Knowledge, attitudes and practices of medical staff towards obesity management in patients with spinal cord injuries: an International survey of four western European countries
Objective: To (1) examine the opinions of medical staff working in spinal cord injury (SCI) centres (SCICs); (2) evaluate their knowledge, attitudes and practices towards obesity prevention and management; (3) report the number of beds and dietitians available at each SCIC. Methods: A 37-item questionnaire was sent to 23 SCICs in the UK, the Netherlands, Belgium and the Republic of Ireland between September 2012 and January 2013. Results: Eighteen SCICs returned the questionnaires for analysis. All respondents stated that they had an interest in obesity treatment but only 2.3% of the respondents received training in obesity management. Sixty-one percent of staff did not consider body mass index (BMI) to be appropriate for use in SCI patients and subsequently less than half of the respondents use BMI routinely. The majority of respondents reported that they are confident in dealing with overweight (74.5%) and obese (66.1%) SCI adults, less than half (44.1%) are confident in treating overweight and obese SCI children. Respondents also indicated the need for nationally adopted guidelines and a lack of physical activity provision. There were 17.5 whole-time equivalent (WTE) dietitians recorded in 22 SCICs, equivalent to 47.8 beds per WTE dietitians (range 10–420). Non-UK SCIC dietitians are significantly better resourced than in UK SCICs (beds per WTE dietitian: 36 vs 124, P=0.035). Conclusion: Medical staff expressed the need to participate in obesity prevention and management. Appropriate training should be considered for all medical staff and the development of specific weight management guidelines and dietetic provision should be considered

Understanding on the reaction of various air pollutants until today continues to grow, even it is hard to find information about the results of the reaction of various air pollutants standard. The aim of the research was to analyze and to correct of the the various air pollutants as well as to determine the health impact of blacksmith in 2014. The samples consisted of 38 blacksmith from 38 working units in Hulu Sungai Selatan of South Kalimantan. Analytical approach of the examination of air pollutants, blood samples and pulmonary functions of selected workers was applied in this study. The results showed a decrease in lung function and abnormalization workers immune response, as a result of exposure to various air pollutants. It is very difficult to determine and predict the causality of air pollution and health impact since there must be various factors contributing to the the health impact. because it is caused by pollutants singly or may be caused from the various reactions of these pollutants, for that, the correction need on the measurement and analysis of air pollutants that made so far, including its impact on the human body. The benefit of this research as a form of correction to use the Threshold Limit Value (TLV) and measurement of air pollutants, including the impact of the target organ in the human body
Status and Powering Test Results of HTS Undulator Coils at 77 K for Compact FEL Designs
The production of low emittance positron beams for future linear and circular lepton colliders, like CLIC or FCC-ee, requires high-field damping wigglers. Just as compact free-electron lasers (FELs) require high-field but as well short-period undulators to emit high energetic, coherent photons. Using high-temperature superconductors (HTS) in the form of coated ReBCO tape superconductors allows higher magnetic field amplitudes at 4 K and larger operating margins as compared to low-temperature superconductors, like Nb-Ti. This contribution discusses the development work on superconducting vertical racetrack (VR) undulator coils, wound from coated ReBCO tape superconductors. The presented VR coils were modularly designed with a period length of 13 mm. Powering tests in liquid nitrogen of multiple vertical racetrack coils were performed at CERN. The results from the measurements are presented for three VR coils and compared with electromagnetic simulations
Health promotion, disease prevention and periodic health checks: perceptions and practice among family physicians in eastern Mediterranean region
