    Effectiveness of Childbirth Education on Nulliparous Women’s Knowledge of Childbirth Preparation, Pregnancy Anxiety and Pregnancy Outcomes

    Background: The emerging number of cesarean sections among nulliparous women due to high pregnancy-related anxiety is a major concern of maternity care providers. Childbirth preparations enable women to cope with pregnancy anxiety and enhance pregnancy outcomes. Limited studies evaluated the impact of childbirth educational interventions on pregnancy-specific anxiety and pregnancy outcomes. Objectives: The aim of this study was to evaluate the effectiveness of childbirth educational intervention on nulliparous women’s knowledge on childbirth preparation, pregnancy anxiety, and pregnancy outcomes. Methods: A randomized controlled trial approach with a two-group pretest/ posttest design was used among hundred nulliparous third trimester pregnant women. All participants were pretested for their knowledge on childbirth preparation and pregnancy anxiety level using knowledge questionnaire, state trait anxiety inventory, and pregnancy-specific anxiety inventory. The experimental group (n = 50) received three sessions of childbirth education. All participants were post- tested before delivery, and their pregnancy outcomes were noted from labor records. Data were collected from a major maternity hospital in Kerala, India. GLM repeated measures analysis and paired t-test were used for data analysis. Results: The experimental group demonstrated a significantly higher level of knowledge on childbirth preparation (P < 0.001), with high reported mean knowledge scores of (54.30 ± 3.86) childbirth preparation than the control group (31.08 ± 1.96). A lower mean scores of pregnancy-specific anxiety among experimental group (102 ± 4.65) (P < 0.001) compared to control group (139.96 ± 4.9) signifies the relevance of childbirth education in reducing pregnancy-specific anxiety. Significant reductions of caesarean birth (50%) among nulliparous women along with a 12% increase in newborn’s birth weights were the main positive birth outcomes. Conclusions: Childbirth education significantly reduced pregnancy-specific anxiety and the adverse pregnancy outcomes. The emerging number of cesarean sections on maternal request due to childbirth anxiety could be reduced by empowering nulliparous women through childbirth education

    Investigation of nano lipid vesicles of methotrexate for anti-rheumatoid activity

Stealth liposomes of MTX were prepared by thin-film hydration method using a PEGylated phospholipid-like DSPE-MPEG 2000. Similarly, conventional liposomes were prepared using phospholipids like DPPC and DSPC. Conventional liposomes were coated with a hydrophilic biocompatible polymer like chitosan. They were investigated for their physical properties and in vitro release profile. Further, in vivo screening of the formulations for their anti-rheumatoid efficacy was carried out in rats. Rheumatoid arthritis was induced in male Wistar-Lewis rats using complete Freund&amp;rsquo;s adjuvant (1 mg/mL Mycobacterium tuberculosis, heat killed in mineral oil).Results: It was found that chitosan coating of the conventional liposomes increased the physical stability of the liposomal suspension as well as its entrapment efficiency. The size of the unsonicated lipid vesicles was found to be in the range of 8&amp;ndash;10 &amp;micro;m, and the sonicated lipid vesicles in the range of 210&amp;ndash;260 nm, with good polydispersity index. Further, chitosan-coated conventional liposomes and the PEGylated liposomes released the drug for a prolonged period of time, compared to the uncoated conventional liposomes. It was found that there was a significant reduction in edema volume in the rat group administered with the test stealth liposomal formulations and chitosan-coated conventional liposomes (PEGylated and chitosan-coated conventional) compared to that of the control and standard (administered with free MTX) group of rats. PEGylated liposomes showed almost equal efficacy as that of the chitosan-coated conventional liposomes.Conclusion: Lipid nano vesicles of MTX can be administered by intravenous route, whereby the drug selectively reaches the target site with reduced toxicity to other organs.Keywords: methotrexate, stealth liposomes, conventional liposomes, chitosan coating, targeted delivery, anti-rheumatoid efficac

    Relative Biological Effectiveness Studies Using 3 MeV Proton Beam from Folded Tandem Ion Accelerator: An Experimental and Theoretical Approach

