    Molecular diagnosis of severe acute respiratory syndrome.

    The etiologic agent of severe acute respiratory syndrome (SARS) has been identified as a new type of coronavirus, known as SARS-coronavirus (SARS-CoV). Although the SARS epidemic has subsided, many authorities, including the World Health Organization (WHO) and the Centers for Disease Control and Prevention (CDC), have warned of the possible re-emergence of this highly infectious disease. Although antibody-based diagnosis of SARS has been demonstrated to be a reliable proof of SARS infection, it is not sensitive enough for detection during the early phase of the disease. To date, based on the publicly released full genomic sequences of SARS-CoV, various molecular detection methods based on reverse-transcription polymerase chain reaction (RT-PCR) have been developed. Although most of the assays have initially been focused on RNA extracted from nasopharyngeal aspirates, urine, and stools, several of the more recently developed assays have been based on the analysis of RNA extracted from plasma and serum. Such assays allow the more standardized quantitative expression of viral loads and are potentially useful for early SARS diagnosis. In this chapter, two real-time quantitative RT-PCR systems for the quantification of SARS-CoV RNA in serum are discussed. The two RT-PCR systems, one aimed toward the nucleocapsid region and the other toward the polymerase region of the virus genome, have a detection rate of up to 80% during the first week of illness. These quantitative systems are potentially useful for the early diagnosis of SARS and can also provide viral load information that might assist clinicians in making a prognostic evaluation of an infected individual.postprin

    Direct Virtual Memory Access from FPGA for High-Productivity Heterogeneous Computing

    Heterogeneous computing utilizing both CPU and FPGA requires access to data in the main memory from both devices. While a typical system relies on software executing on the CPU to orchestrate all data movements between the FPGA and the main memory, our demo presents a complementary FPGA-centric approach that allows gateware to directly access the virtual memory space as part of the executing process without involving the CPU. A caching address translation buffer was implemented alongside the user FPGA gateware to provide runtime mapping between virtual and physical memory addresses. The system was implemented on a commercial off-the-shelf FPGA add-on card to demonstrate the viability of such approach in low-cost systems. Experiment demonstrated reasonable performance improvement when compared to a typical software-centric implementation; while the number of context switches between FPGA and CPU in both kernel and user mode was significantly reduced, freeing the CPU for other concurrent user tasks.     Dishevelled-3 phosphorylation is governed by HIPK2/PP1CĪ±/ITCH axis and the non-phosphorylated form promotes cancer stemness via LGR5 in hepatocellular carcinoma

    Dishevelled-3 (Dvl3) is regarded as a binding hub with many different interacting partners. However, its regulation and mechanism on cancer stemness remain to be explored. In this study, we showed that Dvl3 was significantly overexpressed in human hepatocellular carcinomas (HCCs) and promoted cancer stemness both in vitro and in vivo. We found that the non-phosphorylated (NP)-Dvl3 was more stable than the phosphorylated form, more active in activating Ī²-catenin transcriptional activity, and more potent in enhancing self-renewal ability in HCC cells. Mechanistically, we confirmed that the homeodomain-interacting protein kinase-2 (HIPK2) and E3 ubiquitin ligase ITCH were able to physically bind to Dvl3 protein. Knockdown of HIPK2 and the protein phosphatase regulatory unit C-alpha (PP1CĪ±) resulted in sustained Dvl3 phosphorylation and hence decrease in the NP form of Dvl3. On the other hand, knockdown of E3 ubiquitin ligase ITCH reduced the phosphorylation-induced degradation and stabilized the phosphorylated Dvl3 protein. Furthermore, the NP-Dvl3 enhanced the LGR5 promoter activity to upregulate LGR5 expression, which was associated with increased cancer stemness in HCC. Our findings established that HIPK2/PP1CĪ±/ITCH axis sustains the de-phosphorylation of Dvl3. This post-translational modification of Dvl3 in turn maintains LGR5 expression and enhances the cancer stemness properties in HCC.published_or_final_versio

    Aim of the study: The anti-osteoporotic activity of Erxian Decoction, a Chinese medicinal formula, in a rat model of menopausal osteoporosis was evaluated by microcomputed tomography (microCT). Materials and methods: Menopause causes a decline in both endocrine function and bone mineral density in human. In this study, 20-month-old female Sprague-Dawley-rats (SD-rats) with a low serum estradiol level and bone mineral density were employed. The anti-osteoporotic activity of EXD was assessed by the determination of trabecular material bone mineral density at the L2 mid-vertebral body after treatment. Serum estrogen levels were also determined to assess the effect of EXD on the endocrine status. Results: Results revealed a significant elevation in serum estradiol level and trabecular bone mineral density at the L2 mid-vertebral body in the EXD-treated menopausal rat model. Conclusions: The results obtained from the present investigation revealed that the EXD had anti-osteoporotic activity as evidenced by an increase of serum estradiol level and bone mineral density.     Validation of the English version constitution in Chinese Medicine Questionnaire (CCMQ) among young adults in Malaysia