Introduction: The aim of this study was to identify the current practices and perceptions of family physicians regarding health promotion, disease prevention including periodic screening and health checks in Eastern Mediterranean Region. Methods: A multi-country cross-sectional study was conducted in six countries of EMR, from September 2014 to March 2015. Family Physicians who were currently practicing in different countries of EMR were invited to participate in the study through email. A pre-tested structured questionnaire was used for data collection. Data was entered and analyzed on SPSS 19 and logistic regression analysis was performed. Results: A total of 100 physicians data was included in the final analysis. The majority were female physicians (76%): 63% were 25 to 35 years of age. Approximately 53% of Family physicians always recommend periodic screening and health checks to their patients. The common screening question asked to patients in medical history was related to their blood pressure (86%). Almost all (99%) of the Family physicians believe they should conduct periodic health checks. Those who had postgraduate training in Family Medicine (OR: 0.5; 95% CI: 0.39-1.67) and attended CME sessions regularly (OR: 0.11; 95% CI: 0.01-0.93), are more likely to recommend periodic screening and health checks to their patients. Conclusion: Periodic screening and health check is an important strategy to prevent disease and maintain health. It is an underutilized practice and a great need exists for its implementation in family practice
Comparative genomics of Enterococcus faecalis from healthy Norwegian infants
<p>Abstract</p> <p>Background</p> <p><it>Enterococcus faecalis</it>, traditionally considered a harmless commensal of the intestinal tract, is now ranked among the leading causes of nosocomial infections. In an attempt to gain insight into the genetic make-up of commensal <it>E. faecalis</it>, we have studied genomic variation in a collection of community-derived <it>E. faecalis </it>isolated from the feces of Norwegian infants.</p> <p>Results</p> <p>The <it>E. faecalis </it>isolates were first sequence typed by multilocus sequence typing (MLST) and characterized with respect to antibiotic resistance and properties associated with virulence. A subset of the isolates was compared to the vancomycin resistant strain <it>E. faecalis </it>V583 (V583) by whole genome microarray comparison (comparative genomic hybridization (CGH)). Several of the putative enterococcal virulence factors were found to be highly prevalent among the commensal baby isolates. The genomic variation as observed by CGH was less between isolates displaying the same MLST sequence type than between isolates belonging to different evolutionary lineages.</p> <p>Conclusion</p> <p>The variations in gene content observed among the investigated commensal <it>E. faecalis </it>is comparable to the genetic variation previously reported among strains of various origins thought to be representative of the major <it>E. faecalis </it>lineages. Previous MLST analysis of <it>E. faecalis </it>have identified so-called high-risk enterococcal clonal complexes (HiRECC), defined as genetically distinct subpopulations, epidemiologically associated with enterococcal infections. The observed correlation between CGH and MLST presented here, may offer a method for the identification of lineage-specific genes, and may therefore add clues on how to distinguish pathogenic from commensal <it>E. faecalis</it>. In this work, information on the core genome of <it>E. faecalis </it>is also substantially extended.</p
Long survival of primary diffuse leptomeningeal gliomatosis following radiotherapy and temozolomide: case report and literature review
<p>Abstract</p> <p>Objective</p> <p>Primary diffuse leptomeningeal gliomatosis (PDLG) is a rare neoplasm with a short survival time of a few months. there is currently no standardized therapeutic approach for PDLG.</p> <p>Materials and methods</p> <p>We report on a 53-year-old male patient who presented with epileptic seizures, gait disturbance, paraparesis and sensory deficits in the dermatomes T8-10.</p> <p>Results</p> <p>Magnetic resonance imaging (MRI) revealing numerous spinal and cranial gadolinium-enhancing nodules in the meninges and histopathology led us to diagnose primary diffuse leptomeningeal gliomatosis with WHO grade III astrocytic cells. Consecutively, the patient underwent craniospinal radiotherapy (30 Gy) and 11 sequential cycles of temozolomide. This regimen led to partial tumor regression. Thirteen months later, spinal MRI revealed tumor progression. Second-line chemotherapy with 5 cycles of irinotecan and bevacizumab did not prevent further clinical deterioration. The patient died twenty-two months after diagnosis, being the longest survival time described thus far with respect to PDLG consisting of astrocytic tumor cells.</p> <p>Conclusions</p> <p>Radiochemotherapy including temozolomide, as established standard therapy for brain malignant astrocytomas, might be valid as a basic therapeutic strategy for this PDLG subtype.</p
