    Accurate Machine Learning Atmospheric Retrieval via a Neural Network Surrogate Model for Radiative Transfer

    Atmospheric retrieval determines the properties of an atmosphere based on its measured spectrum. The low signal-to-noise ratio of exoplanet observations require a Bayesian approach to determine posterior probability distributions of each model parameter, given observed spectra. This inference is computationally expensive, as it requires many executions of a costly radiative transfer (RT) simulation for each set of sampled model parameters. Machine learning (ML) has recently been shown to provide a significant reduction in runtime for retrievals, mainly by training inverse ML models that predict parameter distributions, given observed spectra, albeit with reduced posterior accuracy. Here we present a novel approach to retrieval by training a forward ML surrogate model that predicts spectra given model parameters, providing a fast approximate RT simulation that can be used in a conventional Bayesian retrieval framework without significant loss of accuracy. We demonstrate our method on the emission spectrum of HD 189733 b and find good agreement with a traditional retrieval from the Bayesian Atmospheric Radiative Transfer (BART) code (Bhattacharyya coefficients of 0.9843--0.9972, with a mean of 0.9925, between 1D marginalized posteriors). This accuracy comes while still offering significant speed enhancements over traditional RT, albeit not as much as ML methods with lower posterior accuracy. Our method is ~9x faster per parallel chain than BART when run on an AMD EPYC 7402P central processing unit (CPU).     Retreatment for hepatitis C virus direct-acting antiviral therapy virological failure in primary and tertiary settings: The REACH-C cohort

    Virological failure occurs in a small proportion of people treated for hepatitis C virus (HCV) with direct-acting antiviral (DAA) therapies. This study assessed retreatment for virological failure in a large real-world cohort. REACH-C is an Australian observational study (n = 10,843) evaluating treatment outcomes of sequential DAA initiations across 33 health services between March 2016 to June 2019. Virological failure retreatment data were collected until October 2020. Of 408 people with virological failure (81% male; median age 53; 38% cirrhosis; 56% genotype 3), 213 (54%) were retreated once; 15 were retreated twice. A range of genotype specific and pangenotypic DAAs were used to retreat virological failure in primary (n = 56) and tertiary (n = 157) settings. Following sofosbuvir/velpatasvir/voxilaprevir availability in 2019, the proportion retreated in primary care increased from 21% to 40% and median time to retreatment initiation declined from 294 to 152 days. Per protocol (PP) sustained virological response (SVR12) was similar for people retreated in primary and tertiary settings (80% vs 81%; p = 1.000). In regression analysis, sofosbuvir/velpatasvir/voxilaprevir (vs. other regimens) significantly decreased likelihood of second virological failure (PP SVR12 88% vs. 77%; adjusted odds ratio [AOR] 0.29; 95%CI 0.11–0.81); cirrhosis increased likelihood (PP SVR12 69% vs. 91%; AOR 4.26; 95%CI 1.64–11.09). Indigenous Australians had lower likelihood of retreatment initiation (AOR 0.36; 95%CI 0.15–0.81). Treatment setting and prescriber type were not associated with retreatment initiation or outcome. Virological failure can be effectively retreated in primary care. Expanded access to simplified retreatment regimens through decentralized models may increase retreatment uptake and reduce HCV-related mortality

    Cost-effectiveness analysis of an outreach model of Hepatitis C Virus (HCV) assessment to facilitate HCV treatment in primary care