    Constitution in Chinese Medicine Questionnaire (CCMQ) is widely used as a self-administered health instrument. It was used as the guiding tool for prognosis, prevention and treatment of various diseases. This study aims to validate an English version of the Constitution in Chinese Medicine Questionnaire (CCMQ) in Malaysia, which is comprised of nine domains with a total of 67 questions. The survey was conducted in three phases. Phase one involved translation and adaption where the Chinese version of the survey instrument was translated into the English version. In phase two, a Delphi was used to review, amend and validate the translated English version; and in phase three, 114 subjects (aged 18 to 30 years old) were included in the confirmatory factor analysis to determine its psychometric properties. The construct validity analysis of CCMQ showed a good fit for the hypothesised models. Composite reliability (CR) values ranging from 0.869 to 0.908, and average variance extracted (AVE) values ranged from 0.647 to 0.764, indicating good internal consistency reliability and convergent validity, respectively. The heterotrait-monotrait ratio of correlations (HTMT) values obtained for all the domains ranging from 0.151 to 0.652, which fulfilled the criteria of discriminant validity. The English version of the CCMQ was the first statistically validated instrument in Malaysia. It is a reliable and valid instrument for traditional Chinese medicine body constitution (TCMBC) study. Future research is needed to validate the CCMQ as a predicting tool in health-related outcome studies

    The College of Medicine in the Republic of Malawi: towards sustainable staff development

    BACKGROUND: Malawi has a critical human resources problem particularly in the health sector. There is a severe shortage of doctors; there are only few medical specialists. The College of Medicine (COM) is the only medical school and was founded in 1991. For senior staff it heavily depends on expatriates. In 2004 the COM started its own postgraduate training programme (Master of Medicine) in the clinical specialties. METHODS: We explore to what extent a brain drain took place among the COM graduates by investigating their professional development and geographical distribution. Using current experience with the postgraduate programme, we estimate at what point all senior academic positions in the clinical departments could be filled by Malawians. We demonstrate the need for expatriate staff for its most senior academic positions in the interim period and how this can be phased out. Lastly we reflect on measures that may influence the retention of Malawian doctors. RESULTS: Since the start of the COM 254 students have graduated with an average of 17 students per year. Most (60%) are working in Malawi. Of those working abroad, 60% are in various postgraduate training programmes. In 2015, adequate numbers of Malawi senior academics should be available to fill most senior positions in the clinical departments, taking into account a 65% increase in staff to cope with increasing numbers of students. CONCLUSION: There seems to be no significant brain drain among graduates of the COM. The postgraduate programme is in place to train graduates to become senior academic staff. In the interim, the COM depends heavily upon expatriate input for its most senior academic positions. This will be necessary at least until 2015 when sufficient numbers of well trained and experienced Malawian specialists may be expected to be available. Improved pay structure and career development perspectives will be essential to consolidate the trend that most doctors will remain in the country

    Kawasaki disease in Hong Kong, 1994 to 2000

    OBJECTIVE. To describe the epidemiology, clinical characteristics, and management of Kawasaki disease in children in Hong Kong. DESIGN. Retrospective survey of medical records from July 1994 to June 1997, and prospective data collection from July 1997 to June 2000. SETTING. Hospitals with a paediatric unit in Hong Kong. PATIENTS. Patients diagnosed with Kawasaki disease between July 1994 and June 2000 in public hospitals in Hong Kong. MAIN OUTCOME MEASURES. Incidence of Kawasaki disease and coronary artery aneurysm rates. RESULTS. A total of 696 cases of Kawasaki disease were reported. There were 435 (62.5%) boys and 261 (37.5%) girls giving a male to female ratio of 1.7:1. The age ranged from 1 month to 15 years 5 months with a median of 1.7 years. Infants (<1 year) constituted the largest group of patients (223,32.0%) and overall, 638 (91.7%) were younger than 5 years. Skin rash, conjunctivitis, and oral signs were among the principal clinical features present in over 80% of cases. Prominent cervical lymph nodes larger than 1.5 cm were less commonly found (24%). Coronary artery aneurysms or ectasia were present in 15.7% (109/696), 8.5% (59/696), and 5.0% (35/696) of patients at 2, 4, and 8 weeks, respectively. The incidence of Kawasaki disease per 100 000 children under 5 years was significantly higher in the prospective study period than in the retrospective period (39 vs 26, <0.001). CONCLUSION. The incidence of Kawasaki disease is high in Hong Kong and is 39 per 100 000 children below 5 years of age. The coronary artery aneurysm prevalence is 5%. Intravenous gamma-globulin and high-dose aspirin is the mainstay of treatment.published_or_final_versio
