted genes/regions

    Partial purification of Simian virus 40 large T antigen by immunoaffinity chromatography and its detection by enzyme-linked immunosorbent assay.

    Simian virus 40 (SV40) large T antigen was partially purified from small amounts of SV40-infected and SV40-transformed cells by immunoaffinity chromatography with high recovery. T antigen, in both crude and partially purified states, was detected rapidly by a sensitive and quantitative enzyme-linked immunosorbent assay (ELISA). Stability of the partially purified T antigen was found to increase by addition of 0.01% bovine serum albumin (BSA).</p

    金沢大学医薬保健研究域医学系We present a series of 8 consecutive patients who underwent the omental pedicle flap method in the field of thoracic surgery. Four of them had empyema with bronchopleural fistula and four had mediastinal infection. A pedicle of omentum containing the right gastroepiploic artery was passed through a tunnel in the diaphragm into the empyema cavity or the mediastinal abscess cavity. All eight patients were cured successfully by single-stage procedures. We think this series demonstrates the value of omental pedicle flap method which lacks in symptoms due to defected organ and offers an excellent therapeutic effect with easy operative procedure. And, this method produce a quality of life. Further active application of this method can be expected in the management of chest problems.

    Population Structure of Humpback Whales from Their Breeding Grounds in the South Atlantic and Indian Oceans

    Although humpback whales are among the best-studied of the large whales, population boundaries in the Southern Hemisphere (SH) have remained largely untested. We assess population structure of SH humpback whales using 1,527 samples collected from whales at fourteen sampling sites within the Southwestern and Southeastern Atlantic, the Southwestern Indian Ocean, and Northern Indian Ocean (Breeding Stocks A, B, C and X, respectively). Evaluation of mtDNA population structure and migration rates was carried out under different statistical frameworks. Using all genetic evidence, the results suggest significant degrees of population structure between all ocean basins, with the Southwestern and Northern Indian Ocean most differentiated from each other. Effective migration rates were highest between the Southeastern Atlantic and the Southwestern Indian Ocean, followed by rates within the Southeastern Atlantic, and the lowest between the Southwestern and Northern Indian Ocean. At finer scales, very low gene flow was detected between the two neighbouring sub-regions in the Southeastern Atlantic, compared to high gene flow for whales within the Southwestern Indian Ocean. Our genetic results support the current management designations proposed by the International Whaling Commission of Breeding Stocks A, B, C, and X as four strongly structured populations. The population structure patterns found in this study are likely to have been influenced by a combination of long-term maternally directed fidelity of migratory destinations, along with other ecological and oceanographic features in the region

    看護基礎教育における自己教育力育成に向けた”だんだんeポートフォリオ”システムの活用

    “eポートフォリオによる自己教育力育成”が文部科学省平成20年度「質の高い大学教育推進プログラム」に選定され、“だんだんeポートフォリオ”システムを開発し、報告してきた。今回は、入学から2年間活用してきた学生93名を対象に「プロフィール」「学びの蓄積」「課題提出」「参考資料」の機能について評価した。その結果、活用した者はそうでない者よりシステムに対する評価は高かった。このことより、システムに慣れていくことや活用の動機づけが重要と考えられた。また、新たに「看護基本技術自己評価」の機能を追加し、“だんだんeポートフォリオ”システムが一定の完成を得たので紹介する
