    Aetiology of Depression: Insights from epidemiological and genetic research

    This thesis includes several population-based studies that explore the aetiology of depression, with a specific interest on biological factors, genetics and epigenetics, and physical health factors for depression. Unravelling the aetiology of depression could potentially answer some remaining questions about depression, and finally may explain why we consistently fail to develop effective management and treatment tools for depression. Therefore, this thesis aims to apply advanced epidemiological studies and to extend the existing knowledge on the aetiology of depression. Using population- based data, the studies described in this thesis examine several risk factors and predictors that may enlighten the pathophysiological mechanism that underlie the development of depression. Specifically, Chapter 2 of this thesis presents longitudinal studies that examine the impact of potential biomarkers, such as vitamin D (Chapter 2.1), and inflammatory markers (Chapter 2.2) on the occurrence of depression. Chapter 3 of this thesis presents two studies which apply advanced genetic epidemiological methods to study the genetics of depression. Chapter 4 focuses on the epigenetics of depression and presents the largest epigenome wide association population-based study so far. Moreover, we dedicated a chapter to the impact of physical health conditions such as myocardial infarct on depression (Chapter 5.1) as well as one to the physical consequences of depression such as cognitive decline (Chapter 5.2). Finally, Chapter 6 provides a more general discussion of the main findings in this thesis and addresses several methodological considerations of the studies. Clinical implications of the results in this thesis, and future directions are also presented

    A Meta-Analytic Review of the Dark Triad-Intelligence Connection

    We conducted a meta-analytic review of the relations between general mental ability (GMA) and the Dark Triad (DT) personality traits—Machiavellianism, narcissism, and psychopathy—to determine if individuals who display socially exploitative social qualities tend to be more intelligent or less intelligent. Across 48 independent samples, GMA showed no consistent relation with any DT trait. These effects were not sufficient to support either the “evil genius” hypothesis (highly intelligent individuals tend to display socially exploitative personality traits) or the “compensatory” hypothesis (less intelligent individuals compensate for their cognitive disadvantages by adopting manipulative behavioral tendencies). However, these relations were moderated, to some extent, by the sex and age of the participants, type of sample studied, and the measure of GMA

    Improving Outcomes from Breast Cancer in a Low-Income Country: Lessons from Bangladesh

    Women in low- and middle-income countries (LMICs) have yet to benefit from recent advances in breast cancer diagnosis and treatment now experienced in high-income countries. Their unique sociocultural and health system circumstances warrant a different approach to breast cancer management than that applied to women in high-income countries. Here, we present experience from the last five years working in rural Bangladesh. Case and consecutive series data, focus group and individual interviews, and clinical care experience provide the basis for this paper. These data illustrate a complex web of sociocultural, economic, and health system conditions which affect womens' choices to seek and accept care and successful treatment. We conclude that health system, human rights, and governance issues underlie high mortality from this relatively rare disease in Bangladesh

    Clinic-level variations in contact tracing outcomes: protocol for a cross-sectional study of variation and case-mix adjustment in London, UK

    Contact tracing around patients with infectious tuberculosis is used to identify further cases and interrupt transmission. Approaches to contact tracing are thought to vary between different clinical teams, but the extent of this variation and the importance of case mix are unknown. This project will use data from the London Tuberculosis Register to analyse the extent of variation in the number of contacts identified and the proportion that are assessed/screened, and use hierarchical modelling to test whether differences in case-mix explain any variation
