    Knowledge and practices of the nursing professional in the care of the burned patient

    RESUMEN: Introducción: Las quemaduras son un importante y creciente motivo de consulta y hospitalización, que causa en muchos pacientes grados variables de incapacidad y muerte. Además, es una situación traumática desde el punto de vista físico y emocional que tiene en la enfermera un partícipe en el cuidado de este tipo de pacientes. En el contexto colombiano se encuentran pocas investigaciones, en parte por las escasas unidades existentes, lo que motiva a indagar sobre los saberes y prácticas del profesional de enfermería en el cuidado. Objetivo: Comprender los saberes y prácticas del profesional de enfermería en el cuidado del paciente quemado. Metodología: Investigación cualitativa tipo estudio de casos. Participaron 10 enfermeras de las unidades de quemados del Hospital Universitario San Vicente Fundación HUSVF y 3 enfermeras de dos hospitales de IV nivel de complejidad del país, a quienes se les realizaron entrevistas semiestructuradas. Por otra parte, durante 71 horas de observación participante en HUSVF, se realizaron 6 entrevistas informales a dos cirujanos plásticos y cuatro auxiliares de enfermería. El análisis de los datos fue inductivo y concomitantemente se realizó triangulación hermenéutica de técnicas y datos dando lugar a la categorización emergente. Resultados: Emergieron tres categorías: trabajar en un contexto diferente, enfermera: eje central; y saberes y prácticas aprendidas. Conclusiones: El saber del cuidado del paciente quemado se obtiene a través de la experiencia y se perfecciona con el tiempo y la práctica. Además, se considera al profesional de enfermería como el eje central de la unidad de quemados tanto para el paciente, la familia y el equipo multidisciplinar. Being burned is a traumatic situation that a person can live physically and emotionally and the nurse participates in the care of this type of patients. Few investigations are found in the Colombian context, partly because of the few existent units, which motivates to inquire about the knowledge and practices of the nursing professional in this context of care. Objective: To understand the knowledge and practices of the nursing professional in the care of burned patients. Methodology: qualitative study type case study. Ten nurses from the burn units of the Hospital Universitario San Vicente Fundacion HUSVF and 3 nurses from two hospitals of the IV level of complexity of the country participated, to whom semi-structured interviews were conducted; In addition, during 71 hours of participant observation in HUSVF, 6 informal interviews were conducted with two plastic surgeons and four nursing assistants. The analysis of the data was inductive and concomitantly hermeneutical triangulation of techniques and data leading to the emerging categorization. Results: 3 categories emerged: work in a different context, nurse: central axis; and knowledge and practices learned. Conclusions: The knowledge of burned patient care is obtained through experience, it is perfected with time and practice. In addition, the nursing professional is considered as the central axis of the burn unit for both the patient, the family and the multidisciplinary team.     Search for dark matter produced in association with bottom or top quarks in √s = 13 TeV pp collisions with the ATLAS detector

    A search for weakly interacting massive particle dark matter produced in association with bottom or top quarks is presented. Final states containing third-generation quarks and miss- ing transverse momentum are considered. The analysis uses 36.1 fb−1 of proton–proton collision data recorded by the ATLAS experiment at √s = 13 TeV in 2015 and 2016. No significant excess of events above the estimated backgrounds is observed. The results are in- terpreted in the framework of simplified models of spin-0 dark-matter mediators. For colour- neutral spin-0 mediators produced in association with top quarks and decaying into a pair of dark-matter particles, mediator masses below 50 GeV are excluded assuming a dark-matter candidate mass of 1 GeV and unitary couplings. For scalar and pseudoscalar mediators produced in association with bottom quarks, the search sets limits on the production cross- section of 300 times the predicted rate for mediators with masses between 10 and 50 GeV and assuming a dark-matter mass of 1 GeV and unitary coupling. Constraints on colour- charged scalar simplified models are also presented. Assuming a dark-matter particle mass of 35 GeV, mediator particles with mass below 1.1 TeV are excluded for couplings yielding a dark-matter relic density consistent with measurements

    Immunoglobulin, glucocorticoid, or combination therapy for multisystem inflammatory syndrome in children: a propensity-weighted cohort study.