    The effects of hepatitis C virus (HCV), such as morbidity and mortality associated with cirrhosis and liver cancer, is a major public health issue in Australia. Highly effective treatment has recently been made available to all Australians living with HCV. A decision-analytic model was developed to evaluate the cost-effectiveness of the hepatology partnership, compared to usual care. A Markov model was chosen, as it is state-based and able to include recursive events, which accurately reflects the natural history of the chronic and repetitive nature of HCV. Cost-effectiveness of the new model of care is indicated by the incremental cost-effectiveness ratio (ICER), where the mean change to costs associated with the new model of care is divided by the mean change in quality adjusted life-years (QALYs). Ten thousand iterations of the model were run, with the majority (73%) of ICERs representing cost-savings. In comparison to usual care, the intervention improves health outcomes (22.38 QALYs gained) and reduces costs by $42,122 per patient. When compared to usual care, a partnership approach to management of HCV is cost-effective and good value for money, even when key model parameters are changed in scenario analyses. Reduction in costs is driven by improved efficiency of the new model of care, where more patients are treated in a timely manner, away from the expensive tertiary setting. From an economic perspective, a reduction in hospital-based care is a positive outcome and represents a good investment for decision-makers who wish to maximise health gain per dollar spent

    Post-auricular muscle responses (PAMRs) were recorded in sixteen adults with normal hearing and twenty adults with sensorineural hearing loss. Click stimuli were presented at 20 to 80 dB nHL via insert earphones. Only one ear was tested in hearing-impaired subjects, but normal-hearing subjects were tested monaurally and binaurally. PAMR amplitudes declined and latencies increased with decreasing click intensity. Both binaural stimulation and eye turn enhanced the PAMR. In hearing-impaired subjects, PAMR thresholds were correlated with audiometric thresholds for the eyes-turned condition. All normal-hearing subjects had PAMR when recording conditions were optimized and half had responses for the least optimal condition (20 dB nHL, monaural, eyes front). With eyes turned and monaural clicks at 35 dB nHL, the level widely used for infant hearing screening, most normal-hearing adults had a PAMR. Thus the PAMR is a robust response that may be a useful adjunct to ABR for objective hearing assessment.6 page(s

    A novel solution to mitigate pipeline walking, namely the use of pipe-clamping mattresses (PCMs), was first developed for the Malampaya project (Frankenmolen et al. 2017). Comprising a hinged concrete structure designed to clamp onto a section of pipeline, and supporting (post-installed) ballast weight that is transferred directly to the pipeline, PCMs are considered a highly efficient alternative to more traditional solutions such as rock dump or concrete mattress. The original PCM geotechnical design was based primarily on analyses extrapolated from pipeline-seabed interaction, supported by a standard suite of classification and interface tests. Physical model testing was not performed. While observations (taken since installation) show that the PCMs have effectively mitigated pipeline walking in the seabed conditions at Malampaya, their performance in other soil types has not been investigated. To provide further evidence on the effectiveness of PCMs, and investigate their performance over time, a series of centrifuge tests were performed in a soil sample representative of deep-water Gulf of Mexico conditions. In each test, a model representing the PCM was installed on the pipeline section, which was then subjected to cyclic axial displacement. Settlement of the pipeline-PCM system, as well as changes in axial resistance, were directly measured and are reported in this paper.<br/

    Nuisance levels of noise effects radiologists' performance

    This study aimed to measure the sound levels in Irish x-ray departments. The study then established whether these levels of noise have an impact on radiologists performance Noise levels were recorded 10 times within each of 14 environments in 4 hospitals, 11 of which were locations where radiologic images are judged. Thirty chest images were then presented to 26 senior radiologists, who were asked to detect up to three nodular lesions within 30 posteroanterior chest x-ray images in the absence and presence of noise at amplitude demonstrated in the clinical environment. The results demonstrated that noise amplitudes rarely exceeded that encountered with normal conversation with the maximum mean value for an image-viewing environment being 56.1 dB. This level of noise had no impact on the ability of radiologists to identify chest lesions with figure of merits of 0.68, 0.69, and 0.68 with noise and 0.65, 0.68, and 0.67 without noise for chest radiologists, non-chest radiologists, and all radiologists, respectively. the difference in their performance using the DBM MRMC method was significantly better with noise than in the absence of noise at the 90% confidence interval (p=0.077). Further studies are required to establish whether other aspects of diagnosis are impaired such as recall and attention and the effects of more unexpected noise on performance
