    Self-expandable metal stents for left sided colon obstruction from diverticulitis. A single center retrospective series

    Background and Objectives: The incidence of diverticulitis is increasing in western countries. Complicated diverticulitis is defined as diverticulitis associated with localized or generalized perforation, localized or distant abscess, fistula, stricture or obstruction. Colonic symptomatic strictures are often treated with segmental colectomy. The aim of our study is to report our experience with Self Expandable Metal Stents (SEMS) placement to relieve sigmoid obstruction secondary to diverticulitis, either as a permanent solution or as a bridge to elective colectomy. Material and Methods: From January 2016 to December 2018, 21 patients underwent SEMS placement for sigmoid obstruction secondary to diverticulitis at our institution. In four patients with poor general conditions, SEMS was considered the definitive form of treatment. In 17 patients, the stent was placed as bridge to elective colectomy. Data were prospectively collected and retrospectively analyzed. Primary outcomes were postoperative mortality and morbidity after SEMS and subsequent elective colectomy. Results: There was no mortality or major morbidity after SEMS placement or subsequent elective colectomy. No stoma was performed. Conclusions: Placement of Colorectal Self Expandable Stent represents a useful tool to relieve obstruction in patients with left-sided colonic diverticulitis. SEMS placement makes it possible to transform an emergency clinical condition into an elective condition, giving time to resolve the inflammation and the infection inevitably associated with complicated diverticulitis

    Riportiamo un caso di tumore composto unicamente da cellule di Leydig derivate da cellule stromali ovariche e non da cellule di Leydig dell’ilo preesistenti, causa di disordine sessuale e irsutismo in menopausa. Il tumore stromale ovarico composto esclusivamente di cellule stromali di Leydig è raro, < 0,4% tra tutti i tumori ovarici. Può verificarsi anche in donne giovani, ma quasi sempre, si verifica in menopausa o in postmenopausa. Riportiamo un caso di tumore ovarico delle cellule di Leydig nel midollo lontano dall’ilo, derivato da cellule stromali ovariche piuttosto che da cellule di Leydig dell’ilo preesistenti, con sintomi clinici complessi e non comuni. In questa relazione, sottolineiamo a) l’importanza dell’anamnesi, dell’osservazione e della considerazione di tutti i sintomi clinici, b) il valore dell’ecografia transvaginale per la diagnosi, e c) il ruolo del psicologo nel sostenere la paziente interessata nel suo disagio esistenziale e sociale.Haemangiopericytoma (HPC) is a rare vascular tumor comprising 1% of all vascular neoplasms and was first described by Stout and Murray in 1942. They are highly vascularized tumours located in any part of the body. Malignant HPCs represent <1% of all vascular tumours and around 5% of all sarcomatous tumours. The majority has a relatively indolent behaviour with presenting symptoms being vague for several months and not specific. Surgical excision is the mainstay of treatment. We present the case of a 65-year old male with HPC of left infraclavicular region with no associated lymphadenopathy. Surgical management included en bloc excision. The patient did not require any adjuvant therapy and showed no signs of recurrence at 1-year follow up. KEY WORDS: Haemangiopericytoma, Extracranial, Surgical excision, Vascular tumor

    The importance of symptom duration in relation to prognosis of carcinoma of the large intestine

    Between 1971 and 1982, 162 patients underwent surgical treatment for carcinoma of the large intestine; follow-up data was available for 94 per cent of the patients. Tumor stage distribution was homogeneous and did not differ significantly with increasing duration of symptoms. The duration of symptoms was not determinant of survival, whereas stage influenced survival. These results show that patients with carcinoma of the large intestine with a short symptomatic period do not have either less advanced tumors or better survival prospects
