    Road Traffic Noise and Incident Myocardial Infarction: A Prospective Cohort Study

    BACKGROUND Both road traffic noise and ambient air pollution have been associated with risk for ischemic heart disease, but only few inconsistent studies include both exposures. METHODS In a population-based cohort of 57 053 people aged 50 to 64 years at enrolment in 1993-1997, we identified 1600 cases of first-ever MI between enrolment and 2006. The mean follow-up time was 9.8 years. Exposure to road traffic noise and air pollution from 1988 to 2006 was estimated for all cohort members from residential address history. Associations between exposure to road traffic noise and incident MI were analysed in a Cox regression model with adjustment for air pollution (NO(x)) and other potential confounders: age, sex, education, lifestyle confounders, railway and airport noise. RESULTS We found that residential exposure to road traffic noise (L(den)) was significantly associated with MI, with an incidence rate ratio IRR of 1.12 per 10 dB for both of the two exposure windows: yearly exposure at the time of diagnosis (95% confidence interval (CI): 1.02-1.22) and 5-years time-weighted mean (95% CI: 1.02-1.23) preceding the diagnosis. Visualizing of the results using restricted cubic splines showed a linear dose-response relationship. CONCLUSIONS Exposure to long-term residential road traffic noise was associated with a higher risk for MI, in a dose-dependent manner

    Aviation Noise and Cardiovascular Health in the United States: a Review of the Evidence and Recommendations for Research Direction

    In the USA, there is mounting pressure on aviation operators and regulators to address concerns about community impacts of aircraft noise given increasing evidence of adverse health impacts, continuing community complaints, availability of cost-effective programs to reduce exposures to aircraft noise, and more stringent international policies. In the USA, regulation of civil aviation noise is the responsibility of the Federal Aviation Administration (FAA), which requires a “significant body of scientific support,” particularly applicable to the USA, to inform health-based policy and regulatory decisions. However, there have been very few studies investigating the relationship between noise and health in the USA and limited studies across the globe characterizing the effects of aviation noise specifically on cardiovascular health. This review focuses on recent findings on the relationship between aircraft noise and cardiovascular outcomes and directions for future research.Epidemiological studies generally report statistically significant associations between aircraft noise and adverse cardiovascular outcomes, although with limited evidence within the USA. Sleep disturbance, associated with nighttime noise, has been shown to be a risk factor for cardiovascular disease given associations with inflammatory markers and metabolic changes. Given numerous cardiovascular markers, the most appropriate choices depend on the ultimate objectives of the individual studies.Given the state of the literature, future research should leverage emerging tools to estimate aviation, railway, and road traffic noise and apply noise estimates to a range of epidemiological study designs and endpoints to inform causal interpretation and help determine potential intervention strategies

    Sleep duration and disorders in pregnancy: implications for glucose metabolism and pregnancy outcomes

    Humans have an innate requirement for sleep that is intrinsically governed by circadian and endocrine systems. More recently, reduced sleep duration has gained significant attention for its possible contribution to metabolic dysfunction. Significant evidence suggests that reduced sleep duration may elevate the risk for impaired glucose functioning, insulin resistance and type 2 diabetes. However, to date, few studies have determined the implications of reduced sleep duration with regard to glucose control during pregnancy. With the high prevalence of overweight and obesity in women of reproductive age, the occurrence of gestational diabetes mellitus (GDM) is increasing. GDM results in elevated risk of maternal and fetal complications, as well as increased risk of type 2 diabetes postpartum. Infants born to women with GDM also carry a life-long risk of obesity and type 2 diabetes. The impact of reduced sleep on glucose management during pregnancy has not yet been fully assessed and a paucity of literature currently exits. Herein, we review the association between reduced sleep and impaired carbohydrate metabolism and propose how reduced sleep during pregnancy may result in further dysfunction of the carbohydrate axis. A particular focus will be given to sleep-disordered breathing, as well as GDM-complicated pregnancies. Putative mechanisms of action by which reduced sleep may adversely affect maternal and infant outcomes are also discussed. Finally, we will outline important research questions that need to be addressed