    BACKGROUND: Multisystem inflammatory syndrome in children (MIS-C), a hyperinflammatory condition associated with SARS-CoV-2 infection, has emerged as a serious illness in children worldwide. Immunoglobulin or glucocorticoids, or both, are currently recommended treatments. METHODS: The Best Available Treatment Study evaluated immunomodulatory treatments for MIS-C in an international observational cohort. Analysis of the first 614 patients was previously reported. In this propensity-weighted cohort study, clinical and outcome data from children with suspected or proven MIS-C were collected onto a web-based Research Electronic Data Capture database. After excluding neonates and incomplete or duplicate records, inverse probability weighting was used to compare primary treatments with intravenous immunoglobulin, intravenous immunoglobulin plus glucocorticoids, or glucocorticoids alone, using intravenous immunoglobulin as the reference treatment. Primary outcomes were a composite of inotropic or ventilator support from the second day after treatment initiation, or death, and time to improvement on an ordinal clinical severity scale. Secondary outcomes included treatment escalation, clinical deterioration, fever, and coronary artery aneurysm occurrence and resolution. This study is registered with the ISRCTN registry, ISRCTN69546370. FINDINGS: We enrolled 2101 children (aged 0 months to 19 years) with clinically diagnosed MIS-C from 39 countries between June 14, 2020, and April 25, 2022, and, following exclusions, 2009 patients were included for analysis (median age 8·0 years [IQR 4·2-11·4], 1191 [59·3%] male and 818 [40·7%] female, and 825 [41·1%] White). 680 (33·8%) patients received primary treatment with intravenous immunoglobulin, 698 (34·7%) with intravenous immunoglobulin plus glucocorticoids, 487 (24·2%) with glucocorticoids alone; 59 (2·9%) patients received other combinations, including biologicals, and 85 (4·2%) patients received no immunomodulators. There were no significant differences between treatments for primary outcomes for the 1586 patients with complete baseline and outcome data that were considered for primary analysis. Adjusted odds ratios for ventilation, inotropic support, or death were 1·09 (95% CI 0·75-1·58; corrected p value=1·00) for intravenous immunoglobulin plus glucocorticoids and 0·93 (0·58-1·47; corrected p value=1·00) for glucocorticoids alone, versus intravenous immunoglobulin alone. Adjusted average hazard ratios for time to improvement were 1·04 (95% CI 0·91-1·20; corrected p value=1·00) for intravenous immunoglobulin plus glucocorticoids, and 0·84 (0·70-1·00; corrected p value=0·22) for glucocorticoids alone, versus intravenous immunoglobulin alone. Treatment escalation was less frequent for intravenous immunoglobulin plus glucocorticoids (OR 0·15 [95% CI 0·11-0·20]; p<0·0001) and glucocorticoids alone (0·68 [0·50-0·93]; p=0·014) versus intravenous immunoglobulin alone. Persistent fever (from day 2 onward) was less common with intravenous immunoglobulin plus glucocorticoids compared with either intravenous immunoglobulin alone (OR 0·50 [95% CI 0·38-0·67]; p<0·0001) or glucocorticoids alone (0·63 [0·45-0·88]; p=0·0058). Coronary artery aneurysm occurrence and resolution did not differ significantly between treatment groups. INTERPRETATION: Recovery rates, including occurrence and resolution of coronary artery aneurysms, were similar for primary treatment with intravenous immunoglobulin when compared to glucocorticoids or intravenous immunoglobulin plus glucocorticoids. Initial treatment with glucocorticoids appears to be a safe alternative to immunoglobulin or combined therapy, and might be advantageous in view of the cost and limited availability of intravenous immunoglobulin in many countries.     Global economic burden of unmet surgical need for appendicitis