    Proton being the easiest light ion to accelerate and achieve desired beam profile, has been pursued as a popular particulate radiation for therapy applications. In the present study, Saccharomyces cerevisiae D7 strain was used to estimate the RBE values of the 3 MeV proton beam, and an attempt was made to derive mathematical formula for calculating RBE value with respect to the dose. Dosimetry studies were carried out using Fricke dosimetry and Semiconductor Surface Barrier detector to calibrate the absorbed doses of Gamma chamber-1200 and Folded Tandem Ion Accelerator respectively. Gold standard cell survival assay and gene conversion assay were used to compare gamma and proton radiation induced cell death and genetic endpoint. Multi target single hit model was used to derive mathematical formula for RBE estimation. The results show a linear survival-dose response after proton radiation and sigmoid survival-dose response after gamma radiation treatment. The calculated RBE value from the survival and gene conversion studies was 1.60 and 3.93, respectively. The derived mathematical formula is very useful in calculating RBE value, which varies from 3.61 to 1.80 with increasing dose. The estimated RBE value from the mathematical formula is comparable with the experimental values. With the help of the present mathematical formulation, RBE value at any dose can be calculated in the exponential and sigmoidal regions of the survival curve without actually extending the experiment in that dose region, which is not possible using conventional methods

    Study on the morphological and biocompatible properties of chitosan grafted silk fibre reinforced PVA films for tissue engineering applications

    The current study delineates the preparation of novel chitosan grafted silk fibre reinforced Poly (vinyl alcohol) (PVA) composite films with desirable properties. Although silk fibroin has been extensively used for various biomedical applications, its properties could be further re-tailored for its suitability in the field of regenerative medicine. Chitosan was successfully grafted over silk, via acylation with succinic anhydride and thereby the fibres were incised and used for the preparation of the films. The grafted silk fibre reinforced PVA films were subjected to FTIR studies, microscopic analysis by atomic force microscopy (AFM) and optical microscopy techniques, X-ray diffraction (XRD) analysis and further evaluated for in vitro biocompatibility studies. The composite films demonstrated improved surface roughness with increasing concentration of the fibre and its dispersion in the polymer matrix was observed. Furthermore, in vitro biocompatibility and cellular behaviour such as adhesion and proliferation of mouse fibroblasts as well as astrocyte cells was studied and the results showed improved proliferative activity, when compared to the pristine PVA films. These results were further supported by the results confirmed by MTT assay demonstrating the films to be non-toxic. The efficiency and feasibility of the films to be used for tissue engineering, was further evaluated by haemocompatibility studies using human erythrocytes, thus making them a potential material to be used for biomedical applications. © 201

    IL-17 is essential for host defense against cutaneous Staphylococcus aureus infection in mice

    Staphylococcus aureus is the most common cause of skin and soft tissue infections, and rapidly emerging antibiotic-resistant strains are creating a serious public health concern. If immune-based therapies are to be an alternative to antibiotics, greater understanding is needed of the protective immune response against S. aureus infection in the skin. Although neutrophil recruitment is required for immunity against S. aureus, a role for T cells has been suggested. Here, we used a mouse model of S. aureus cutaneous infection to investigate the contribution of T cells to host defense. We found that mice deficient in γδ but not αβ T cells had substantially larger skin lesions with higher bacterial counts and impaired neutrophil recruitment compared with WT mice. This neutrophil recruitment was dependent upon epidermal Vγ5+ γδ T cell production of IL-17, but not IL-21 and IL-22. Furthermore, IL-17 induction required IL-1, TLR2, and IL-23 and was critical for host defense, since IL-17R–deficient mice had a phenotype similar to that of γδ T cell–deficient mice. Importantly, γδ T cell–deficient mice inoculated with S. aureus and treated with a single dose of recombinant IL-17 had lesion sizes and bacterial counts resembling those of WT mice, demonstrating that IL-17 could restore the impaired immunity in these mice. Our study defines what we believe to be a novel role for IL-17–producing epidermal γδ T cells in innate immunity against S. aureus cutaneous infection