    Background There is a substantial gap in provision of adequate surgical care in many low- and middle-income countries. This study aimed to identify the economic burden of unmet surgical need for the common condition of appendicitis. Methods Data on the incidence of appendicitis from 170 countries and two different approaches were used to estimate numbers of patients who do not receive surgery: as a fixed proportion of the total unmet surgical need per country (approach 1); and based on country income status (approach 2). Indirect costs with current levels of access and local quality, and those if quality were at the standards of high-income countries, were estimated. A human capital approach was applied, focusing on the economic burden resulting from premature death and absenteeism. Results Excess mortality was 4185 per 100 000 cases of appendicitis using approach 1 and 3448 per 100 000 using approach 2. The economic burden of continuing current levels of access and local quality was US 92492millionusingapproach1and92 492 million using approach 1 and 73 141 million using approach 2. The economic burden of not providing surgical care to the standards of high-income countries was 95004millionusingapproach1and95 004 million using approach 1 and 75 666 million using approach 2. The largest share of these costs resulted from premature death (97.7 per cent) and lack of access (97.0 per cent) in contrast to lack of quality. Conclusion For a comparatively non-complex emergency condition such as appendicitis, increasing access to care should be prioritized. Although improving quality of care should not be neglected, increasing provision of care at current standards could reduce societal costs substantially

    Search for direct top squark pair production in final states with two leptons in s=13\sqrt{s} = 13 TeV pppp collisions with the ATLAS detector

    International audienceThe results of a search for direct pair production of top squarks in events with two opposite-charge leptons (electrons or muons) are reported, using 36.1 fb136.1~\hbox {fb}^{-1} of integrated luminosity from proton–proton collisions at s=13\sqrt{s}=13 TeV collected by the ATLAS detector at the Large Hadron Collider. To cover a range of mass differences between the top squark t~\tilde{t} and lighter supersymmetric particles, four possible decay modes of the top squark are targeted with dedicated selections: the decay t~bχ~1±\tilde{t} \rightarrow b \tilde{\chi }_{1}^{\pm } into a b-quark and the lightest chargino with χ~1±Wχ~10\tilde{\chi }_{1}^{\pm } \rightarrow W \tilde{\chi }_{1}^{0} , the decay t~tχ~10\tilde{t} \rightarrow t \tilde{\chi }_{1}^{0} into an on-shell top quark and the lightest neutralino, the three-body decay t~bWχ~10\tilde{t} \rightarrow b W \tilde{\chi }_{1}^{0} and the four-body decay t~bνχ~10\tilde{t} \rightarrow b \ell \nu \tilde{\chi }_{1}^{0} . No significant excess of events is observed above the Standard Model background for any selection, and limits on top squarks are set as a function of the t~\tilde{t} and χ~10\tilde{\chi }_{1}^{0} masses. The results exclude at 95% confidence level t~\tilde{t} masses up to about 720 GeV, extending the exclusion region of supersymmetric parameter space covered by previous searches

    Measurements are made of differential cross-sections of highly boosted pair-produced top quarks as a function of top-quark and ttˉt\bar{t} system kinematic observables using proton--proton collisions at a center-of-mass energy of s=13\sqrt{s} = 13 TeV. The data set corresponds to an integrated luminosity of 36.136.1 fb1^{-1}, recorded in 2015 and 2016 with the ATLAS detector at the CERN Large Hadron Collider. Events with two large-radius jets in the final state, one with transverse momentum pT>500p_{\rm T} > 500 GeV and a second with pT>350p_{\rm T}>350 GeV, are used for the measurement. The top-quark candidates are separated from the multijet background using jet substructure information and association with a bb-tagged jet. The measured spectra are corrected for detector effects to a particle-level fiducial phase space and a parton-level limited phase space, and are compared to several Monte Carlo simulations by means of calculated χ2\chi^2 values. The cross-section for ttˉt\bar{t} production in the fiducial phase-space region is 292±7 (stat)±76(syst)292 \pm 7 \ \rm{(stat)} \pm 76 \rm{(syst)} fb, to be compared to the theoretical prediction of 384±36384 \pm 36 fb
